BACKGROUND: Lennox-Gastaut syndrome (LGS) is a severe form of drug-resistant epilepsy that begins during childhood and frequently leads to significant neurological impairments. Patients with LGS are likely to receive improper oral nutrition because of issues such as dysphagia and aspiration risk, potentially resulting in long-term tube feeding and eventual gastrostomy tube placement. Therefore, we investigated the effects of gastrostomy tube placement on nutrition outcomes and frequency of hospitalization in LGS.
METHODS: We retrospectively examined 67 patients diagnosed with LGS who had unde

BACKGROUND: Evidence on the effect of small-quantity lipid-based nutrient supplements (SQ-LNSs) on early child growth and development is mixed.
OBJECTIVE: This study assessed the effect of daily consumption of 2 different SQ-LNS formulations on linear growth (primary outcome), psychomotor development, iron status (secondary outcomes), and morbidity in infants from age 6 to 12 mo within the context of a maize-based complementary diet.
METHODS: Infants (n = 750) were randomly assigned to receive SQ-LNS, SQ-LNS-plus, or no supplement. Both SQ-LNS products contained micronutrients and essential

OBJECTIVE: To evaluate the nutritional status of crack users and to analyze its correlation with drug use profiles.
METHODS: Cross-sectional study with 108 crack users. Anthropometric data were assessed through body mass index (BMI) and bioimpedance (BIA) measurements. A blood test to analyze hematocrit, hemoglobin, glucose, and lipid profiles was also performed. Crack use was determined through a standardized interview.
RESULTS: Based on BMI and BIA, most individuals were eutrophic (about 70%). Regarding hematological parameters, we found that hemoglobin and hematocrit levels were below nor

BACKGROUND: Nutrition during infancy may have a long-term impact upon weight gain and eating style. How infants are introduced to solid foods may be important. Traditionally, infants are introduced to solid foods via spoon-feeding of purees. However, baby-led weaning advocates allowing infants to self-feed foods in their whole form. Advocates suggest this may promote healthy eating styles, but evidence is sparse. The aim of the current study was to compare child eating behaviour at 18-24 months between infants weaned using a traditional weaning approach and those weaned using a baby-led weanin

Scurvy is a disease that results from a vitamin C deficient diet. Since vitamin C is available in many food products, and especially in citrus fruits, the disease is rare in developed countries. Clinical manifestations of scurvy include general weakness, cutaneous and gum bleeding, pain in the lower limbs and inability to stand and walk (pseudo paralysis). The diagnosis of scurvy requires a high level of clinical suspicion, typical radiographic features and low Levels of vitamin C in the plasma. We report a case of a 7-year-old patient with a medical history of hydrocephalus, failure to thrive

BACKGROUND: In Malawi and other high HIV prevalence countries, studies suggest that more than 30% of all severely malnourished children admitted to inpatient nutrition rehabilitation units are HIV-infected. However, clinical algorithms designed to diagnose paediatric HIV are neither sensitive nor specific in severely malnourished children. The present study was conducted to assess : i) whether HIV testing can be integrated into Community-based Therapeutic Care (CTC); ii) to determine if CTC can improve the identification of HIV infected children; and iii) to assess the impact of CTC programmes

Early childhood growth retardation persists in developing countries despite decades of nutritional interventions. Adequate food is necessary, but not sufficient, to ensure normal growth where there is ubiquitous exposure to infection. Pathways associated with infection, small intestinal mucosal damage and chronic immunostimulation remain largely undemonstrated in countries other than The Gambia. We conducted a longitudinal study of one squatter and one middle-class group (n 86, 3-18 month olds) to assess these relationships in Nepal. Growth, mucosal damage index (MDI; urinary lactose:creatinin

Stature, sitting height, stature by weight, and head circumference change with varying economic conditions during early childhood. Our hypothesis is that adult head shape, as well as head size, is influenced by changes in childhood nutrition. When economic conditions are bad, nutrition and health suffer, and the result is dolichocephaly. To test this hypothesis, we measured the head length, width, and circumference of 398 adult males in Jordan. Fifty-six percent are ethnic Jordanians, and 44% are ethnic Palestinians. We divided the modern history of Jordan and the West Bank into four periods d

