Current status of organ transplant in Islamic countries.

OBJECTIVES: The Organization of Islamic Cooperation consists of 57 member states whose people are mainly followers of the Islamic religion. During the past several decades, organ transplants have been increasingly used for the treatment of end-stage organ failures worldwide. This study is to investigate the current status of organ transplant in Islamic countries. MATERIALS AND METHODS: For data collection a literature, review was carried out. Information from international registries was used and key persons from some countries were contacted. RESULTS: In all 5 Islamic countries of North Afr…

Recent Trends in Kidney Transplant in the United States.

The number of new wait list registrants has increased in the last decade but not to the same magnitude as the increase in the number of end stage renal disease patients in the United States. The number of wait list patients has increased at a much higher pace due to the lack of kidney supply. The overall number of kidney transplants only increased slightly. Paired exchange kidney transplant is a viable source of increasing the availability of kidney transplant and also offers access to transplant to patients with immunologic barriers to their intended donors. Paired donor exchange results in s…

Living kidney donor transplantation: an alternative with limitations.

BACKGROUND: The major issues involved in the decision to donate are the perioperative risk and the risk of chronic kidney disease or even end-stage renal disease. The usual glomerular filtration rate (GFR) in kidney donors after transplantation is approximately 70% of the predonation rate; however, some have a GFR <60 mL/min/1.73 m(2). So after kidney donation, mild to moderate renal insufficiency may occur. Thus, it is important to identify predictor factors of postdonation kidney function. OBJECTIVES: To evaluate the influence of predictor factors in the evolution of the remaining kidney fu

Quality of life in renal transplant recipient and donor.

OBJECTIVES: Quality of life (QoL) assessment in renal transplant patients has become an important tool in evaluating outcomes. In this work the QoL of the renal transplant donor and recipient are compared to healthy, age- and BMI-matched individuals. MATERIALS AND METHODS: The donors were all living related. The immunosuppression protocol was prednisolone, cyclosporine/tacrolimus, and mycophenolate mofetil/azathioprine. Renal function was stable. Quality of life was assessed by KDQOL-SF-36. It includes 36 items divided into 8 scales. RESULTS: Comparison among healthy subjects (n = 20), kidne

Trabecular bone score in kidney transplant recipients.

SUMMARY: It is uncertain whether bone mineral density (BMD) can accurately predict fracture in kidney transplant recipients. Trabecular bone score (TBS) provides information independent of BMD. Kidney transplant recipients had abnormal bone texture as measured by lumbar spine TBS, and a lower TBS was associated with incident fractures in recipients. INTRODUCTION: Trabecular bone score (TBS) is a texture measure derived from dual energy X-ray absorptiometry (DXA) lumbar spine images, providing information independent of bone mineral density. We assessed characteristics associated with TBS and

Evaluation of renal functions in pediatric liver transplantation.

AKI is an important complication after LT. As our LT series contains a quite high number of children with ALF unlike published studies, we aimed to determine pre-LT and long-term renal functions in children both with ALF and with CLD. Demographic and disease-related data of 134 transplanted children were evaluated retrospectively. Pre-LT and follow-up GFR and pediatric RIFLE scores were determined. Mean pre-LT GFR was not dependent on the disease presentation or severity of chronic disease. While there was an initial decline until first week of post-LT in CLD children, an increase was observed

Utility of HLA Antibody Testing in Kidney Transplantation.

HLA antigens are polymorphic proteins expressed on donor kidney allograft endothelium and are critical targets for recipient immune recognition. HLA antibodies are risk factors for acute and chronic rejection and allograft loss. Solid-phase immunoassays for HLA antibody detection represent a major advance in sensitivity and specificity over cell-based methods and are widely used in organ allocation and pretransplant risk assessment. Post-transplant, development of de novo donor-specific HLA antibodies and/or increase in donor-specific antibodies from pretransplant levels are associated with ad

[Recurrence of primary diseases after liver transplantation].

