Factors Explaining the Severity of Acute Achilles Tendinopathy Among Runners: A Comprehensive Cross-Sectional Analysis.

BACKGROUND: Achilles tendinopathy (AT) is a prevalent musculoskeletal injury among runners, accounting for approximately 10% of all running-related injuries. AT can result in persistent symptoms and impact the quality of life of runners. The Victorian Institute of Sport Assessment questionnaire (VISA-A) is a widely used self-reported tool for assessing severity of AT. However, the anatomic, biomechanical, psychological, and social factors that influence its score are still poorly understood. The aim of this study is to identify the factors that explain the severity of AT based on the VISA-A sc

The total ankle arthroplasty learning curve with third-generation implants: a single surgeon's experience.

BACKGROUND: Renewed interest in total ankle arthroplasty (TAA) has developed globally as a result of recent literature supporting new-genera … e events. Our results demonstrate that some common complications could be avoided altogether (nerve/tendon injuries), some decreased quickly with experience (intraoperative fractures and component malpositioning), and some persisted unchanged throughout this study (wound complications). These findings should influence surgical training, surgeon willingness to ado …

[Transplantation of allogeneic menisci].

The increase in severe ligament injuries of the knee has led to consideration of the need for meniscal transplantation in reconstructive operations for chronic rotational instability. Transplantation of the medial meniscus was carried out in 30 sheep, which were divided up into 2 groups, each containing 15 sheep. In the first group lyophilized, y-sterilized allogeneic menisci were transplanted and these underwent complete remodelling in 48 weeks. In the other group, deep-frozen allogeneic menisci were used and these remained fully functional without remodelling. Twenty-three transplantations o

Allograft Reconstruction Technique for Chronic Pectoralis Major Tears.

BACKGROUND: Allograft reconstruction is indicated for chronic pectoralis major tears because such tears will have fibrotic changes, retraction, and stiffness that prevent primary repair1-3. Various reconstruction techniques have been described, particularly in cases in which the myotendinous unit is degenerated. Autografts (such as hamstring tendons and bone-patellar tendon-bone grafts) and allografts (such as Achilles tendon, fascia lata, and dermal grafts) have both been utilized4,5. In the present video article, we describe a technique utilizing a doubled Achilles tendon allograft, which pr

Principles of management of acute hand injuries.

The care of the severely injured hand is a complex, technically demanding specialty. The majority of hand injuries, however, can be well cared for in the emergency ward or operating room by physicians without advanced training by following basic principles of diagnosis and treatment. Hopefully, by adherence to these principles, the morbidity of "minor" hand injuries can be minimized.

Percutaneous repair of acute Achilles tendon tears with minimally invasive augmentation with the tendon of flexor hallucis longus: a 5 years outcome study.

BACKGROUND: Even after surgical repair, acute Achilles tendon rupture (ATR) may leave residual strength and function deficits. Augmenting percutaneous repair with the flexor hallucis longus (FHL) tendon may limit elongation and reinforce the repair. We report 5-year outcomes of acute ATR managed with percutaneous repair alone or with minimally invasive FHL augmentation. SOURCES OF DATA: A prospective comparative cohort of 53 patients (27 percutaneous repair alone, 26 with FHL augmentation) with a minimum 5 years follow-up. Objective and subjective outcomes were collected. AREAS OF AGREEMENT:

Peroneus longus autograft versus hamstring tendon autograft in primary anterior cruciate ligament reconstruction: a systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: Anterior cruciate ligament reconstruction (ACLR) is a commonly performed procedure aimed at restoring knee stability and function following ligament injury. Although hamstring tendon (HT) autografts are widely used, concerns regarding donor-site morbidity and graft-related limitations persist. The peroneus longus tendon (PLT) has recently gained attention as an alternative autograft, offering favourable biomechanical properties and potentially reduced morbidity. However, evidence comparing clinical outcomes between PLT and HT autografts remains limited and inconsistent. Therefore,

Gait Characteristics and Intersegmental Foot and Ankle Motion in Patients With Neglected Achilles Tendon Rupture.

