Outcomes of the Starfish procedure using muscle transfer for independent digital control of a myoelectric prosthesis.

BACKGROUND: Surgical and technological advancements have revolutionized myoelectric prosthetic options for patients with upper extremity amputations, but partial hand amputations have remained a challenge. The Starfish procedure is a novel surgical technique that involves transferring intrinsic hand muscles to a subcutaneous location to allow immediate signal detection and independent digital control of a myoelectric prosthesis. We report the outcomes of our cohort of patients treated with this procedure. METHODS: Twenty-one patients underwent the Starfish procedure between 2015 and 2021. All

Transrectal osteopathic manipulation treatment for chronic coccydynia: feasibility, acceptability and patient-oriented outcomes in a quality improvement project.

CONTEXT: Pain of the coccyx, coccydynia, is a common condition with a substantial impact on the quality of life. Although most cases resolve with conservative care, 10 % become chronic and are more debilitating. Treatment for chronic coccydynia is limited; surgery is not definitive. Osteopathic manipulative treatment (OMT) is the application of manually guided forces to areas of somatic dysfunction to improve physiologic function and support homeostasis including for coccydynia, but its use as a transrectal procedure for coccydynia in a primary care clinic setting is not well documented. The development of postoperative knee chondrolysis after intra-articular pain pump infusion of an anesthetic medication: a series of twenty-one cases.

BACKGROUND: Postoperative chondrolysis in the knee joint caused by continuous intra-articular pain pumps infusing bupivacaine is a serious complication that severely affects function. We report the clinical course of a series of twenty-one patients who were referred to our clinic with this complication. METHODS: A physical examination and a review of medical records were conducted. The condition of the articular cartilage was determined from operative notes, photographs, magnetic resonance images, and radiographs. Knee function was assessed with the Cincinnati Knee Rating System. RESULTS: Th

The long-term outcome of microvascular decompression for trigeminal neuralgia.

BACKGROUND: Several surgical procedures to treat trigeminal neuralgia (tic douloureux) are available, but most reports provide only short-term follow-up information. METHODS: We describe the long-term results of surgery in 1185 patients who underwent microvascular decompression of the trigeminal nerve for medically intractable trigeminal neuralgia. The outcome of the procedure was assessed prospectively with annual questionnaires. RESULTS: Of the 1185 patients who underwent microvascular decompression during the 20-year study period, 1155 were followed for 1 year or more after the operation.

Arthroscopic Latarjet With Cortical Buttons Versus Open Latarjet With Screws: A Short-Term Comparative Study.

BACKGROUND: The arthroscopic bone block procedure according to Latarjet remains a controversial subject, and few comparative studies have demonstrated the benefit of arthroscopy over open surgery. PURPOSE/HYPOTHESIS: The objective of this study was to compare both procedures by analyzing the short-term clinical results. The hypothesis was that the arthroscopic procedure is superior to the standard open procedure. STUDY DESIGN: Cohort study; Level of evidence, 3. METHOD: This was a retrospective comparative study. Patients treated for chronic anterior instability by arthroscopic Latarjet wit

Clinical Outcomes Following the Latarjet Procedure in Contact and Collision Athletes.

BACKGROUND: Few studies have evaluated the success of the Latarjet procedure for recurrent anterior glenohumeral instability in the contact or collision athlete. The purpose of this study was to evaluate the return-to-sport and functional results of the Latarjet procedure in this select group. METHODS: One hundred and nine consecutive contact or collision athletes (112 shoulders) treated with an open Latarjet procedure for recurrent anterior glenohumeral instability were retrospectively identified. Seventy-three shoulders in 73 patients (67%) were evaluated at a mean follow-up of 52 months (r

Arthrodesis of the Metacarpophalangeal and Interphalangeal Joints of the Hand: Current Concepts.

Metacarpophalangeal arthrodesis and interphalangeal arthrodesis are excellent tools in the surgeon's armamentarium to restore function of the disabled hand. Typical indications for these procedures are pain, deformity, and/or stiffness. Arthrodesis is generally considered a salvage procedure to be used when other reconstructive procedures, such as arthroplasty, are not possible or would be associated with a high rate of complication or failure. To determine the most functional position for arthrodesis in each patient, the surgeon should preoperatively evaluate the compromised joint in the cont

Anal stenosis.

