[Psychosocial effects in patients with penile cancer : A systematic review].

BACKGROUND: Psychological stress of patients with penile cancer arises from the cancer diagnosis itself and the treatment consequences. In addition, there is cancer-specific distress. There is the chance of cure in localized stages and in those with limited regional lymph node metastases but this requires surgery and often adjuvant chemotherapy. This systematic review gives a summary of the existing literature to date. MATERIALS AND METHODS: A critical database search using Medline was made in Ovid from 1946 to 2017, in the Cochrane Central Register of Controlled Trials (CENTRAL) and in the W

Discovery of novel inhibitors for the treatment of glaucoma.

INTRODUCTION: Glaucoma is a neurodegenerative disease with heterogeneous causes that result in retinal ganglionic cell (RGC) death. The discovery of ocular antihypertensives has shifted glaucoma therapy, largely, from surgery to medical intervention. Indeed, several intraocular pressure (IOP)-lowering drugs, with different mechanisms of action and RGC protective property, have been developed. AREAS COVERED: In this review, the authors discuss the main new class of kinase inhibitors used as glaucoma treatments, which lower IOP by enhancing drainage and/or lowering production of aqueous humor.

Hypothalamic-pituitary-adrenocortical axis: neuropsychiatric aspects.

Evidence of aberrant hypothalamic-pituitary-adrenocortical (HPA) activity in many psychiatric disorders, although not universal, has sparked long-standing interest in HPA hormones as biomarkers of disease or treatment response. HPA activity may be chronically elevated in melancholic depression, panic disorder, obsessive-compulsive disorder, and schizophrenia. The HPA axis may be more reactive to stress in social anxiety disorder and autism spectrum disorders. In contrast, HPA activity is more likely to be low in PTSD and atypical depression. Antidepressants are widely considered to inhibit HPA

Beyond the guidelines in the treatment of peripheral T-cell lymphoma: new drug development.

Peripheral T-cell lymphomas (PTCLs) are a rare and diverse group of neoplasms with a poor prognosis. Management of these disorders has been largely extrapolated from the treatment of aggressive B-cell lymphomas; however, therapeutic responses to this approach are neither adequate nor durable for most patients with PTCL. Given the rarity of PTCL, much of the literature consists of studies with small sample size and anecdotal case reports. Therefore, no consensus exists on the best therapeutic strategy for either newly diagnosed or relapsed/refractory PTCL. This article reviews promising novel a

[Continuous therapy with albendazole for hepatic alveolar echinococcosis associated with obstructive jaundice].

To observe clinical results of continuous therapy with albendazole for hepatic alveolar echinococcosis associated with obstructive jaundice, we treated 6 patients continuously with albendazole at a dosage of 20 mg.kg-1.d-1. Jaundice disappeared and serum bilirubin level returned to normal within 1-2.5 months in the patients. Ultrasound and/or CT scanning of liver before treatment displayed irregular heterogenous lesions, obscure hilar region with marked intrahepatic biliary dilatation. In one severe jaundiced patient, the common bile duct and the pancreatic duct were dilated and the portal and

Patient-specific targeted bronchial thermoplasty: predictions of improved outcomes with structure-guided treatment.

Bronchial thermoplasty is a recent treatment for asthma in which ablative thermal energy is delivered to specific large airways according to clinical guidelines. Therefore, current practice is effectively "blind," as it is not informed by patient-specific data. The present study seeks to establish whether a patient-specific approach based on structural or functional patient data can improve outcomes and/or reduce the number of procedures required for clinical efficacy. We employed a combination of extensive human lung specimens and novel computational methods to predict bronchial thermoplasty

Local Promoter Methylation Disorder algorithm reveals bidirectional epigenetic disruption in DNMT3A-mutated AML and predicts azacitidine treatment response.

BACKGROUND: DNMT3A mutations occur in 20-25% of acute myeloid leukemia (AML) cases and are associated with poor prognosis, yet the epigenetic mechanisms underlying treatment response remain poorly understood. Traditional methylation analyses focus on mean levels, overlooking the heterogeneity that may be central to therapeutic vulnerability. We developed a Local Promoter Methylation Disorder (LPMD) algorithm to quantify methylation instability and evaluate its clinical utility in predicting azacitidine response. METHODS: The LPMD algorithm was developed using the GSE62298 dataset (68 AML pati

Efficacy of non-surgical, non-pharmacological treatments for congenital muscular torticollis: a systematic review and meta-analysis.

BACKGROUND: Congenital Muscular Torticollis (CMT) is the third most common musculoskeletal condition in infancy, and if untreated can lead to significant disability. While a range of conservative treatments are commonly used in the management of CMT, an understanding of their efficacy and safety is limited. This systematic review and meta-analysis, without language or discipline restriction, was conducted to address this knowledge gap. METHODS: Electronic searches of CENTRAL, PubMed, 22 other electronic databases, three trials registers and Google Scholar, were conducted for randomised contro

PRRC2A-mediated m6A modification on NCOA4 promotes thymic epithelial tumor progression through PKM2-induced glycolysis.

