An ultrastructural study of glomerular basement membrane in rheumatoid arthritis patients with urinary abnormalities.

We measured the thickness of the glomerular basement membrane (GBM) in 48 rheumatoid arthritis (RA) patients with proteinuria and/or hematuria and studied its relationship to clinical features of RA. Ten cases with minor glomerular abnormalities, renal cancer, donor of renal transplantation, were studied as controls. Secondary glomerular diseases and hereditary thin basement membrane disease (TBMD) were excluded. Mean GBM thickness was 289 +/- 74 nm (mean +/- SD) in RA patients, which was significantly thinner than that of control group (342 +/- 38 nm) (p < 0.01). Mean GBM thickness were 276 +

[Radiographic criteria of differential diagnosis of hereditary tubulopathies in children].

In search for x-ray signs of skeletal involvement specific for each type of hereditary tubulopathies, the authors analyze the results of clinical and x-ray examinations of 144 children aged 2 to 16. Vitamin D-resistant rickets which was diagnosed in 83 children was found to be characterized by varus deformations of the lower limb bones, by systemic osteoporosis of various degrees with hypertrophy of the osseous beams in the epimetaphyseal compartments and accelerated bone age. In renal tubular acidosis diagnosed in 28 children the most typical findings were valgus deformation of the lower limb

A simple method for diagnosing traumatic occlusion of the vertebral artery at the craniovertebral junction.

STUDY DESIGN: A simple method, thin-slice contrast-enhanced computed tomographic (CT) scan, was evaluated as a first-line rapid screening procedure for diagnosing traumatic occlusion of the vertebral artery at the craniovertebral junction. OBJECTIVES: This procedure was performed in patients suffering from injuries of the high cervical spine or the craniovertebral junction. Vertebral angiography was undertaken after this procedure to confirm the diagnosis. SUMMARY OF BACKGROUND DATA: Well-enhanced vertebral arteries can be demonstrated clearly in normal subjects by using this method. Occlusi

Immunolocalization of B-50 (GAP-43) in the mouse olfactory bulb: predominant presence in preterminal axons.

Because the growth-associated protein B-50 (GAP-43) has been implicated in neurite outgrowth as well as in synaptic plasticity, we studied its light and electron microscopical distribution in the mouse olfactory bulb, an area of the nervous system which exhibits a high degree of synaptic plasticity. Immunofluorescent staining with monospecific affinity-purified anti-B-50 antibodies revealed that B-50 is most abundantly expressed in the olfactory nerve fibre layer and the granule cell layer neuropil, while little staining was observed in the external plexiform layer and in cell bodies. B-50 is

Reinforced vascular grafts: a comparative study.

Reinforced vascular grafts are designed to resist compression and flexion forces, for example in axillo-femoral or below-knee femoropopliteal reconstruction. This study compared eight types of 6-mm reinforced grafts (five polytetrafluoroethylene (PTFE) and 3 Dacron). On a specially designed rig, graft diameter and flow were measured during compression by a standard series of weights up to 3.6 kg, and on another rig grafts were flexed through 135 degrees. Meadox Microvel graft resisted compression best (p less than 0.05) but there were no important differences between the other grafts (Vascutek

[Familial hematuria and Alport's syndrome].

In their review the authors discuss the history of Alport's syndrome, its nomenclature, incidence, genetics, clinical diagnosis. The authors emphasize electron microscopic findings and criteria of the diagnosis (haematuria or renal failure in the family, progressive nervous deafness, typical changes of the basal glomerular membrane (GBM) and ophtalmological findings of lenticonus or perimacular spots. Familial haematuria (FH) is according to the authors defined as haematuria in several members of the family. Based on data in the literature and the authors' experience, the authors discuss the d

The salvage of the lower limbs: choice of the prosthetic material in femoro-distal and axillo-femoral bypass.