A majority of primary diseases for which orthotopic liver transplantation is carried out may recur in the liver allograft, mostly in adults. As the indication criteria, transplantation surgery and post-transplantation care improve, the patients survival lengthens as well, leading to concurrently increasing incidence as well as an increase in the relevance of recurrent diseases, which are the most significant cause of late liver graft dysfunction. The frequency, clinical consequences and therapeutic options of different disease recurrence vary considerably. Even recently the worst prognosis has

Review of combined liver and kidney transplantation in children.

In this review, we focused on CLKT with regard to indication, results, outcome, and future developments. PH1 is one of the most common diagnoses for adult and pediatric patients qualifying for CLKT. The other major indication for combined transplantation is ARPKD. CLKT appears to be superior to sequential liver and kidney transplantation in the majority of patients and overall results following CLKT are now good, even in small children. Clinical observations suggest that there is an immunological advantage of CLKT in comparison with isolated liver or kidney transplantation. More clinical studi

Patients with psychotic disorders in solid-organ transplant.

BACKGROUND: Psychotic disorders are considered a relative or absolute contraindication to organ transplant, but information about their impact on transplant is limited. OBJECTIVE: To describe the clinical course of psychotic patients while they were on the waiting list and the outcomes of patients with psychotic disorders undergoing evaluation for organ transplant. METHODS: Thirty-eight transplant candidates with a diagnosis of psychotic disorder were analyzed in this descriptive study. The following variables were collected before transplant: demographics, type of transplant, cause of organ

Kidney transplantation in an adult patient with VACTERL association.

The vertebral, anal, cardiac, tracheoesophageal, renal, and limb birth defects (VACTERL) association is a rare, non-random constellation of congenital abnormalities among which urinary tract anomalies can be included. In the presence of these anomalies, patients are suspected to have a higher rate of renal failure than average. We report a case of a 22-year-old woman with VACTERL association and consequent end stage renal failure. A live-related kidney transplant was carried out successfully and the postoperative course was uncomplicated. The patient had immediate graft function. Risk factors

Persistent severe polyuria after renal transplant.

Polydipsia and polyuria are common symptoms in patients with diabetes insipidus (DI), which can be due to inadequate vasopressin production (cranial DI) or vasopressin insensitivity (nephrogenic DI). Clinical diagnosis of the subtypes of DI can be tricky. We present a 44-year-old man with a strong family history of DI who had been diagnosed with autosomal dominant nephrogenic DI from infancy. At the age of 40, he had progressed to end-stage renal failure. When he experienced unresolving severe polyuria after renal transplant, further investigations revealed that he was misdiagnosed and that he

BK Virus Nephropathy in Heart Transplant Recipients.

Polyomavirus-associated nephropathy (PVAN) has become an important cause of kidney failure in kidney transplant recipients. PVAN is reported to affect 1% to 7% of kidney transplant recipients, leading to premature transplant loss in approximately 30% to 50% of diagnosed cases. PVAN occurring in the native kidneys of solid-organ transplant recipients other than kidney only recently has been noted. We report 2 cases of PVAN in heart transplant recipients, which brings the total of reported cases to 7. We briefly review the literature on the hypothesized causes of PVAN in kidney transplant recipi

Hematopoietic stem cell transplantation for infantile osteopetrosis.

We report the international experience in outcomes after related and unrelated hematopoietic transplantation for infantile osteopetrosis in 193 patients. Thirty-four percent of transplants used grafts from HLA-matched siblings, 13% from HLA-mismatched relatives, 12% from HLA-matched, and 41% from HLA-mismatched unrelated donors. The median age at transplantation was 12 months. Busulfan and cyclophosphamide was the most common conditioning regimen. Long-term survival was higher after HLA-matched sibling compared to alternative donor transplantation. There were no differences in survival after H…

Contrast imaging study of the horseshoe kidney for transplantation.

In the surgical setting, horseshoe kidneys (HKs) may be a cause for confusion because of their complicated morphology, especially in the vicinity of the vascular and urinary collecting systems around the isthmus of the HK. In the patients with HK, analysis of the anatomical structure of the isthmus is both useful and necessary. The aim of this study is to observe the vascular and collecting system of the HK using anatomical and contrast imaging technique, then make use of the knowledge for clinical anatomy. A HK voluntarily donated post-mortem to our department in 2013 by an 80-year-old woman
