Long-term protection after immunization with protein-polysaccharide conjugate vaccines in infancy.

The polysaccharide-encapsulated bacteria, Haemophilus influenzae type b, Neisseria meningitidis and Streptococcus pneumoniae are important causes of invasive bacterial infection in childhood, accounting for most of the cases of bacterial pneumonia and meningitis worldwide. Protein-polysaccharide conjugate vaccines have been developed over the last 20 years and have proven very effective in controlling these infections. Although studies have consistently shown that herd immunity is critical for population protection, long-term individual protection against polysaccharide-encapsulated bacteria a

Airway microbiology and host defences in paediatric non-CF bronchiectasis.

Bronchiectasis in children without cystic fibrosis is most common in socioeconomically disadvantaged communities. Recurrent pneumonia in early childhood and defective pulmonary defences are important risk factors. These help establish a 'vicious cycle' of impaired mucociliary clearance, infection, airway inflammation and progressive lung injury. Haemophilus influenzae, Streptococcus pneumoniae, Moraxella catarrhalis and Pseudomonas aeruginosa are the main infecting pathogens. H. influenzae predominates across all ages, while P. aeruginosa is found in older children with advanced disease. It is

Fatal necrotising fasciitis and myositis in a cat associated with Streptococcus canis.

Necrotising fasciitis and necrotising myositis are rare but serious life threatening conditions reported mainly in human beings and dogs. Most cases to date have been caused by beta-haemolytic streptococci of Lancefield groups A, B, C or G. Necrotising fasciitis has been reported only twice in cats and necrotising myositis has never been described. This paper describes a fatal case of necrotising fasciitis and myositis with pneumonia and septicaemia in a nine-year-old cat. The cat had been undergoing treatment for a suspected tear of the cranial cruciate ligament, but on the seventh day of tre

[Treatment of community acquired pneumonia].

The S3 guidelines for diagnosis and therapy use German epidemiological data to set the standards for a risk and degree of severity based treatment of community acquired pneumonia (CAP). Over the last few years, a change in pathogen epidemiology has been recognized with the significance of staphylococci and enterobacteria increasing. CAP is becoming increasingly a disease of the elderly, for whom a more broadly effective initial therapy appears to be needed. Resistant pathogens play an important role in the course of the disease. For patients with a low risk of mortality, a reduction in the len

Gatifloxacin, gemifloxacin, and moxifloxacin: the role of 3 newer fluoroquinolones.

Gatifloxacin, gemifloxacin, and moxifloxacin are the newest fluoroquinolones and show excellent in vitro activity against a wide variety of respiratory tract pathogens, many gram-negative aerobic organisms, and Bacteroides fragilis. These agents may be administered as oral and/or intravenous formulations with excellent bioavailability. The pharmacodynamics of these 3 new fluoroquinolones is more favorable than that of levofloxacin or ciprofloxacin for Streptococcus pneumoniae. All 3 agents are approved for the treatment of acute exacerbation of chronic bronchitis and community-acquired pneumon

Clinical features of HIV disease in developing countries.

HIV disease progresses from an asymptomatic period of variable duration, through mild symptoms, to severe disease characteristic of cellular immunodeficiency. The rate of progression from infection to severe disease is probably similar world-wide. However, individuals in developing countries have more symptomatic disease, in keeping with the high incidence of morbidity in the general population, and poor survival with advanced disease. The clinical manifestations of severe HIV-related immunosuppression vary with geographical region. Tuberculosis (TB) is the most important severe opportunistic

Advancing pleural effusion diagnosis: Application of Molecular Culture ID in pathogen detection.

The diagnosis of pleural infection has traditionally relied on microbiological culture, a method often limited by slow bacterial growth and difficulties in culturing fastidious organisms. This study introduces Molecular Culture ID, a rapid and comprehensive molecular technique capable of detecting and identifying a wide range of bacteria down to the species level. By analyzing 440 pleural effusion samples derived from patients suspected of pleural infection and comparing the outcome to routine diagnostics, we demonstrate that Molecular Culture ID significantly outperforms traditional culture.