The choice of the prosthetic material in arterial surgery of patients with rest pain and/or impending gangrene of the lower limbs still presents several problems. The poor run-off, the small caliber of the distal vessels, the crossing of the joint areas and the length of the bypass may lead to early occlusion of the prosthesis. This experience is based on 268 femoro-distal (214 femoro-popliteal below the knee and 54 femoro-tibial) and on 121 axillo-femoral/popliteal bypasses (89 axillo-femoral, 28 axillo-bifemoral and 4 axillo-popliteal). In the 389 surgical procedures we have employed the aut

Encapsulated papillary neoplasms of the thyroid. A study of 14 cases followed for a minimum of 10 years.

Fourteen cases of encapsulated papillary thyroid neoplasm in which extracapsular extension was not observed and a minimum of 10 years follow-up was available are herein presented. The cases were divided into three categories: encapsulated papillary carcinomas (five cases), which had cytologic features typical of papillary thyroid carcinoma (vesicular or indented nuclei) and an entirely or predominantly thick capsule with capsular invasion; encapsulated papillary neoplasms of undetermined malignancy (seven cases), in which the cytologic features were also typical of papillary carcinoma but the

Relationship of gingival thickness and bleeding to loss of probing attachment in shallow sites following nonsurgical periodontal therapy.

Periodontal sites of shallow initial probing depth often seem to lose probing attachment following various types of periodontal therapy, including nonsurgical therapy. The susceptibility to this treatment-associated probing attachment loss may conceivably be related to gingival architecture as well as to the inflammatory status of the tissues. This study was designed to study the relationship of buccolingual gingival thickness and bleeding on probing in shallow buccal sites (less than or equal to 3.5 mm probing depth) to loss of probing attachment following nonsurgical therapy. 3 months follow

Alcoholic cardiomyopathy and sudden cardiac death.

Seven hundred fifty-two cases of instantaneous sudden cardiac death were studied. Alcoholic cardiomyopathy was found in 127 cases (16.9%), predominantly in men under age 50 (73%). The heart was enlarged, with an average mass of 441 +/- 8 g. Light microscopy revealed uneven thinning and atrophy of the cardiomyocytes, with fatty infiltration and development of fatty tissue around the vessels of the left ventricle and interventricular septum, up to the subendocardial areas. In the majority of cardiomyocytes, electron microscopy showed atrophy of the myofibrils, pronounced dilation of the sarcopla

Distribution of Thy-1-like immunoreactivity in normal, sympathetically denervated and grafted mouse irides.

The distribution of Thy-1-like immunoreactivity was studied in whole-mounts of adult mouse iris and intraocular iris grafts 4 days and 4 weeks postoperatively. After fixation in picric acid/paraformaldehyde, the irides were incubated with a polyclonal rabbit anti-mouse brain Thy-1 antibody. In the adult mouse iris, a dense network of thin bundles and individual fibres was seen on the dilator plate and in the sphincter. Fluorescence paucities, resembling Schwann cell nuclei, were frequently seen along the bundles. Numerous mast cells, stained specifically with the Thy-1 antibody, were scattered

Fibronectin production by synovial intimal cells.

The distribution of fibronectin in the inflamed synovium has been described previously using immunohistochemical methods. Under favourable conditions, it is possible to demonstrate apparent cytoplasmic staining of the intimal cell layer. We have further investigated the localisation of fibronectin in the synovial intimal cells using higher resolution techniques with peroxidase-antiperoxidase staining and high power light microscopy of semi-thin Araldite sections and immunoelectron microscopy using a protein A-gold technique. Ultrastructural studies of cerebral arteries and collateral vessels in moyamoya disease.

Moyamoya disease was originally defined as a characteristic syndrome of recurrent headaches, occlusion of the distal internal carotid arteries and the foggy (moyamoya) clusters of collateral vessels at the base of the brain as demonstrated by cerebral angiography. The etiology is unknown and pathobiology is poorly understood. We examined the intracranial arteries in 3 patients to demonstrate characteristic changes and to obtain a better understanding of the basis mechanisms of the disease. Controls were obtained from 3 normotensive patients who died as a result of cancer. Occluded internal car

Evaluation of the Amniostat-FLM assay for assessment of fetal lung maturity.

