Treatment of Recurrent Peroneal Tendon Dislocation by Peroneal Retinaculum Reattachment Without Fibular Groove Deepening.

There are numerous studies recommending fibular groove deepening in the surgical treatment of peroneal tendon dislocation; however, there are some disadvantages to fibular groove deepening procedures. In this study, we evaluated the results of anatomic reattachment of the peroneal retinaculum without fibular groove deepening as a treatment for traumatic peroneal tendon dislocation. Thirty-six patients with recurrent peroneal tendon dislocation, who underwent retinaculum repair without a fibular groove deepening procedure performed by 2 surgeons between March 2004 and March 2017, were enrolled

Gastrocnemius Recession as an Alternative to Midfoot Arthrodesis for Painful Midfoot Arthritis.

Arthrodesis has been described as the gold standard of treatment for midfoot degenerative joint disease (DJD) but has also been associated with nonunion, increased tourniquet times, technical difficulty, and a long postoperative non-weightbearing period. Although it is postulated that a contracted gastrocnemius may cause midfoot pain, there are no reports of using the procedure as a primary treatment for midfoot DJD. The purpose of this study was to determine whether gastrocnemius recession resolved midfoot pain from osteoarthritis and eliminated the need for midfoot arthrodesis. Eleven patien

Orthoplastic Management of Open Midfoot Injuries: Is Functional Limb Salvage Possible?

High energy open midfoot injuries are uncommon but devastating injuries. A combination of complex fracture dislocations and soft tissue injury patterns render reconstruction challenging. The aim of this study was to assess the surgical and patient reported outcomes following orthoplastic management of open midfoot injuries in a Major Trauma Center. A retrospective review of all open midfoot fractures admitted to our unit between January 2015 and December 2016 was undertaken. Demographics, operative details, complications, additional surgeries, and patient reported outcomes in the form of EQ-5D

[Detection of malnutrition in geriatric trauma patients : A reassessment].

BACKGROUND AND OBJECTIVE: Malnutrition in geriatric trauma patients is associated with an increased risk of complications and mortality and is therefore a key risk factor. The assessment of the affected patients plays an important role in improving the outcome of this growing patient group. MATERIAL AND METHODS: In 2016 a questionnaire was sent to 571 departments specialized in traumatology and orthopedics throughout Germany. The following were recorded: level of care, expertise in geriatric trauma, nutrition-based screening procedures and nutritional ward round procedures. RESULTS: The resp

Posterior Cervical Muscle-Preserving Interspinous Process Approach and Decompression: More Minimally Invasive and Modified Shiraishi's Selective Laminectomy.

OBJECTIVE: The cervical extensor musculature is important in cases of neck pain and loss of cervical lordosis after laminoplasty. Therefore, various surgical methods have been developed to preserve the muscle during laminoplasty. We have developed a posterior cervical muscle-preserving interspinous process (MIS) approach and decompression method. We have described the operation details and clinical outcomes of selected patients who have undergone this procedure. METHODS: The MIS approach and decompression method were performed in 20 consecutive patients who had only required central decompres

A rare complication of knee hematoma after genicular nerve radiofrequency ablation.

BACKGROUND: Genicular nerve radiofrequency ablation (RFA) is an intervention to treat patients with chronic knee pain who have failed previous conservative, pharmacologic, and surgical interventions. Vascular complications following interventional procedures of the knee are extremely rare. A delay in diagnosis may be detrimental for the viability of the affected limb and may ultimately require amputation. CASE PRESENTATION: A 76-year-old man with a history of severe bilateral knee osteoarthritis and grade 4 chondromalacia presented to our clinic with refractory, severe bilateral knee pain and

Comparison of Inpatient and Outpatient Preoperative Factors and Postoperative Outcomes in 2-Level Cervical Disc Arthroplasty.