Bacterial β-carbonic anhydrases: a six-step prioritization framework for antibacterial target validation.

Bacterial β-carbonic anhydrases (β-CAs) are actively studied as potential targets for next-generation antibacterial agents. Despite extensive biochemical and structural characterization, their validation as isoform-specific therapeutic targets remains incomplete. Here, we developed and applied a six-filter validation framework-encompassing genetic essentiality, physiological relevance, biochemical feasibility, structural rationale, cellular target engagement, and therapeutic context-to integrate disparate data into a strategic prioritization map across representative pathogens (Escherichia col

CH•••S hydrogen bonds drive molecular recognition of ergothioneine by the microbial transporter.

Many bacteria harbor an ATP-binding cassette (ABC) transporter named EgtU specific for the human dietary antioxidant and 2-thioimidazole-containing low-molecular weight thiol ergothioneine (ET). How the solute binding domain, EgtUC, discriminates among ET and other similar molecules is unknown. Here, we use a "chimeric" mutagenesis strategy and two distantly related EgtUCs from Streptococcus pneumoniae and Helicobacter pylori to show that a suite of EgtUC alkyl CH•••S hydrogen bonds to the ET thione S atom are central determinants of molecular recognition. Small perturbations in CH•••S distanc

Pneumococcal meningitis complicated by extensive cerebral venous sinus thrombosis in an immunocompromised patient.

A man in his 40s who takes adalimumab for Crohn's disease presented with a 1-day history of fever, headache and rapid onset of agitation with reduced consciousness. He was intubated, and a subsequent lumbar puncture revealed severe Streptococcus pneumoniae meningitis. He was started on antibiotics and steroids; however, on sedation hold, he demonstrated abnormal posturing. Neurological imaging revealed extensive cerebral venous sinus thromboses (CVSTs). He was started on a heparin infusion and transferred to a tertiary neurological centre, where he was managed with a split treatment dose of en

Purulent Pericarditis Leading to Cardiac Tamponade and Constrictive Physiology.

BACKGROUND: Purulent pericarditis is a rare infection of the pericardial space, characterized by the accumulation of pus. CASE SUMMARY: A middle-aged woman presented with chest pain. Serial echocardiograms over approximately 16 hours demonstrated the development of cardiac tamponade and obstructive shock. Emergency pericardiocentesis yielded purulent fluid, which grew Streptococcus pneumoniae. Despite hemodynamic improvement, she subsequently developed constrictive physiology (CP), which was diagnosed with cardiac catheterization and cardiac magnetic resonance. DISCUSSION: This case illustra

The vaccine dosing effect overcomes the reduced immunogenic potential and in vivo efficacy of 33F pneumococcal serotype.

Serotype 33F is an emerging Streptococcus pneumoniae (Spn) serotype associated with asymptomatic nasopharyngeal (NP) colonization and invasive pneumococcal disease (IPD). Serotype 33F is a component in the advanced version of the pneumococcal conjugate vaccine 15 (PCV 15) formulation. However, in the murine vaccination model, serotype 33F exhibits reduced immunogenicity, correlating with reduced protection against 33F. To investigate if the reduced immunogenic potential of serotype 33F can be overcome by optimizing the vaccine dosing, we immunized C57BL/6 mice with a range of CRM197 conjugated

Development and qualification of an automated capillary Western method for the identification of polysaccharide serotypes in pneumococcal conjugate vaccine (PCV).

Streptococcus pneumoniae bacterial infection causes mortality in both adults and infants. To mitigate the impact of the disease, several Pneumococcal conjugate vaccines (PCVs) have been manufactured for the U.S. market, including the recent approval of the 15-valent PCV Vaxneuvance™ from MSD. These vaccines demonstrate high efficacy for both the adult and pediatric dose. These PCVs contain multiple unique serotypes in the final, formulated vaccine product, and identifying a specific polysaccharide, in the presence of other serotypes, is a critical quality attribute that must be demonstrated th

Thrombotic microangiopathy in children.