Over the last decades, health indicators have witnessed major improvements in the Sultanate of Oman. This study was aimed at factors associated with underweight among children in four regions of Oman, as, in 1998, underweight was prevalent among 17.9% of children aged less than five years. A case-control study was conducted in 2002: 190 cases were 6-35-month old children with weight-for-age < -2 z-scores. Controls were individually matched by village of residence, sex, and age. The questionnaire included anthropometry of children, child-feeding practices, morbidity, anthropometry of mothers, p

BACKGROUND: Young infants are at risk of vitamin A deficiency. Supplementation of breastfeeding mothers improves the vitamin A status of their infants, but there are no data regarding its effect on infant mortality, and data on the effect of directly supplementing infants during the first few weeks of life are conflicting.
OBJECTIVE: The objective was to measure the effect on infant mortality of supplementing neonates and their HIV-negative mothers with single, large doses of vitamin A during the immediate postpartum period.
DESIGN: A randomized, placebo-controlled, 2-by-2 factorial design t

Evidence that fetal nutrition triggers permanent adjustments in a wide range of systems and health outcomes is stimulating interest in the evolutionary significance of these responses. This review evaluates the postnatal adaptive significance of fetal developmental plasticity from the perspective of life history theory and evolutionary models of energy partitioning. Birthweight is positively related to multiple metabolically costly postnatal functions, suggesting that the fetus has the capacity to distribute the burden of energy insufficiency when faced with a nutritionally challenging environ

BACKGROUND: The purpose of this study was to provide a detailed evaluation of adherence to nutrition supplements by patients with a lower limb fracture.
METHODS: These descriptive data are from 49 nutritionally "at-risk" patients aged 70+ years admitted to the hospital after a fall-related lower limb fracture and allocated to receive supplementation as part of a randomized, controlled trial. Supplementation commenced on day 7 and continued for 42 days. Prescribed volumes aimed to meet 45% of individually estimated theoretical energy requirements to meet the shortfall between literature estima

Inadequate folate intake and in consequence deficient of plasma folate status may have a negative impact on human health. Among elderly the most important effects are related to hyperhomocysteinemia, a significant risk factor for cardiovascular diseases, procarcinogenic effects and cognitive dysfunctions. To prevent such situation and improve the quality of life of elderly people, in Poland as in many other countries, different strategies for increasing folate intake were applied, among them food fortification. At the same time it is important to educate people because food fortification and i

Undernutrition and intestinal parasitic infections affect childhood development and morbidity in many developing countries. Undernutrition may increase susceptibility to parasitic infections which in turn impair the nutritional status of the host. The relationship between intestinal parasitic infections and nutritional status in 400 Mexican schoolchildren was investigated. More than half of the children in the study showed intestinal parasites and polyparasitism. The prevalence of helminth infections was significantly higher in Oaxaca than in Sinaloa (P < 0.05). Z scores for weight-for-age (WA

STATEMENT OF PROBLEM: It is unclear whether the replacement of conventional mandibular dentures with implant-supported overdentures alters the diet and thus improves the nutritional intake of edentulous persons.
PURPOSE: The purpose of this study was to compare the pretreatment and posttreatment diets of edentulous diabetic patients who received new dentures with either a conventional complete mandibular denture (CD) or a mandibular implant-supported overdenture (IOD).
MATERIAL AND METHODS: New dentures were made for 89 edentulous diabetic patients with acceptable metabolic control without i

A constant in the practice of general pediatrics is an emphasis on infant nutrition and monitoring of growth. These topics are the focus of this issue's "Office pediatrics" section. Much of the new research and publication concerning nutrition in childhood in the past year continues to focus on breastfeeding-its short-term and long-term benefits and strategies to increase the number of women who successfully nurse their infants. The documentation of breastfeeding's benefits is becoming increasingly strong, even in developed countries such as the United States. At the same time, the many weakne

Low function (deficiency), within the 'normal range', of each of five immunity functions is associated with immunopathological disease, and/or defective antigen handling. These are probably genetically determined, either polygenic or single gene, but environmental factors such as diet influence them greatly, and the vulnerability may be especially great in the newborn period. The relevant systems are those involved in the immune elimination of antigen (antibody and macrophages) and those possibly involved in the immune exclusion of antigen (IgA, the alternative pathway of complement, and cilia
