[Experiences with endoscopic placed endoprosthesis in stenosing malignancies of the esophagus and cardia].

Seventy-five patients with obstructing esophagogastric malignancies were palliatively treated by fiberendoscopic insertion of prosthetic tubes. The indications for endoscopic intubation are discussed, the procedure is described. The mortality rate was 6.6%. Complications were obstructions due to food impaction (10), tumor overgrowth (5) or mucosa folds (3), reflux and aspiration (7), dislocation of the prosthesis (5), perforation (1) and desintegration of the tube (1). Normal swallowing was restored in 98% and in general, there was a marked improvement in the quality of life. In summary, we co

Voice preservation in pyriform sinus carcinoma by hemicricolaryngopharyngectomy.

Hemicricolaryngopharyngectomy was performed for anterior pyriform sinus carcinomas with preservation of voice and swallowing. The surgical specimen includes the homolateral half of the hypopharynx, larynx, and cricoid ring. The pharynx is reconstructed with a pectoralis myocutaneous flap, a deltopectoral flap, skin or dermal graft. A semirigid mucosa-lined speech tube is created by folding in the remaining half of the larynx. This tube, which is 4 to 5 mm in diameter, serves as a phonatory structure. Speech is produced by occluding the stoma with the finger and shunting air via this dynamic ps

Nasopharyngeal reflux and neonatal apnea.

Although oropharyngeal dysfunction has been reported as a cause of dysphagia in newborns, to our knowledge oropharyngeal dysfunction and neonatal apnea have not been associated previously. We studied two infants in whom apnea and bradycardia developed primarily while feeding. The diagnosis was established by observation of the swallowing mechanism using cinefluoroscopy while the infant ingested a dilute barium meal. Thickened formula feedings and use of a special nipple resulted in complete cessation of apnea and bradycardia. Careful attention to the details that surround episodes of apnea is

Congenital cholinergic nervous system dysfunction in identical twins.

We report identical male twins with secretory diarrhea, craniofacial abnormalities often found in G syndrome, hypospadias, and abnormalities of the cholinergic nervous system including achalasia, decreased tearing, corneal hypesthesia, positive Mecholyl test (eye and esophagus), and decreased sweating. The children also lacked fungiform papillae but were clinically distinct from patients with either the Riley-Day syndrome or acquired dysfunction of the autonomic nervous system. Assessment of autonomic nervous system function should be considered in infants with recurrent diarrhea or swallowing

Symptomatology of taeniasis caused by Taenia saginata.

The course of an infection with T. saginata has been studied for two years on a volunteer who had swallowed a 16-20 week-old Cysticercus bovis. The pathognomonic character of the infections was shown to be the spontaneous release of cestode proglottids first appearing in the faeces at day 87 p. i. An incidental release of a larger number of segments occurred after an excessive consumption of alcohol. The next release of proglottids followed after 42 to 88 days. The importance of the results of this observation both for the immediate study of the field case and an elucidation of the epidemiolog

Cancer associated with collagen-vascular disease.

Two cases of cancer, each associated with a different collagen-vascular disease, are reported. The first patient, a 71-year-old white man, had a history of acute dermatomyositis and malignancy for a few weeks only. Death was associated with adenocarcinoma of the lesser curvature of the stomach with metastases to the liver and beyond. The second patient, a 69-year-old white man, had had symptoms associated with Raynaud's phenomenon for more than a decade and difficulty in swallowing, attributed to progressive systemic sclerosis for more than two years. He died with an epidermoid carcinoma of th

Aerophagia induced by the nasal obstruction on experimental animals.

The excessive accumulation of gas in the gastrointestinal tracts was invariably induced on experimental animals (mice, rats, guinea pigs, hamsters and rabbits) by simply obstructing nasal passages. The analysis of the gas showed the almost identical composition to the ambient air or flutus which was largely due to swallowed air. Also the numerous small foams were found on and underneath the epithelial lining of small intestine. The pathological evaluation was done both macroscopically and microscopically. Dying animals after nasal obstruction showed hemorrhagic and necrotic changes in the jeju

Bilateral injury to the hypoglossal nerve.

