Non-iatrogenic spinal accessory nerve palsy due to lymph node compression.

Injury to the spinal accessory nerve (SAN) is most commonly of iatrogenic or traumatic origin. The incidence of SAN palsy resulting specifically from lymph node compression is difficult to ascertain, as such cases are typically reported in association with malignancies or following surgical procedures. We report the case of an adolescent who presented with clinical features consistent with SAN palsy, including spontaneous shoulder asymmetry, in the absence of prior trauma or surgical intervention to the cervical region. Surgical exploration revealed compression of the SAN by enlarged lymph nod

[Pyoderma gangraenosum after implantation of a shoulder endoprothesis].

Pyoderma gangrenosum is an inflammatory, ulcerative, neutrophilic dermatosis that is clinically characterized by rapidly evolving, painful, necrotic skin lesions. It is typically associated with chronic systemic inflammatory or neoplastic diseases, but may also occur secondary to cutaneous injury or surgery (pathergy). Post-surgical pyoderma gangrenosum typically develops around surgical sites within the immediate postoperative period, mimicking early wound infection. However, common treatment strategies including antibiotics and debridement fail to improve or even worsen symptoms. Postoperati

Tumoral calcinosis in the neck region involving an unusual site in a hemodialysis patient.

Tumoral calcinosis is a rare disorder characterized by calcium salt deposition in the periarticular soft tissue region. It often occurs in the hip, elbow, shoulder, foot, and wrist. Soft tissue calcification associated with tumoral calcinosis is rare in the head and neck. The present case describes a 65- year-old male who had been on hemodialysis for 3 years. A bulky mass, containing chalky amorphous calcified material, occurred at the lower neck and superior mediastinum. Chronic renal failure is a common condition associated with secondary tumoral calcinosis. The pathogenesis has been progres

Actinomycetoma by Nocardia brasiliensis in a girl with Down syndrome.

We describe the case of a 14-year-old girl with Down syndrome and a large cutaneous plaque localized to the right neck and shoulder that had enlarged over five years after a minor traumatic injury. The plaque was characterized by numerous inflammatory nodules and fistulae that secreted purulent discharge. Nocardia grains were identified and Nocardia brasiliensis was identified by culture. Histopathology examination showed a chronic inflammatory infiltrate with granuloma development. The treatment scheme was with Diaminodiphenylsulfone 50/mg/d and Trimethoprim-Sulfamethoxazole 800/160 mg BID. T

The origin of the long head of the biceps from the scapula and glenoid labrum. An anatomical study of 100 shoulders.

We dissected 105 cadaveric shoulders to study the origin of the tendon of the long head of biceps, and examined histologically the interrelationship between the tendon, the supraglenoid tubercle and the superior labrum of the glenoid. In all specimens approximately 50% of the biceps tendon arose directly from the superior glenoid labrum with the remainder attached to the supraglenoid tubercle. The main labral origin was from the posterior labrum in more than half of the specimens, and in a quarter this was the only labral attachment. On the basis of the biceps attachment to the anterior or pos

Motor neuron disease in the United States, 1971 and 1973-1978: patterns of mortality and associated conditions at the time of death.

Mortality rates for deaths "due to" and "with" motor neuron disease are presented for the first time. Age-specific mortality rates increase with age until 70 to 74 years and then decline. There appear to be no major differences by race in the age-adjusted mortality rates, but these rates are higher for males both white and nonwhite. A case-control study of all deaths with amyotrophic lateral sclerosis (ALS) was conducted for deaths due to ALS in the year 1971. Conditions associated with ALS at the time of death include pneumonia and bronchopneumonia, symptoms referable to respiratory system, s

The surgical anatomy of the spinal accessory nerve and the internal branch of the superior laryngeal nerve.

Injury to the spinal accessory nerve may occur during conservation neck dissection. In supraglottic laryngectomy preservation of the internal branch of the superior laryngeal nerve has largely been ignored. Cadaver dissection and diagrams are used to review the surgical anatomy of these two structures. Particularly important are the relationship of the spinal accessory nerve to the sternocleidomastoid muscle, its interaction with the cervical plexus, and its superficial path inferiorly. Landmarks for the identification of the internal branch of the superior laryngeal nerve are indicated, and t

Establishment of the model of motorcyclist ejection injury.

OBJECTIVE: To establish the device and model of motorcyclist ejection injury. METHODS: Based on our laboratory devices, a motorcyclist ejection injury simulation system was developed. A total of 18 pigs were approved by the local animal experimentation and ethics committee to serve as the motorcyclist substitutes. In this ejection motion, the animal rode freely at the motor driver seat and was straightly accelerated by means of our custom motorcyclist ejection injury simulation system. When it was speeded up to the preset velocity (v equal to 30, 40 or 50 km/h) at the preset position, the ani

Clinical Effect of Button Plate Fixation in the Treatment of Anterior Sternoclavicular Joint Dislocations.

OBJECTIVE: Multiple reconstruction approaches for the anterior sternoclavicular joint have been described. No gold standard technique has been established. Owing to the well-established role of open reduction and internal fixation by means of plates and/or screws, rigid fixation is associated with the risk of implant failure, migration, and need for removal. This study aimed to evaluate the safety and efficacy of using button plates for the treatment of anterior sternoclavicular joint dislocation. METHODS: From January 2018 to May 2021, seven patients with a median age of 47 (range 37-57) yea

Residual articular incongruity is not a predictor of poor functional outcomes in operatively treated intra-articular elbow fractures.

BACKGROUND: Intra-articular and comminuted elbow fractures are complex injuries that present a tremendous surgical challenge. The goal of surgical fracture reduction is an anatomical restoration of the articular surface to restore physiological biomechanics of the elbow joint and to regain an unrestricted elbow function. However, these fractures are often accompanied by residual articular step-off, gap displacement, and intra-articular loose bodies. To date, the potential correlation between the quality of fracture reduction and the postoperative functional outcomes has not been investigated f

Shoulder Motion Overestimated by Mallet Scores.

BACKGROUND: Brachial plexus birth injuries (BPBI) occur in up 0.4 to 4.6 per 1000 live births. Weakness about the shoulder and development of glenohumeral joint contractures are common sequalae of BPBI. Shoulder function in children with BPBI is frequently assessed using the modified Mallet classification to evaluate upper extremity motion deficits. The purpose of this study was to assess the accuracy of the abduction, external rotation, and hand-to-mouth Mallet classification scores in children with BPBI using motion capture. METHODS: A retrospective study of 107 children with BPBI who under
