The use of trochanteric slide osteotomy in the treatment of displaced acetabular fractures.

From January 2003 and February 2006, 31 displaced acetabular fractures were treated by open reduction and internal fixation. The fractures were managed using a single approach, involving a straight lateral incision centered over the greater trochanter, trochanteric osteotomy and dislocation of the femoral head. The mean age of the patients was 48 (range 20-74 years) with a mean follow up 24 months (range from 20 to 42 months). Ten fractures were classified as simple, and 21 as complex fractures. The mean time to surgery was 4.5 days (range from 0 to 14 days). Mean operating time was 118 min (r

Fixation Strategies for Metacarpal Shaft Fractures: A Comparison of Plate-and-Screw, Intramedullary Screw, and Percutaneous Wire Techniques.

PURPOSE: The optimal fixation for metacarpal shaft fractures remains a topic of debate. This study compares clinical outcomes of three fixation methods-closed reduction with percutaneous Kirschner wire pinning, open reduction internal fixation (ORIF), and intramedullary fixation (IMN)-focusing on time to motion, range of motion (ROM), and complications. We hypothesized that IMN would allow earlier and more reliable return of ROM without increasing complications. METHODS: A retrospective review of 84 patients (104 diaphyseal fractures) treated at a level I trauma center between 2018 and 2024 w

A Challenging Case of Extraordinary - Long standing Forearm Non-union Treated with Extensive Debridement, Locking Plates, and Bone Grafting. Technical Considerations in Multiple-operated, Neglected Non-unions.

INTRODUCTION: Perilunate dislocations are rare high-energy injuries, and the diagnosis is not infrequently missed at initial presentation. The combination of fractures resulting in a trans-styloid, trans-scaphoid, and trans-triquetral perilunate fracture dislocation is extremely rare. Early recognition and diagnosis of these injuries is prudent to restore patient function and prevent morbidity. This injury pattern may progress through several distinct phases often involving the greater or lesser arc. The injury begins with traumatic disruption of the scapholunate joint, followed by an ordered

SUPraclavicular Block for Emergency Reduction of Upper Limb Injuries Versus Bier Block (SUPERB): An Open-Label, Noninferiority Randomised Controlled Trial.

OBJECTIVES: To assess the effectiveness of ultrasound-guided supraclavicular block (UGSCB), performed by emergency physicians, for closed reduction of upper limb fractures or dislocations when compared with Bier block (BB). METHODS: This was an open-label, noninferiority randomised controlled trial. Adults aged ≥ 18 years presenting to an urban district ED with distal radius and/or ulnar fractures requiring emergent reduction were included. Patients were randomised to either UGSCB using 0.75% ropivacaine or BB using 0.5% lignocaine or 0.5% prilocaine, performed by emergency physicians. The pr

[Treatment of patellar fractures with cable through the bone and Kirschner with a hole in the tail].

OBJECTIVE: To study clinical effects of a new internal fixation by using a cable through the bone and Kirschner with a hole in the tail, for the treatment of patellar fractures. METHODS: From May 2012 to July 2013, thirty-four patients with patellar fractures were treated with cable through the bone and Kirschner with a hole in the tail. All the patients had close fracture, including 12 transverse fractures and 22 comminuted fractures. There were 18 males and 16 females, ranging in age from 26 to 81 years old, with an average of (46.0 ± 3.0) years old. After open reduction, two appropriate len

Scapular neck fracture--the influence of permanent malalignment of the glenoid neck on clinical outcome.

A scapular neck fracture is considered unstable if it is associated with an ipsilateral clavicular fracture or an acromioclavicular (AC) joint dislocation. Currently, it is recommended that stabilization of a disrupted shoulder girdle must be achieved through open reduction and internal fixation of the clavicular fracture or by reduction of the AC joint, without addressing the scapular neck. However, if the displaced glenoid neck is not simultaneously reduced, malalignment of the glenoid neck may persist. The purpose of this retrospective study was to analyze the effect of associated shoulder

Prolonged surgical duration, higher body mass index and current smoking increases risk of surgical site infection after intra-articular fracture of distal femur.