The hypoglossal nerve supplies motor function to the tongue. Two cases of bilateral, post-traumatic injury to the hypoglossal nerve are described. Physical and electromyographic examinations in these cases showed evidence of bilateral injury to the hypoglossal nerve as well as the absence of injury to the other closely associated cranial nerves. (A review of the English literature in the past 20 years shows no cases of isolated bilateral damage to the hypoglossal nerve, although there are several cases of unilateral or combined injury.) The disabilities resulting from hypoglossal nerve palsy a

[Botulism in a number of yearlings (author's transl)].

An outbreak of botulism in four yearlings of a single herd is reported. One animal recovered spontaneously. The other animals underwent euthanasia (animal A) or were slaughtered (animals B and C). Cl. botulinum toxin type C was detected in the sera of animals A and B as well as in the liver and abomasal contents of animal A. In addition, Cl. botulinum was isolated from the liver of animal A. The symptoms are described. Total muscular weakness, also resulting in lingual paralysis and inability to swallow, was the most important clinical feature. The mechanism of action of Cl. botulinum toxin, p

Wound botulism.

Botulism occurring in patients with wounds has been thought of as a rare disease. A patient with a lacerating wound of his hand and wrist and an avulsion of his fourth finger developed diplopia, dizziness, and slurred speech one week later, followed by generalized weakness and difficulty in swallowing. Repetitive nerve stimulation studies showed signs of neuromuscular block consistent with the diagnosis of botulism. Results of bacteriologic and immunologic tests were not revealing, but the subsequent course of progressing, and retrogressing, bulbar signs and symptoms with eventual nearcomplete

Viscous and Elastic Efficacies in Thickening Liquid Foods for Dysphagia Patients: Examination Using Newtonian and Shear-Thinning Viscoelastic Liquids.

A role of the shear-thinning property of thickened liquids used for supporting the dysphagia patients was examined utilizing the rheologically various properties of test liquids. The test liquids prepared through exploration of viscoelastic liquids were a Newtonian viscous liquid and non-Newtonian ones whose shear-thinning levels were different. For these test liquids, the flow experiments were performed with the swallowing model apparatus. The viscous effect in the flows in the apparatus was estimated by Stokes' law of resistance. The elastic effect, in other words, the elastic efficacy in th

Torus palatinus osteonecrosis: A hitherto unreported complication of long-term Denosumab use.

Torus palatinus is an osseus exostosis found in the midline of the hard palate. It is a relatively common condition and is benign in nearly all cases. However, complications such as airway obstruction, sleep apnea, and swallowing difficulties do exist. One of the more serious complications is osteonecrosis. Osteonecrosis of tori has scantly been reported in the literature. Reported risk factors include bisphosphonate use, trauma, and anticoagulation. Here, we present a case of torus palatinus osteonecrosis in the setting of Denosumab use, a monoclonal antibody for the treatment of osteoporosis

[Liver abscess triggered by a fishbone].

BACKGROUND: Liver abscess caused by foreign bodies constitutes a rare medical problem that few of us will encounter in a clinical setting. CASE PRESENTATION: We describe a case where a woman presented with sepsis and abdominal pain. Computed tomography (CT) of her abdomen revealed a large hepatic abscess containing a foreign body. Based on the size, shape and density of the object, a fishbone was suspected. INTERPRETATION: We hypothesise that she swallowed a fishbone, which then perforated the gastrointestinal tract and lodged in the liver. After interdisciplinary discussion it was decided t

Percutaneous Gastrostomy Tube Placement under Quadratus Lumborum Block: A Case Report.

