Treatment of Behçet's disease.

Behçet's disease is a systemic inflammatory disorder capable of inducing thrombophlebitis. Clinicopathologically, it is characterized by neu … ve helped to achieve remarkable progress in treatment. Particularly notable is the effect on severe uveitis of cyclosporine, which is more effective than colchicine and conventional immunosuppressants. With the advent of cyclosporine, the number of patients with Behçet's disease who lose their eyesight has clearly decreased.

Ocular disease associated with Epstein-Barr virus infection.

Epstein-Barr virus (EBV) is a ubiquitous DNA virus of the herpesvirus genus with a high prevalence rate for antibody (about 90%) in the adult population. It is the most common causative agent of infectious mononucleosis syndrome. During recent years an increasing number of ocular disease entities have been reported to be linked to EBV infection. These entities include oculoglandular syndrome, conjunctivitis, dry eye, keratitis, uveitis, choroiditis, retinitis, papillitis and ophthalmoplegia. While EBV-specific serologic tests can now document recent and past primary infection with EBV and also…

Chorioretinal scars in Fuchs' heterochromic iridocyclitis.

To determine the incidence of chorioretinal lesions in patients with Fuchs' heterochromic iridocyclitis, we reviewed the records of all patients with this disease seen at the Indiana University Uveitis Service, Indianapolis, since 1963. For comparison the records of age- and sex-matched patients with HLA-B27-positive iritis were also reviewed. The incidence of chorioretinal lesions in one or both eyes of patients with Fuchs' heterochromic iridocyclitis was significantly higher than in the patients with HLA-B27-positive iritis or than expected in the general population. Among the types of lesio…

Immunoregulation by alpha 1 antitrypsin.

alpha 1 antitrypsin (alpha 1 AT) deficiency is a common genetic disorder seen in about 10% of the population. It predisposes to the development of a large number of inflammatory and immunologic disorders including rheumatoid arthritis, systemic lupus erythematosus, juvenile chronic arthritis, anterior uveitis, ankylosing spondylitis, fibrosing alveolitis and emphysema. We have investigated immunologic function in subjects with severe alpha 1 AT deficiency and demonstrated serum mediated enhancement of lymphocyte response to PHA and increased zymosan activation of mononuclear cells and neutroph…

Angioimmunoblastic lymphadenopathy with retinitis and drug related exacerbations: a clinicopathological case study.

A patient with angioimmunoblastic lymphadenopathy with dysproteinemia was followed over a three year period from diagnosis to death. He presented with arthralgias, uveitis and respiratory insufficiency and developed hyperuricemic renal failure upon institution of treatment. Aggressive combination chemotherapy was required to reverse progressive thrombocytopenia and pulmonary involvement. A complete remission was achieved twice. There was a striking temporal relationship between the administration of antibiotics or allopurinol and exacerbations of the disease. Hypocomplementemia and transient e…

Ocular abnormalities occurring with vitiligo.

One hundred twelve patients with vitiligo were examined for ocular abnormalities. Discrete areas of depigmentation with associated pigment hyperplasia clinically appearing to involve the choroid and retinal pigment epithelium were observed in 44 patients, and active uveitis was seen in nine patients. The changes observed suggest that the spectrum of diseases that includes Harada's disease and the Vogt-Koyanagi syndrome may be broader than previously appreciated. Patients with these syndromes may represent the most severe examples of vitiligo and uveal inflammation. The occurrence of symptoms o…

The ocular manifestations of Wegener's granulomatosis. Fifteen years experience and review of the literature.

Ocular manifestations of Wegener's granulomatosis may occur secondary to contiguous granulomatous sinusitis or as a result of focal vasculitis. Contiguous granulomatous sinus disease causes nasolacrimal duct obstruction, proptosis and ocular muscle or optic nerve involvement. Focal vasculitis unrelated to contiguous upper respiratory tract disease is manifested by conjunctivitis, episcleritis, scleritis, corneoscleral ulceration, uveitis, and granulomatous vasculitis of the retina and optic nerve. A review of 29 cases of Wegener's granulomatosis and three cases of lymphomatoid granulomatosis s…

Evaluation of eyes enucleated for scleritis.

A series of 30 enucleated eyes, all of which had a primary histological diagnosis of scleritis, was analysed. The average age of patients at enucleation was 68 years; many of these patients had had the disease for more than 30 years. In 40 per cent the diagnosis of scleritis was unsuspected and was often masked by multiple complications. Scleritis with uveitis and glaucoma was the most common combination to come to enucleation. In 82 per cent, pain was the reason for enucleation, which suggests that some patients were on inadequate levels of steroid treatment or were unable to tolerate them. I…

Lymphocytic choriomeningitis virus-induced ocular immunopathology.

Lymphocytic choriomeningitis (LCM) virus of the mice provides perhaps the most extensively studied model of viral-induced immunopathology. Intraocular inoculation of this virus in the adult mouse produces a severe and progressive uveitis with involvement also of the cornea and retina. Intracerebral infection of the newborn rat with LCM virus produces a severe retinopathy, with only minimal involvement of the uveal tract. We review in this paper recent studies on these two animal models of LCM virus disease which demonstrate: (1) the histopathology of the ocular lesion; (2) that immunosuppressi…

A Case of Sarcoidal Granulomatous Dermatitis and Vasculitis in a 28-Year-Old Woman.

Granulomatous vasculitis represents a rare cutaneous manifestation of sarcoidosis, a multisystem disease characterized by noncaseating granulomas. We report the case of a 28-year-old woman with new-onset tender, nonpruritic, erythematous papules coalescing into plaques and subcutaneous nodules on her lower legs, accompanied by anterior uveitis, exertional dyspnea, and constitutional symptoms including fatigue and night sweats. Punch biopsy revealed non-necrotizing epithelioid granulomas with vasculitic changes, consistent with cutaneous sarcoidal granulomatous vasculitis. Although granulomatou…