Results of the "Amniostat-FLM" assay, a rapid semiquantitative test for phosphatidylglycerol, were compared with determinations of the lecithin/sphingomyelin (L/S) ratio, with phosphatidylglycerol measured by two-dimensional thin-layer chromatography, and with results of the "shake test" for 94 specimens of amniotic fluid. Correlation between results with the Amniostat and the other tests was excellent. All four tests are very accurate when predicting lung maturity. The predictive value of a negative test, i.e., that hyaline membrane disease would not occur, was between 92 and 100%. However, t

Alport's syndrome: experience at Hôpital Necker.

We review the characteristic morphologic features identifiable by electron microscopy that have been described in patients presenting with Alport's syndrome. They are diffuse thickening and splitting of the glomerular basement membrane (GBM), which are either isolated or associated with thinning. In occasional cases, only diffuse thinning can be seen. Our study of 100 families followed in Necker's hospital, of which 60 patients have had electron microscopic examination of their renal parenchyma, demonstrates that these GBM changes are highly suggestive of Alport's syndrome. All the patients in

Ruffed cell: a new type of neuron with a distinctive initial unmyelinated portion of the axon in the olfactory bulb of the goldfish (Carassius auratus). III. Three-dimensional structure of the ruffed cell dendrite.

The three-dimensional structure of the ruffed cell dendrite in the olfactory bulb of the goldfish (Carassius auratus) was studied by means of light and high-voltage electron microscopy of Golgi-impregnated material, combined Golgi-electron microscopy, and electron microscopy of serial thin sections. Ruffed cell dendrites ramify in the glomerular area with tufted branching patterns. They give rise to many appendages of various shapes and sizes: sheetlike processes, fingerlike projections, or small tuftlike appendages. These appendages, singly or together, appear to entwine or cover other neuron

Early disruptions in brain capillary blood flow control in a mouse model of Alzheimer's disease.

In Alzheimer's disease (AD), vascular dysfunction and impaired control of brain blood flow can disrupt cellular bioenergetics decades before cognitive decline. Identifying the earliest vascular changes is therefore critical for developing therapies that intervene before the cascading events leading to neuronal decline. Brain energy supply is dynamically controlled by the cerebral vasculature, where capillary thin-strand pericytes are emerging as key players. We recently identified a central role for ATP-sensitive potassium (K+, KATP) channels in enabling thin-strand pericytes to sense local ne

Six-Month Changes in Periorbital and Perioral Wrinkles and Facial Skin Quality following Low-Cross-Linked Hyaluronic Acid.

INTRODUCTION: Fine wrinkles in the periorbital and perioral regions present therapeutic challenges because of thin dermal structure and high mobility. In these delicate facial areas, high-G' fillers may be limited by concerns regarding superficial placement and palpability, whereas non-cross-linked boosters often provide only short-lived effects. Low-G' hyaluronic acid formulations with favorable cohesivity, low complex viscosity, and high water affinity were developed to improve durability while maintaining flexibility suitable for intradermal treatment in thin, highly dynamic facial regions.

Soft pneumatic actuators for wearable systems: a structural classification and integration-oriented evaluation.

Soft pneumatic actuators promise gentle, body-safe assistance, yet many fail at the moment of integration: inflation alters geometry, increases profile, and stresses seams and ports. This topical review reframes actuator selection through a structure-first lens that links how an actuator is built to how it deforms and how it embeds in garments. Actuators are classified by structural architecture, deformation behavior, and fabrication method, and evaluated against four integration criteria central to wearable systems: geometric compatibility, fabrication scalability, system integration readines

Analysing ocular structural morbidities in younger adults using anti-seizure drugs: a cross-sectional study.

BACKGROUND: The primary treatment for managing epilepsy consists of anti-seizure drugs (ASDs). Nevertheless, their prolonged use has been associated with various ocular complications. The structural alterations, such as thinning of the retinal nerve fiber layer (RNFL) and macular edema, have received limited research attention in younger adults undergoing extended ASD treatment. OBJECTIVE: This study aimed to evaluate the structural and functional changes in the eyes specifically, refractive error, tear film quality, macular health, and RNFL thickness among young adults who are on long-term A