The quadratus lumborum block is a technique that is not widely applied in abdominal surgery. The influence of the mode of anesthesia on the outcome of polymorbid patients is a controversial issue in the medical literature. We report a case in which we performed a quadratus lumborum block type 2 on a woman who was admitted to Riga's 1st hospital in need of gastrostomy, due to difficulty swallowing solid foods and liquids caused by hypopharynx carcinoma. On account of the patient's difficult airway, general anesthesia was deemed unsafe for the patient, with a risk of patient death. Percutaneous

Mysterious ureteral migration of a wooden toothpick from the digestive tract: case report and diagnostic considerations.

Migration of foreign bodies into the urinary tract is a rare event. In certain instances, to unravel the way that objects arrived in the urinary tract is not easy. We report the case of an accidentally swallowed wooden toothpick that migrated and was found in the left ureterovesical junction, protruding into the bladder. Even though the computed tomography scan is widely employed to evaluate the urinary tract, this resource does not have a good sensitivity for detecting foreign bodies. Our report presents an insight into the best imaging approach if wooden toothpicks are suspected. In the pres

An 18-Month-Old Child Suffering From Achalasia Cardia Successfully Treated With Pneumatic Dilation.

Achalasia cardia is an idiopathic esophageal motility disorder. It is rare in children and infrequent in below school-going age groups. The "bird's beak" appearance of the lower esophagus on the esophagogram (barium swallow) is a classical radiological finding in the cases of esophageal Achalasia. The goals of achalasia therapy are symptom relief and improvement of esophageal emptying to prevent megaesophagus. The most effective treatment options are pneumatic dilation and surgical myotomy (Heller's myotomy). Pneumatic dilation is the initial treatment of choice and does not preclude myotomy.

A Missed Diagnosis of Laryngotracheal Injury Secondary to Emergency Intubation: Lessons Learned.

Iatrogenic laryngotracheal trauma is a potentially fatal complication of endotracheal intubation, especially in an emergency setting. Symptoms are almost always related to speech, breathing, and swallowing. Hoarseness being the commonest symptom, while shortness of breath and stridor always signify more devastating injury. We present a case of iatrogenic subglottic and tracheal stenosis, which was misdiagnosed in the emergency department during the first visit. This case report highlights the importance of salient history and thorough examination with a high index of suspicion in a stridorous

Paediatric submental epidermoid cyst.

A young girl presented with an insidious onset, gradually increasing midline submental swelling of 1-year duration and pain on swallowing for 6 months. Ultrasonography of the neck suggested a hypoechoic cystic swelling of the submental region between the muscles of the floor of the mouth, with no increased vascularity. An extraoral surgical enucleation was done and a postoperative biopsy suggested an epidermoid cyst. Epidermoid cysts of the submental region are extremely rare and any midline head and neck lesion in children requires critical examination and evaluation to avoid complications. H

A Rare Presentation of a Rare Disease: Oropharyngeal Dysphagia as The Main Manifestation of Myasthenia Gravis.

Oropharyngeal dysphagia is defined as the inability or difficulty to initiate swallowing. It has a wide array of etiologies including structural and neurologic diseases. Myasthenia gravis (MG) is a rare autoimmune condition caused by antibodies against the post-synaptic membranes of the neuromuscular junction, leading to fatigable weakness of skeletal muscles. Bulbar symptoms are less prevalent than ocular symptoms or limb weakness but can be particularly morbid. Non-neurologists are more likely to be the first providers to evaluate patients with dysphagia and should be familiar with MG. We re

Establishing a Foundation for Optimal Feeding Outcomes in the NICU.

Many infants in the NICU experience difficulties with oral feeding, which contribute to prolonged length of stay. Oral feeding is a complex task involving sensorimotor functioning of the face, mouth, and larynx, as well as coordination of sucking, swallowing, and breathing. The care provided in the NICU starting at birth sets the stage for future oral feeding. The purpose of this article is to describe strategies that will establish a positive foundation to support optimal oral feeding. Nurses can use these strategies to protect newborns from noxious stimuli and promote positive auditory, tact
