Mixed-methods, participatory action research study exploring palliative and end-of-life care for LGBTIQ+ people in Switzerland: a protocol for the TRUST-PALL study.

INTRODUCTION: Lesbian, gay, bisexual, trans, intersex, queer/questioning and other sexual and gender minorities (LGBTIQ+) face systemic barriers and discrimination in healthcare settings, leading to significant health disparities. These challenges persist in palliative and end-of-life care (PEOLC), where older LGBTIQ+ people often lack family support and experience social isolation. Despite the increasing ageing of the LGBTIQ+ population in Switzerland, there is limited evidence on their specific PEOLC needs. Additionally, healthcare providers' knowledge and practices regarding LGBTIQ+ inclusi

Artificial intelligence (AI) in nursing education: An evolutionary concept analysis.

BACKGROUND: Although Artificial Intelligence (AI) has been explored for decades in nursing education, the existing body of knowledge provides no clear definition of the phenomenon. As technological advancements continue to shape teaching and learning environments, establishing a clear and robust definition of AI in nursing education is critical. AIM: To clarify the concept of AI in nursing education from an evolutionary perspective. METHODS: We conducted a concept analysis following Rodgers' evolutionary method. The searches were performed in September 2024 in MEDLINE/PubMed, CINAHL, Scopus,

A randomized controlled trial of a multimodality digital health intervention to aid the delivery of the modified ketogenic diet therapy for epilepsy: The Keto eCoach trial protocol.

OBJECTIVE: Dietary therapy is an effective treatment for drug-resistant epilepsy; however, low adherence presents a significant barrier to use in the clinical setting. This study aims to evaluate a multimodality digital health intervention to overcome this barrier and improve outcomes in adults undertaking the modified ketogenic diet for epilepsy. METHOD: This is a two-arm, randomized, controlled trial. A total of 136 participants will be randomized 1:1 to receive the modified ketogenic diet as per standard clinical care, or with the addition of the multimodality digital health intervention f

Personalised approach to hypertension treatment: protocol for the HYPERMARKER randomised controlled trial.

INTRODUCTION: Blood pressure treatment response is variable in individual patients, and the choice of medical therapy is often dependent on clinician experience. Treatment choices could be personalised by patient empowerment and metabolomic profiles, and augmented by machine learning, but robust evaluation is lacking on how these can be combined to enhance clinical effectiveness. The HYPERMARKER trial will evaluate how an individualised choice of medication class can address the avoidable global health and economic burdens of hypertension. METHODS AND ANALYSIS: The HYPERMARKER trial is a proo

Framing the Canadian Health Technology Assessment as an Organisational Process: A Bibliometric Analysis.

Health Technology Assessment (HTA) is an organizational process used to determine the value of health-technologies. There is considerable variation in how HTA is developed globally, with Canada at the forefront of this field, having pioneered the use of HTA and implemented it extensively across healthcare organisations at all levels. This study is relevant for its unique focus on citation networks and author keywords regarding HTA in Canada, providing a holistic view by analysing HTA as an organizational process. By identifying fundamental process elements and differentiating research streams

Protocol of the RADIO-STAR trial: a phase 1 safety and dose finding study of hypofractionated radiotherapy to the stellate ganglia for the treatment of ventricular arrhythmia.

INTRODUCTION: Sympathetic activation is the hallmark of cardiac disease, driving disease progression and triggering ventricular arrhythmia (VA). Despite optimal medical therapy, many patients experience recurrent VAs refractory to medical therapy, leading to repetitive implantable cardioverter defibrillator (ICD) therapy, worse quality of life and adverse outcomes. Cardiac sympathetic denervation (CSD) through surgical removal of the stellate ganglia is an effective treatment for refractory VAs but carries a high complication rate. We hypothesise that high precision image guided radiotherapy c

A mixed-methods longitudinal observational study exploring physical activity during pregnancy in women with pre-existing diabetes, support needs and associations with diabetes management: a study protocol.

