Point-of-care ultrasound reveals extensive pathology in Gabonese preschool-age children with urogenital schistosomiasis.

BACKGROUND: Historically, urinary tract pathology caused by S. haematobium infection was thought to affect predominantly school-age children (SAC) and adults. Increasing availability of ultrasound data from endemic areas demonstrates that even younger children develop potentially irreversible pathology. METHODOLOGY/PRINCIPAL FINDINGS: Point-of-care ultrasound for urinary schistosomiasis and urine microscopy were performed across age groups in 105 patients with symptomatic urogenital schistosomiasis in Lambaréné, Gabon. Of 96 ultrasound scans with sufficient image quality, bladder wall thicken

Postoperative Urinary Catheterization Thresholds of 500 versus 800 ml after Fast-track Total Hip and Knee Arthroplasty: A Randomized, Open-label, Controlled Trial.

BACKGROUND: No evidence-based threshold exists for postoperative urinary bladder catheterization. The authors hypothesized that a catheterization threshold of 800 ml was superior to 500 ml in reducing postoperative urinary catheterization and urological complications after fast-track total hip arthroplasty (THA) and total knee arthroplasty (TKA). METHODS: This was a randomized, controlled, open-label trial that included patients greater than or equal to 18 yr who underwent THA or TKA in three Danish, fast-track, orthopedic departments. Consenting patients were eligible if they were cooperativ…

Preventing hospital-acquired urinary tract infection in the United States: a national study.

BACKGROUND: Although urinary tract infection (UTI) is the most common hospital-acquired infection in the United States, to our knowledge, no national data exist describing what hospitals in the United States are doing to prevent this patient safety problem. We conducted a national study to examine the current practices used by hospitals to prevent hospital-acquired UTI. METHODS: We mailed written surveys to infection control coordinators at a national random sample of nonfederal US hospitals with an intensive care unit and >or=50 hospital beds (n=600) and to all Veterans Affairs (VA) hospital

Urinary dysfunction in patients with systemic lupus erythematosis.

AIMS: Nervous system involvement occurs in about half of patients with systemic lupus erythematosis (SLE). Seizures and psychiatric disorders are the most common manifestations; spinal cord lesions are uncommon. We had eight such patients who presented with urinary dysfunction. METHODS: The patients consisted of two men and six women, mean age 42 years, range 28-72 years. All patients were suffering from SLE for 2-25 years under immunosuppressant therapy. Their neurological manifestations were subacute encephalomyelopathy in three, subacute myelopathy in one, and chronic myelopathy in four. U

Inflammatory Myofibroblastic Tumour of the Bladder in a Young Male: A Rare Case Report.

Inflammatory myofibroblastic tumour (IMT) of the urinary bladder is a rare mesenchymal neoplasm characterised by spindle-shaped myofibroblastic cells and an accompanying inflammatory infiltrate. Although its aetiology remains unclear, histopathological and immunohistochemical analyses are essential for diagnosis. We report the case of an 18-year-old male who presented with sudden-onset, painless gross haematuria. Initial evaluation revealed severe anaemia, necessitating blood transfusion. Renal ultrasound demonstrated a large echogenic lesion within the bladder. Cystoscopy revealed a large, po…

A randomized controlled trial of cystoinflation to prevent bladder injury in the adhesive disease of multiple caesarean sections.

Caesarean sections carry the risk of urinary bladder injury due to formation of adhesions obscuring pelvic planes. Visualizing bladder during retro-fill (cystoinflation) makes it recognizable as it rises into the abdomen taking a tense rounded contour. We conducted a prospective randomized controlled trial to find out whether improved identification of bladder margins by cystoinflation could decrease bladder injury rate and blood loss without causing urological complications. This study included 214 healthy women with previous operative deliveries undergoing elective caesarean section and foun…

Roles of mesenchymal stem cells and exosomes in interstitial cystitis/bladder pain syndrome.