BACKGROUND: This study aimed to investigate incidence of surgical site infection (SSI) following the surgery of intra-articular fractures of distal femur. METHODS: Between July 2014 and December 2017, inpatient medical records of consecutive patients who had intra-articular fractures of distal femur treated by open reduction and plate/screw fixation were inquired to identify whether they had a SSI. After discharge, patients who had a SSI and were readmitted for treatment of SSI were also allocated to the case group. Univariate and multivariate logistic regression analyses were performed to de

The effect of ulnar styloid fractures on patient-rated outcomes after volar locking plating of distal radius fractures.

PURPOSE: Ulnar styloid fractures commonly occur with distal radius fractures (DRFs). Ulnar styloid fractures that involve the insertion of the radioulnar ligaments can cause distal radioulnar joint (DRUJ) instability, and the literature suggests that these fractures should be treated with open reduction internal fixation (ORIF). However, in the absence of DRUJ instability, the effects of ulnar styloid fractures are unknown. The purpose of this study is to evaluate the outcome of ulnar styloid fractures without DRUJ instability on patient-rated outcomes after DRF ORIF. METHODS: Between 2003 an

Use and abuse of flexible intramedullary nailing in children and adolescents.

UNLABELLED: Elastic stable intramedullary nailing (ESIN) has became a well-accepted method of osteosynthesis of diaphyseal fractures in children and adolescents for many reasons including the following: no need for postoperative cast, primary bone union with avoidance of growth plate injury, and minimum invasive surgery. OPERATIVE TECHNIQUE: The principle is to introduce 2 elastic nails, titanium or stainless steel, into the medullary canal through a metaphyseal approach. The bended nails must have their maximum of curve at the level of the fracture, and their orientation, most often face to

First carpal-metacarpal joint dislocation and trapezial fracture treated with external fixation in an adolescent.

The intrinsic joint stability of the first carpal-metacarpal joint (CMC) makes dislocation a rare injury with fewer than 40 cases described. The degree to which supporting ligaments have been disrupted is reflected clinically by a spectrum of joint stability. Close review of radiographs and an attentive physical examination are necessary to make the diagnosis. Acute treatment has consisted of closed or open reduction with K-wires for stabilization or casting with failures leading to chronic joint instability and the need for soft tissue tendon suspension to maintain joint alignment. We present

Far Posterior Approach for Rib Fracture Fixation: Surgical Technique and Tips.

BACKGROUND: The present video article describes the far posterior or paraspinal approach to posterior rib fractures. This approach is utilized to optimize visualization intraoperatively in cases of far-posterior rib fractures. This technique is also muscle-sparing, and muscle-sparing posterolateral, axillary, and anterior approaches have been shown to return up to 95% of periscapular strength by 6 months postoperatively1. DESCRIPTION: Like most fractures, the skin incision depends on the fracture position. The vertical incision is made either just medial to a line equidistant between the palp

A Functional Outcome on Different Surgical Treatment Modalities of Bimalleolar Fractures in Adults.

OBJECTIVE: This study aimed to evaluate and compare the functional outcomes of adult patients with displaced closed bimalleolar ankle fractures treated with open reduction and internal fixation (ORIF) using different fixation modalities. The choice of implant and fixation technique was determined by fracture morphology, bone quality, and intraoperative stability assessment rather than age-specific criteria. The study further assessed whether functional outcomes differed across these modality-based treatment approaches. METHODOLOGY: A prospective observational study was conducted between Septe

Locked Plate Fixation of Proximal Humerus Fractures in Non-elderly Adults: Analysis of Short-Term Outcomes.

Introduction  Proximal humerus fractures (PHFs) represent the third most common adult fracture, increasingly affecting women and middle-aged individuals. While older patients typically sustain PHFs via low-energy trauma, younger populations more frequently incur these injuries through high-energy mechanisms. Surgical decision-making is influenced by patient-specific factors, with open reduction and internal fixation (ORIF) using locking plates considered a reliable technique for select cases. Despite demonstrated utility, limited data exist regarding functional outcomes of locking plate fixati

Assessment of the responsibility between a road traffic accident and medical defects after the traffic accident injury of knee joint.

A 48-year-old Chinese woman was hit by a car in a road traffic accident. Local county hospital considered that her right knee was injured, but didn't find any sign of fracture from X-ray imaging. Then the hospital gave diagnosis of soft tissue contusion and the patient started to exercise with burden 21 days after her right lower limb was fixed by plaster slab. Four months later, she had to go back to the county hospital for recheck due to persistent pain on her right knee. Then, the right tibia outer plateau fracture was found. The patient rejected the advice of open reduction and internal fi
