Finger Reconstruction With Distally Based Dorsal Metacarpal Flaps: A Systematic Review.

BACKGROUND: Distally based dorsal metacarpal flaps for the reconstruction of finger soft-tissue defects are classified into 3 categories: dorsal metacarpal artery perforator flap, reverse dorsal metacarpal artery (RDMA) flap, and extended reverse dorsal metacarpal artery (ERDMA) flap. The present systematic review aimed to evaluate differences in the outcomes of commonly encountered defects and postoperative complications among these three flaps. METHODS: PubMed, Scopus, and Web of Science were systematically searched from when the flats were first reported to May 2021. Random-effects meta-an

The pedicled submental flap in facial reconstruction.

Submental flap (SF) is a reconstructive technique that can be utilised for reconstruction of medium to large oral cavity defects, head and neck cancer, and pharyngocutaneous fistula. The submental artery island flap is pedicled on the submental artery and veins. The vascular pedicle has a length of up to 8cm. If it is further dissected to its origin from the facial artery, the pedicle can be lengthened by an additional 1-2cm to reach the lateral canthus and zygomatic arch. The use of SF for the reconstruction of head and neck defects has re-emerged over the past decade due to various reasons.

The application of indocyanine green (ICG) and near-infrared (NIR) fluorescence imaging for assessment of the lymphatic system in reconstructive lymphaticovenular anastomosis surgery.

INTRODUCTION: Lymphedema has traditionally been managed through noninvasive means with complete decongestive therapy. However, complete decongestive therapy is an intensive program that requires lifelong adherence by patients with lymphedema. More recently, reconstructive surgical procedures have shown promise in improving lymphedema by physiologically restoring lymphatic function. One of these types of procedures, lymphaticovenular anastomosis, relies on technological advances in imaging, particularly indocyanine green lymphangiography. AREAS COVERED: This article reviews indocyanine green a

G-ear: a user-friendly tool for assisted autologous ear reconstruction.

The major breakthroughs in the fields of reverse engineering and additive manufacturing have dramatically changed medical practice in recent years, pushing for a modern clinical model in which each patient is considered unique. Among the wide spectrum of medical applications, reconstructive surgery is experiencing the most benefits from this new paradigm. In this scenario, the present paper focuses on the design and development of a tool able to support the surgeon in the reconstruction of the external ear in case of malformation or total absence of the anatomy. In particular, the paper descri

Modified Unilateral Incomplete Cleft Lip Repair with Primary Nasal Overcorrection: A Muscle-Driven Technique.

Millard's rotation-advancement principle, first-described 60 years ago, continues to guide surgeons in reconstruction of the unilateral cleft lip. Modifications of the rotation-advancement repair are commonly used; however, distinction between the complete and incomplete forms of cleft lip has received less emphasis in surgical procedure descriptions. As a unilateral incomplete cleft lip could present with more tissue overall and a larger lip on the cleft side than a complete form of cleft lip, the authors have adopted a deformity-specific technical approach to treat each particular cleft form

Reconstruction of recalcitrant pressure ulcers with latissimus dorsi muscle-splitting free flaps.

BACKGROUND: Reconstruction of recalcitrant pressure ulcers is very challenging because all available local tissues have been exhausted. Although occasionally suggested as reconstructive options in some reports, free flaps are still not favored for pressure ulcers because of the less available recipient vessels in buttock area and the need for position change. Here, we describe our experience with latissimus dorsi muscle-splitting free flaps harvested in prone position for recalcitrant pressure ulcers. METHODS: Between January 2012 and January 2020, 10 patients of recalcitrant pressure ulcers

Improved cephalic index following early cranial vault remodeling in patients with isolated nonsyndromic sagittal synostosis.

OBJECTIVE: Isolated nonsyndromic sagittal synostosis (SS) is the most common form of craniosynostosis in children, accounting for approximately 60% of all craniosynostoses. The typical cranial measurement used to define and follow SS is the cephalic index (CI). Several surgical techniques have been suggested, but agreement on type and timing of surgery is lacking. This study aimed to evaluate the authors' institutional experience of surgically treating SS using a modified subtotal cranial vault remodeling technique in a population-based cohort. Special attention was directed toward the effect

Secondary Raised Intracranial Pressure After Cranial Vault Remodeling for Isolated Sagittal Craniosynostosis.

The management of sagittal craniosynostosis has evolved over the decades as teams seek to refine their surgical approaches to idealize head shape with the least possible morbidity. Here, the authors identify the incidence of raised intracranial pressure (ICP) and its risk factors, requiring secondary surgical intervention after cranial vault remodeling (CVR) procedure at a single tertiary referral craniofacial unit. A retrospective case-control study was performed on the patients with isolated non-syndromic sagittal craniosynostosis. All patients who underwent CVR in our unit and had a minimum

Custom Interlocking Implants for Primary and Secondary Reconstruction of Large Orbital Floor Defects: Case Series and Description of Workflow.

Large fractures of the orbital floor present several technical challenges for the craniomaxillofacial surgeon. One major challenge is limited surgical access as it pertains to the adaption of a large implant within the confines of a small incision. Transfacial approaches or techniques to extend the incision may improve access but have the potential drawback of creating unesthetic scars, nerve injury, and lid contracture. In this series we present a novel solution combining virtual surgical planning and the use of a 2 piece, interlocking patient specific implant to address the problem of limite