Neglected Achilles tendon rupture is associated with persistent weakness and altered gait; however, the detailed intersegmental foot and ankle biomechanics have not been fully characterized. This study quantitatively evaluated temporospatial, kinematic, and kinetic gait alterations in patients with neglected Achilles tendon rupture using a multi-segment foot model. Thirteen patients were compared with 26 healthy participants selected by propensity score matching based on age, sex, and body mass index. Intersegmental foot and ankle kinematics were calculated using the DuPont foot model, and ank

Shear Wave Tensiometry in the Evaluation of Achilles Tendon Loading: A Cross-Sectional Study on Conservatively Treated Tendons After Rupture.

PURPOSE: The aim of this study was to quantify differences in the shear wave speed (SWS) between a conservatively treated Achilles tendon (AT) after rupture and the unaffected contralateral tendon. METHODS: Twenty-nine participants who received conservative treatment following Achilles tendon rupture (ATR) were enrolled. Measurements were taken during a single follow-up visit, which occurred between 1 and 7 years after the rupture. Tendon load was assessed using a shear wave tensiometer comprising a set of four accelerometers attached to the tendon. Stiffness, thickness, and cross-sectional a

Anterior cruciate ligament reconstruction using quadriceps tendon autograft is a viable option for small-statured female patients.

PURPOSE: The choice of graft for anterior cruciate ligament (ACL) reconstruction remains controversial. The quadriceps tendon (QT) autograft is a good alternative for ACL reconstruction. However, concerns regarding its use in short-statured patients, related to donor site morbidity, anterior knee pain, or loss of muscle strength remain. This study aimed to compare muscle strength and morbidity between patients with short and normal statures following ACL reconstruction with a QT autograft. METHODS: A total of 73 female patients (mean age, 33.8 ± 11.5 years) who underwent primary ACL reconstru

Extreme stiff knee recurvatum following manipulation under anesthesia and botulinum neurotoxin injection.

In this article we report a case of stiff, neglected extreme recurvatum knee deformity in a 17-year-old female. This is the first reported case in the literature of an acquired ligamentous stiff extreme knee recurvatum following manipulation under anesthesia and botulinum neurotoxin injection. Μodified Judet quadricepsplasty combined with Ilizarov external hinged frame fixation was implemented. This dual technique can be considered as a rational approach for such an extreme deformity as it stabilizes, progressively corrects, and prevents recurrence. Biomechanical Study of a Multifilament Stainless Steel Cable Crimp System Versus a Multistrand Ultra-High Molecular Weight Polyethylene Polyester Suture Krackow Technique for Achilles Tendon Rupture Repair.

Currently, Achilles tendon rupture repair is surgically addressed with an open or minimally invasive approach using a heavy, nonabsorbable suture in a locking stitch configuration. However, these sutures have low stiffness and a propensity to stretch, which can result in gapping at the repair site. Our study compares a new multifilament stainless steel cable-crimp repair method to a standard Krackow repair using multistrand, ultra-high molecular weight polyethylene polyester sutures. Eight matched pairs of cadavers were randomly assigned for Achilles tendon repair using either Krackow techniqu

Carpal Tunnel Syndrome: A Potential Early, Red-Flag Sign of Amyloidosis.

Carpal tunnel syndrome (CTS) can be caused by the deposition and accumulation of misfolded proteins called amyloid and is often an early manifestation of systemic amyloidosis. In patients undergoing surgery for idiopathic CTS, a recent study identified amyloidosis by tenosynovial biopsy in 10.2% of men older than 50 years and women older than 60 years; all positive patients had bilateral symptoms. These findings have led to a renewed interest in amyloidosis as an etiology of CTS. The 2 most common systemic amyloidoses, immunoglobulin light chain and transthyretin amyloidosis, affect the heart,

Diagnostic Characteristics of Standard Radiographs and Magnetic Resonance Imaging of Ruptures of the Tibialis Posterior Tendon.

The present study aimed to diagnose complete rupture (CR) and longitudinal rupture (LR) of the posterior tibial tendon (PTT) from the magnetic resonance imaging findings in patients with PTT dysfunction and to analyze and compare the radiographs from each group to identify radiographic indicators related to the progression of PTT injury that would allow the radiographic diagnosis of CR. We evaluated 32 feet in 27 patients with PTT dysfunction (mean age 66.5, range 49 to 82, years). Radiographs were used to acquire weightbearing anteroposterior images of the foot, which were used to measure the