The syndrome of thrombotic microangiopathy (TMA) is a clinical-pathological entity characterized by microangiopathic hemolytic anemia, thrombocytopenia, and end organ involvement. It comprises a spectrum of underlying etiologies that may differ in children and adults. In children, apart from ruling out shigatoxin-associated hemolytic uremic syndrome (HUS) and other infection-associated TMA like Streptococcus pneumoniae-HUS, rare inherited causes including complement-associated HUS, cobalamin defects, and mutations in diacylglycerol kinase epsilon gene must be investigated. TMA should also be c

Peptide Ligands of AmiA, AliA, and AliB Proteins Determine Pneumococcal Phenotype.

The Ami-AliA/AliB oligopeptide permease of Streptococcus pneumoniae has been suggested to play a role in environmental sensing and colonisation of the nasopharynx by this human bacterial pathogen by binding peptides derived from bacterial neighbours of other species in the microbiota. Here, we investigated the effects of the peptide ligands of the permease's substrate binding proteins AmiA, AliA, and AliB on pneumococcal phenotype. AmiA and AliA ligands reduced pneumococcal growth, increased biofilm production and reduced capsule size. In contrast, AliB ligand increased growth and greatly incr

Streptococcus mitis Expressing Pneumococcal Serotype 1 Capsule.

Streptococcus pneumoniae's polysaccharide capsule is an important virulence factor; vaccine-induced immunity to specific capsular polysaccharide effectively prevents disease. Serotype 1 S. pneumoniae is rarely found in healthy persons, but is highly invasive and a common cause of meningitis outbreaks and invasive disease outside of the United States. Here we show that genes for polysaccharide capsule similar to those expressed by pneumococci were commonly detected by polymerase chain reaction among upper respiratory tract samples from older US adults not carrying pneumococci. Serotype 1-specif

First description of a catalase-negative Staphylococcus aureus from a healthy carrier, with a novel nonsense mutation in the katA gene.

We have screened 2568 healthy individuals (mostly children) for Staphylococcus aureus and Streptococcus pneumoniae nasal carriage between 2010 and 2012. Out of the isolated 751 S. aureus strains, we found one methicillin-sensitive catalase-negative S. aureus (CNSA). Our CNSA isolate possessed a novel nonsense point mutation in the katA gene leading to early truncation of the protein product. The strain was resistant to penicillin and erythromycin, but sensitive to all other tested antibiotics and carried the enterotoxin A gene. It belonged to sequence type 5 (ST5), which is a successful, world

Purulent pericarditis as a complication of bacteraemic Enterococcus faecalis urinary tract infection.

Purulent pericarditis is a rare clinical entity in the modern antibiotic era. The most common portal of entry is thought to be direct extension from a primary lung source and is usually caused by Staphylococcus aureus, Streptococcus pneumoniae or Haemophilus influenzae We report the case of a man aged 69 years who presented with purulent pericarditis due to Enterococcus faecalis likely caused by haematogenous spread from a urinary tract source. Urgent pericardiocentesis was vital and restored his haemodynamic stability. He was treated for a total duration of 4 weeks with susceptible antibiotic

Risk factors for community-acquired bacterial meningitis.

BACKGROUND: Bacterial meningitis is a significant burden of disease and mortality in all age groups worldwide despite the development of effective conjugated vaccines. The pathogenesis of bacterial meningitis is based on complex and incompletely understood host-pathogen interactions. Some of these are pathogen-specific, while some are shared between different bacteria. METHODS: We searched the database PubMed to identify host risk factors for bacterial meningitis caused by the pathogens Streptococcus pneumoniae, Neisseria meningitidis and Haemophilus influenzae type b, because they are three