INTRODUCTION: Pregnancy in women with pre-existing type 1 or type 2 diabetes (T1D, T2D) is associated with increased risk of complications, largely driven by maternal glucose control. Hormonal changes during pregnancy make glucose management more challenging. Physical activity (PA) may improve glucose control and reduce complications; however, little is known about PA patterns in this population and no pregnancy-specific PA guidance exists for women with pre-existing diabetes. Understanding the behaviours and experiences of both pregnant women and the healthcare professionals (HCPs) who suppor

Non-invasive brain stimulation on functional capacity in prefrail older adults: a protocol for a randomised clinical trial.

INTRODUCTION: Therapeutic physical exercise (TPE) is considered the cornerstone intervention to prevent or reverse frailty; however, prefrail individuals often face challenges achieving the exercise intensity required to induce multisystem adaptations. Non-invasive neuromodulation techniques-including virtual running-based visual illusion (VR) and, transcranial direct current stimulation (tDCS),-have shown promising effects on neuroplasticity, motor performance and executive function. The primary objective of this study is to compare the effect of adding two neuromodulation strategies to a mul

Measured and supervised physical exercise for older individuals with asthma, chronic obstructive pulmonary disease and obstructive sleep apnoea (MIOLI): protocol of a randomised controlled trial.

INTRODUCTION: Asthma, chronic obstructive pulmonary disease (COPD) and obstructive sleep apnoea (OSA) are prevalent chronic respiratory diseases associated with increased comorbidity, mortality and healthcare costs. Physical activity and exercise are widely recommended as part of treatment for these conditions, yet the specific effects of Nordic walking (NW) remain underexplored. The aims of this randomised controlled trial (RCT) are to improve physical fitness, functional capacity and respiratory health and increase regular physical activity and quality of life of older adults with asthma and

VIR-SAAF: a study protocol for a national decentralised randomised controlled trial of home-based sleep apnoea screening in patients with atrial fibrillation in Denmark.

BACKGROUND: Obstructive sleep apnoea (OSA) is a common and modifiable risk factor for atrial fibrillation (AF), yet it remains frequently unrecognised and undertreated. Despite guideline recommendations to consider OSA screening, no consensus exists on optimal strategies. The VIR-SAAF (VIRtual national study with home Sleep Apnea investigation in patients with Atrial Fibrillation) trial investigates whether home-based OSA screening and referral for potential treatment improve quality of life, physical activity and AF-related outcomes compared with standard care. METHODS AND ANALYSIS: VIR-SAAF

Apomorphine and psilocybin to improve recovery from coma in the intensive care unit: protocol for a prospective, open-label, dose-escalation study.

INTRODUCTION: Recovery of consciousness after brain injury remains a major challenge in the intensive care unit (ICU), and pharmacological treatments that directly promote awakening from coma and other disorders of consciousness (DoC) are lacking. We hypothesise that simultaneously targeting the two core components of consciousness by pharmacological means-arousal with apomorphine and awareness with psilocybin-is safe in the ICU and may facilitate evidence of cerebral reserves after brain injury. METHODS AND ANALYSIS: This trial aims to evaluate the safety and potential efficacy of apomorphin

Epidemiology, seroprevalence, and circulation of Chikungunya virus in Southern Africa (SADC region): A systematic review.

Chikungunya virus (CHIKV) is an emerging arbovirus that causes major morbidity in tropical regions, but its epidemiology in Southern Africa is poorly defined. This study compiles research on CHIKV circulation, clinical characteristics, and surveillance strategies in the region, specifically in the Southern African Developing Community (SADC) block. A systematic search of peer-reviewed publications published between January 2012 and October 2025 that reported CHIKV detection, seroprevalence, or clinical symptoms in eight Southern African countries was conducted. Eligible studies included human

Estimating the burden of leptospirosis in the Caribbean: Insights from environmental and sociodemographic factors.