OBJECTIVE: The purpose of this study was to evaluate factors associated with inpatient admission following 2-level cervical disc arthroplasty (CDA). A secondary aim was to compare outcomes between those treated on an inpatient versus outpatient basis. METHODS: Using data from the American College of Surgeons National Surgical Quality Improvement Program database, multivariate logistic regression analysis was used to assess the independent effect of each variable on inpatient or outpatient selection for surgery. Statistical significance was defined by p-values <0.05. The factors considered wer

[Endo-exo prostheses : Osseointegrated percutaneously channeled implants for rehabilitation after limb amputation].

BACKGROUND: In 1999 the first endo-exo femoral prosthesis (EEFP) was implanted in Germany in a patient who had suffered a traumatic above-knee amputation. This procedure involves a skeletally anchored exoprosthetic device that is inserted into the residual femur. The distal part of the implant protrudes transcutaneously and allows attachment to a prosthetic limb which provides direct force transmission to the external prosthetic components. The technique originated from dental implantology and helps to avoid possible problems resulting from treatment of amputated limbs using socket prostheses.

Case Report: Prune perineum syndrome: a rare case with an unfavourable outcome.

Prune perineum syndrome (PPS) is a rare anomaly, with only two previous case reports, both dying in the perinatal period. We report the first case of PPS that reached childhood. The patient presented with a hypoplastic genitalia and bilateral cryptorchidism. There was no evidence of an anal orifice. A significant prune-like mass was observed, extending from the perineum to both gluteal regions and to a cephalic mid-line bony prominence, with a 1cm central orifice that discharged urine. MRI confirmed the previous findings and revealed a right crossed ectopic kidney, intestinal malrotation, a hy

Chronic pain after carpal tunnel surgery: epidemiology and associated factors.

BACKGROUND: Chronic postoperative pain (CPOP) has been assessed after major orthopedic surgeries but not after carpal tunnel surgery (CTS). This study aimed at describing the evolution of nocturnal and diurnal pains during the year following CTS, and at looking for factors associated with CPOP. METHODS: Cohort of adult outpatients operated by one single surgeon, under regional anaesthesia (RA). Patients were questioned in the recovery room, and phoned 3 days and 12 months later. A multivariate analysis tested the association between CPOP and preoperative demographics, regional anaesthesia pro

Understanding cauda equina syndrome: protocol for a UK multicentre prospective observational cohort study.

INTRODUCTION: Cauda equina syndrome (CES) is a potentially devastating condition caused by compression of the cauda equina nerve roots. This can result in bowel, bladder and sexual dysfunction plus lower limb weakness, numbness and pain. CES occurs infrequently, but has serious potential morbidity and medicolegal consequences. This study aims to identify and describe the presentation and management of patients with CES in the UK. METHODS AND ANALYSIS: Understanding Cauda Equina Syndrome (UCES) is a prospective and collaborative multicentre cohort study of adult patients with confirmed CES man

Synovial Chondromatosis in Patient Presenting with Chronic Venous Stasis Ulcers.

Synovial chondromatosis (SC) is a rare, benign synovial growth most frequently involving the knee or hip joint. Common presenting symptoms include pain throughout the affected joint, reduced range of motion, and a palpable mass. We present an unusual case of SC presenting with symptoms of chronic venous stasis ulcer. A 49-year-old patient presented with swelling, hyperpigmentation, and ulcerations of his right lower extremity. Work-up including duplex and computed tomography scan revealed a calcified mass in the hip joint, highly suspicious for SC. A joint surgical approach from a vascular and

Open, Arthroscopic, and Percutaneous Cheilectomy for Hallux Rigidus.

Cheilectomy consists of excision of the dorsal exostosis and part of the metatarsal head. It is typically performed for patients in the earlier stages of hallux rigidus presenting with dorsal pain and dorsiflexion stiffness in the absence of through-range symptoms, rest pain, and plantar pain and with negative result on grind test. If joint motion-preserving surgery is appropriate, then cheilectomy is generally considered to be the first-line surgical choice. In addition to the standard open technique, minimally invasive surgery in the form of either percutaneous or arthroscopic surgery is ava