Thymic epithelial tumors (TETs) are heterogeneous neoplasms. While most early-stage cases are curable after resection, advanced or aggressive subtypes have limited treatment options and poor outcomes. The molecular mechanisms underlying their metabolic reprogramming remain poorly understood. Proline-rich Coiled-coil 2a (PRRC2A), a N6-methyladenosine (m6A) reader protein, is significantly upregulated in TET tissues and is associated with poor clinical outcomes. Multi-omics analyses and molecular experiments revealed that PRRC2A binds to m6A-modified sites within the NCOA4 mRNA, thereby stabiliz

Modern multidisciplinary management of brain metastasis.

Brain metastases (BrM) are no longer considered to be a uniform end-stage complication of cancer, but as a biologically and clinically heterogeneous disease requiring coordinated management. This article synthesises the fundamentals of BrM diagnosis and treatment, relevant to primary and urgent care settings. Treatment sequencing should account for acuity of neurologic symptoms, anticipated intracranial response, extracranial disease tempo, steroid requirements, and risks of local therapy. Focal radiotherapy is now the dominant local modality for limited BrM. Surgery remains central for large,

Advances in Delta-like Ligand 3-Targeted Diagnosis and Treatment.

Neuroendocrine neoplasms (NENs) are rare clinical entities that have long posed significant challenges in diagnosis and treatment due to their high histological heterogeneity, a scarcity of effective therapeutic targets, and consequently, limited treatment options. Delta-like ligand 3 (DLL3), an inhibitory ligand of the Notch signaling pathway, plays a crucial role in neuroendocrine tumors and is highly expressed in malignancies such as small-cell lung cancer and small-cell neuroendocrine prostate cancer. With the continuous advancement of DLL3-targeted technologies in recent years, an increas

Use of multisensory information as facilitator of group schema therapy for borderline personality disorder.

BACKGROUND: Group schema therapy (GST) has been found to be an effective form of treatment for borderline personality disorder (BPD) patients. Yet high drop-out rates and heterogenous treatment responses remain challenges in the real-life clinical care of BPD. METHOD: We suggest a novel method for facilitating BPD group therapy using recordings (videos) made by patients at home according to the therapists' instructions and watched together in the group session. Patient examines his/her observations on the video in the group under the guidance of psychotherapist. These video recordings are use

Changing understanding of acromegaly epidemiology and early mortality risk.

Epidemiologic research for rare diseases such as acromegaly is challenging due to low prevalence, heterogeneous data sources, and regional variability. Here, we review recent epidemiologic studies and provide a synthesis of the changing landscape of acromegaly and an overview of mortality rates and their determinants. Over the past few decades, the reported incidence and prevalence of acromegaly have increased, likely due to improved diagnostic tools, earlier diagnosis, and more efficient management of the disease, leading to increased life expectancy. Available data suggest that the delay in

New learnings from the molecular pathology of the synovial tissue in rheumatoid arthritis: from pathogenesis to therapeutic targeting toward precision medicine.

Despite the success of targeted therapies in rheumatoid arthritis, the lack of predictive biomarkers of response leads to an empirical treatment approach, often delaying effective intervention due to non-response to the initially selected individualized medication regimens in approximately 40% of patients. Cellular and molecular deconvolution of synovial tissue heterogeneity reveals discrete disease phenotypes, enabling disease stratification and response prediction. Recent advances in single-cell RNA sequencing and mass cytometry have defined more refined synovial molecular signatures and cel

Parallels between bipolar disorder and ATP1A3-related diseases: a window into the investigation of lithium for alternating hemiplegia of childhood.

BACKGROUND: Alternating hemiplegia of childhood (AHC) shares many aspects with the psychiatric condition, bipolar disorder. BODY: AHC is a heterogeneous genetic disorder that can manifest in various fashions but will usually involve paroxysmal neuropsychiatric symptoms that are precipitated by environmental events or stressors, superimposed over developmental abnormalities of various severities. It occurs as a consequence of a variety of mutations of the alpha3 subunit of the sodium pump (ATP1A3). Treatment is generally symptomatic with flunarizine a nonselective calcium channel blocker, but o

Resistance breakers: a novel approach to tackle biofilm associated infections and antimicrobial resistance.

Antimicrobial resistance (AMR) has emerged as one of the greatest global health concerns and its threat to effective treatment of infectious diseases is accelerating. Biofilms remain a major contributor to antimicrobial resistance and persistence of infections. Bacterial biofilms are sessile communities of bacteria surrounded by a self-produced extracellular polymeric substance (EPS). Bacterial cells in biofilms are less susceptible to conventional antibiotics and to host immune effector mechanisms as the cells are protected by the EPS, exhibit altered gene expression as well as heterogeneity

Efficacy of Lebrikizumab on Pruritus: A Narrative Review.

Atopic dermatitis (AD) is a chronic, relapsing, and heterogeneous skin disease characterized by eczematous morphology and intense pruritus, significantly impacting the quality of life of affected individuals. A primary cytokine implicated in AD is interleukin-13 (IL-13), which directly drives pruritus skin sensitization, contributing to pruritus. Lebrikizumab, a high-affinity IgG4 monoclonal antibody, targets IL-13 to block inflammatory and neuronal sensitization processes. This review aims to provide a comprehensive summary of lebrikizumab's efficacy in managing pruritus in patients with AD,