Anal stenosis occurs most commonly following a surgical procedure, such as hemorrhoidectomy, excision and fulguration of anorectal warts, endorectal flaps, or following proctectomy, particularly in the setting of mucosectomy. Patients who experience anal stenosis describe constipation, bleeding, pain, and incomplete evacuation. Although often described as a debilitating and difficult problem, several good treatment options are available. In addition to simple dietary and medication changes, surgical procedures, such as lateral internal sphincterotomy or transfers of healthy tissue are other po

The Sauvé-Kapandji procedure for posttraumatic wrist disorders: further experience.

A prospective survey was conducted to evaluate the outcome of the Sauvé-Kapandji procedure for posttraumatic wrist disorders. Eighty four patients were treated, all with posttraumatic disorders of the distal radioulnar joint, 73 as an isolated procedure, 11 in combination with another wrist procedure. There was significant pain decrease and high patient satisfaction (74%). The range of motion increased in the flexion/extension arc from 109 degrees to 124 degrees (p = 0.006) and, in those with limited forearm rotation, from 71 degrees to 134 degrees (p = 0.006). According to the Mayo Clinic wri

The misuse of the Lapidus procedure: re-evaluation of the preoperative criteria.

Fusion of the metatarsocuneiform joint has been documented in the literature for a number of conditions affecting the first ray. The fixation techniques have advanced greatly since Lapidus advocated the procedure, but the indications need to remain rigid and narrow. The review of the literature illustrates numerous complications, and this case presentation specifically depicts the long convalescence of the failed fusion of the first metatarsocuneiform joint. The Lapidus procedure ultimately should be used as a last resort to eliminate painful arthrosis from the metatarsocuneiform joint, reduce

The "turn-up" plasty: one solution to a complex problem.

This case study presents a patient with severe osteomyelitis of the femur, which did not respond to vigorous antibiotic therapy. Compromised kidney function, malnutrition, and copious drainage from an open wound led the patient and physician to select a seldom used procedure, the "turn up" plasty to treat the problem. This procedure provides a stump for an above-the-knee prosthesis providing the patient with more optimal function than if the femur and lower leg were amputated. The procedure involves removal of the infected femur, foot amputation, knee disarticulation and turning the lower leg

Transabdominal preperitoneal videolaparoscopic hernioplasty without mesh fixation.

BACKGROUND: Laparoscopic inguinal hernia repair using the transabdominal preperitoneal (TAPP) technique requires a stapler for mesh fixation, making the method expensive. AIMS: To demonstrate the feasibility of videolaparoscopic inguinal hernioplasty (TAPP) without mesh fixation in patients who are not stage 3 according to the European Hernia Society classification. METHODS: Forty patients underwent videolaparoscopic inguinal hernioplasty (TAPP) with a polypropylene mesh and without fixation from October 2017 to October 2018. After the procedure, outpatient follow-up appointments were schedu

A Practical Guide to Lower Extremity Nerve Blocks for the Sports Medicine Surgeon.

BACKGROUND: Peripheral nerve blocks (PNBs) are vital in the administration of surgical analgesia and have grown in popularity for use in lower extremity arthroscopic procedures because of their capacity to safely and effectively control pain. The number and specificity of PNBs, however, have made choosing the best option for a procedure nebulous for orthopaedic surgeons. PURPOSE: To present a narrative literature review of the PNBs available for arthroscopic hip and knee procedures that is adapted to an audience of orthopaedic surgeons. STUDY DESIGN: Narrative literature review. METHODS: A

Meta-analysis and trial sequential analysis of randomized evidence comparing general anesthesia vs regional anesthesia for laparoscopic cholecystectomy.

BACKGROUND: In an effort to further reduce the morbidity and mortality profile of laparoscopic cholecystectomy, the outcomes of such procedure under regional anesthesia (RA) have been evaluated. In the context of cholecystectomy, combining a minimally invasive surgical procedure with a minimally invasive anesthetic technique can potentially be associated with less postoperative pain and earlier ambulation. AIM: To evaluate comparative outcomes of RA and general anesthesia (GA) in patients undergoing laparoscopic cholecystectomy. METHODS: A comprehensive systematic review of randomized contro