Interstitial cystitis/bladder pain syndrome (IC/BPS) is characterized by several symptoms of higher sensitivity of the lower urinary tract, such as bladder pain/discomfort, urgency, urinary frequency, pelvic pain and nocturia. Although the pathophysiology of IC/BPS is not fully understood, the hypothesis suggests that mast cell activation, glycosaminoglycan (GAG) layer defects, urothelium permeability disruption, inflammation, autoimmune disorder and infection are potential mechanisms. Mesenchymal stem cells (MSCs) have been proven to protect against tissue injury in IC/BPS by migrating into b

Xanthogranulomatous cystitis and bilateral ureteral reflux due to bladder neck obstruction: a case report.

Xanthogranulomatous cystitis (XC) is a rare benign disease of chronic granulomatous inflammation. We report a 23-year-old woman with xanthogranulomatous cystitis. She was referred to our hospital with the chief complaint of a 1-year history of frequent, urgent dysuria with recurrent fever. An imaging examination showed bilateral ureteral reflux and a normal bladder. Urodynamic findings suggested bladder outlet obstruction and increased post-void residual urine. Finally, the patient underwent endoscopy, and bladder neck obstruction was confirmed. Additionally, we found multiple granulomatous ma

Primary bladder neck obstruction in female: 'an enigmatic disorder'.

A 36-year-old woman presented with problems of lower urinary tract symptoms for 2 years duration. Patient was being managed as a case of urethral stricture with routine calibration. Micturating cystourethrogram showed failure of bladder neck to open. On urodynamic study, she was found to have bladder outlet obstruction with high pressure, low flow pattern. Based on these findings, patient was diagnosed to have primary bladder neck obstruction (PBNO). She was also being evaluated for primary infertility and was to undergo in vitro fertilisation. She successfully underwent bladder neck incision

Feasibility of a deep learning-based diagnostic platform to evaluate lower urinary tract disorders in men using simple uroflowmetry.

PURPOSE: To diagnose lower urinary tract symptoms (LUTS) in a noninvasive manner, we created a prediction model for bladder outlet obstruction (BOO) and detrusor underactivity (DUA) using simple uroflowmetry. In this study, we used deep learning to analyze simple uroflowmetry. MATERIALS AND METHODS: We performed a retrospective review of 4,835 male patients aged ≥40 years who underwent a urodynamic study at a single center. We excluded patients with a disease or a history of surgery that could affect LUTS. A total of 1,792 patients were included in the study. We extracted a simple uroflowmetr

Compensatory bladder behaviors ("coping") in women with overactive bladder.

AIMS: There is little information on how often or within what contexts individuals with OAB use compensatory behaviors ("coping") to manage symptoms. We sought to examine how frequently women with OAB report using coping behaviors and whether these are associated with psychosocial factors. METHODS: One hundred twenty adult women with OAB completed the OAB questionnaire (OAB-q), ICIQ-FLUTS questionnaire, PROMIS Anxiety and Depression, Perceived Stress Scale, patient perception of bladder condition, and demographic and clinical data. Responses from five items from the OAB-q Quality-of-Life scal

Vesicocutaneous fistula: continent vesicostomy, an easier route for comfortable clean intermittent catheterization.

INTRODUCTION: Clean intermittent catheterization (CIC) through the urethra is the treatment of choice for patients with neurogenic bladder (NGB) or other etiologies that lead to incomplete bladder emptying. However, urethral catheterization can be problematic. Vesicocutaneous fistula (VCF) is a continent catheterizable channel with a low rate of complications. The aim of the study was to evaluate the safety and effectiveness of VCF as a route for CIC. MATERIAL AND METHODS: The authors retrospectively reviewed patients who underwent creation of the VCF for bladder drainage from November 2001 t

An adolescent boy progressing insidiously to end-stage renal disease: Answers.

Hinman syndrome was a non-neurogenic neurogenic bladder and the most severe form of dysfunctional voiding disorder. The bladder-sphincter discoordination causes damage to the bladder and upper urinary tract if it is not diagnosed early and treated adequately. This case emphasizes the following important message: nighttime wetting is not a benign condition in every child. Parental awareness should be raised about voiding disorders, as it may be possible to prevent important renal diseases such as Hinman syndrome.