INTRODUCTION: Previous studies up to early 2000s found that leptospirosis incidence in humans was high across the Caribbean region (CR), yet up-to-date and reliable surveillance data are scarce. Limited research capacity in the region has further contributed to less robust characterisation of transmission drivers, perpetuating a cycle of neglect. To address these gaps and support evidence-based public health responses, this study aims to update incidence estimates in the CR by integrating data from multiple data sources, including peer-reviewed publications, surveillance reports and environmen

Phase II multicentre double-blind randomised controlled trial of a Bivalent VaccInation against Salmonella Typhi and Paratyphi A (BiVISTA) using a controlled human infection model of paratyphoid A infection: study protocol.

INTRODUCTION: Enteric fever, primarily caused by Salmonella enterica Typhi and Salmonella enterica Paratyphi A (SPA), is endemic mainly in South Asia, disproportionately affecting school-age children. Although typhoid conjugate vaccines (TCVs) are effective and implemented in many countries, no licensed vaccine exists against paratyphoid A. Bivalent vaccines targeting both S. Typhi and SPA may address this gap. Although field efficacy trials are not considered feasible, controlled human infection models (CHIMs) offer an alternative pathway for evaluating vaccine efficacy. This will be the firs

Australian Pancreatic Cyst Registry: protocol for a prospective national clinical quality registry of intraductal papillary mucinous neoplasms.

INTRODUCTION: Intraductal papillary mucinous neoplasms (IPMNs) are common pancreatic cystic neoplasms with malignant potential. Current evidence on IPMN management is derived largely from retrospective surgical cohorts that over-represent high-risk cases and provide limited insight into the long-term outcomes of conservatively managed lesions. Australia currently lacks a prospective national dataset to evaluate surveillance pathways, practice patterns, guideline adherence and long-term outcomes in individuals with IPMN. Variation between international guideline recommendations may also contrib

Risk of colonisation and infection by extended-spectrum β-lactamase-producing Enterobacterales and carbapenemase-producing Enterobacterales in patients with neutropenia: protocol for the SCREEN-IN multicentre prospective observational study.

INTRODUCTION: Infections caused by extended-spectrum β-lactamase-producing Enterobacterales (ESBL-E) and carbapenemase-producing Enterobacterales (CPE) are a major concern in patients with neutropenia with haematological diseases, as they may lead to inadequate empirical therapy and poor outcomes. However, the relationship between intestinal colonisation and subsequent infection in this population remains insufficiently characterised. The SCREEN-IN Study aims to identify factors associated with incident ESBL-E/CPE colonisation and to develop and validate a prediction model for infection caused

Mixed-methods study exploring medium to longer-term outcomes following selective dorsal rhizotomy in ambulatory children with cerebral palsy at a tertiary hospital in the UK: MOSAiC study protocol.

INTRODUCTION: Selective dorsal rhizotomy (SDR) is one of the treatment options available for spasticity management in ambulatory children and young people with cerebral palsy (CYPwCP). Although improvements in gross motor function one to two years after surgery have been established, evidence of longer-term benefit requires further investigation. Given the irreversible nature of SDR and the increased rehabilitation commitments required from families and clinicians, providing evidence of longer-term benefits is essential to support their decision-making. This study aims to investigate medium (3

Neurocognitive and Emotional Outcomes in Childhood Cancer: A Developmental Perspective.

BACKGROUND: Childhood cancer survivors (CCSs) are at heightened risk of long-term neurocognitive and emotional difficulties that can affect educational attainment, social participation, and overall quality of life. These outcomes vary across developmental stages and are influenced by treatment modality, age at diagnosis, and central nervous system (CNS) involvement. METHODS: A comprehensive literature search was conducted in PubMed, Scopus, PsycINFO, and Web of Science for articles published between January 2000 and June 2024. Search terms included combinations of "childhood cancer survivors,
