A total of 216 patients with a ruptured aneurysm of the anterior part of the circle of Willis were enrolled into this prospective randomized study of timing of the operation after aneurysmal subarachnoid hemorrhage (SAH). We describe a rare case of a patient who had a spontaneously ruptured pineal epidermoid cyst concurrent with a thalamic germinoma. Computed tomographic scans led to the detection of both tumors. A definite diagnosis of the free fat in the ventricle was made by its movable feature and characteristic absorption value on computed tomographic scan. Surgical treatment produced a good result.

A randomized study was conducted to investigate the effects of antenatal corticosteroids and ampicillin in the management of preterm pregnancies under 34 weeks complicated by premature rupture of membranes. Benign cystic teratoma of the ovary is a common neoplasm which only rarely presents as a result of ovarian rupture. Rupture into the rectum has been described infrequently. We describe a case of rupture of an ovarian cystic teratoma which presented as a change in bowel habit, due to faeces filling the cyst and compressing the rectum.

A case of subarachnoid hemorrhage caused by the rupture of a dissecting aneurysm of the vertebral artery is reported. The initial angiogram was normal, and repeat angiography carried out 12 days later revealed a dissecting aneurysm of the right vertebral artery. This angiographic change seems to be due to the progression of minimal dissection that was not found at the time of first angiogram.

The incidence of bleeding duodenal varices is exceedingly rare. We report a case of a 35-year-old chronic alcoholic who died of severe fatal upper gastrointestinal bleeding from a ruptured duodenal varix located in the second portion of duodenum. This diagnosis was suspected at endoscopy and was confirmed at autopsy. A review of literature shows 20 case reports of bleeding duodenal varices with two fatalities. Amniotic fluid volume was serially assessed by real-time ultrasound in 90 patients who presented with premature rupture of the membranes (PROM) and not in labor. The degree of oligohydramnios was correlated to the outcome of pregnancy, as reflected by pregnancy prolongation, intrapartum fetal heart rate patterns consistent with umbilical cord compression, cesarean section rate, fetal distress, infection, and perinatal mortality rate. These data suggest that in patients with PROM the degree of oligohydramnios is positively correlated with unfavorable pregnancy outcome.

One knee was injured at a soccer match, the other 5 months later subsequent to a minor fall. There was no evidence of systemic disease. A case of ruptured distal middle cerebral aneurysm is reported, A 20-year-old man with Marfan's syndrome suffered from sudden loss of consciousness. A CT scan and cerebral angiogram revealed a distal middle cerebral aneurysm with a large intracerebral hematoma. The aneurysm was clipped and the hematoma was evacuated successfully. This case is presented and the literature related to the aneurysm associated with Marfan's syndrome is reviewed.
