Amide proton transfer imaging reveals cerebral metabolic alterations associated with cognitive impairment in type 2 diabetes mellitus.

BACKGROUND: Amide proton transfer (APT) imaging indirectly reflects tissue metabolic changes by detecting variations in the concentration of mobile amide protons and tissue pH. Type 2 diabetes mellitus (T2DM) is often accompanied by cognitive dysfunction and diabetic encephalopathy, both of which pose serious threat to human health and quality of life. This study aimed to evaluate the potential of APT imaging as a novel biomarker for detecting cerebral metabolic alterations and to investigate its associations with cognitive impairment in patients with T2DM. METHODS: This study included 32 T2D

Alternative projections of mortality and disability by cause 1990-2020: Global Burden of Disease Study.

BACKGROUND: Plausible projections of future mortality and disability are a useful aid in decisions on priorities for health research, capital investment, and training. Rates and patterns of ill health are determined by factors such as socioeconomic development, educational attainment, technological developments, and their dispersion among populations, as well as exposure to hazards such as tobacco. As part of the Global Burden of Disease Study (GBD), we developed three scenarios of future mortality and disability for different age-sex groups, causes, and regions. METHODS: We used the most imp

Serious Illness Conversations in the Emergency Department for Older Adults With Advanced Illnesses: A Randomized Clinical Trial.

IMPORTANCE: Conversations between seriously ill patients and clinicians about values and goals (ie, serious illness conversations [SICs]) can lead to patient-centered care toward the end of life. However, many patients have not had SICs when arriving in the emergency department (ED) and are at risk of receiving undesired care. OBJECTIVE: To determine the effect of an ED-based, multimodal SIC intervention (known as ED GOAL) compared with usual care on patient-reported engagement in advance care planning (ACP) and clinician-documented SICs after leaving the ED. DESIGN, SETTING, AND PARTICIPANT

Does Adding TENS to Pelvic Floor Rehabilitation Effect on Urodynamics and Clinical Results in Children With Spina Bifida?

INTRODUCTION: Children with spina bifida (SB) experience a reduced quality of life due to neurogenic bladder dysfunction and the risk of renal failure later in life. Pelvic floor rehabilitation (PFR) and transcutaneous electrical nerve stimulation (TENS) have gained prominence in recent years due to their positive effects in treating different types of bladder issues in both adults and children. Our study aims to investigate the effects of using these two methods together in children with SB and to delineate their potential value for clinical practice. MATERIALS AND METHODS: Children with neu

Stereotactic radiation therapy for oligometastases or oligorecurrence within mediastinal lymph nodes.

BACKGROUND: Clean intermittent catheterization (CIC) is an established treatment for lower urinary tract dysfunction and has been used across age groups with voiding problems due to various etiologies. Although medically safe and straightforward, CIC may create social and emotional challenges for adolescents and their caregivers. During adolescence, when autonomy and peer acceptance become central, the need for continuous catheterization in daily life and, at times, dependence on another person can be difficult to cope with and may affect psychological well-being and peer relations. Shared Decision Making and Health-Related Quality of Life in Atopic Dermatitis and Chronic Urticaria.

Shared decision-making (SDM) between patients and healthcare professionals is a patient-centered approach that integrates clinical evidence with patient values and preferences to arrive at healthcare decisions. The major components of shared decision making include seeking the patient's participation and forming a partnership, helping the patient explore and compare treatment options, learning about the patient and their preferences, creating choice awareness, and jointly making and evaluating management decisions. Goals of treatment for chronic spontaneous urticaria (CSU) and atopic dermatiti

The hemoglobin-red blood cell distribution width ratio is a reliable predictor of recurrence of ketoacidosis in patients with type 2 diabetes.

BACKGROUND: Diabetic ketoacidosis is a common disease in the emergency department, characterized by severe metabolic disorders, which poses a significant threat to the life and health of patients with type 2 diabetes. Hemoglobin reflects the body's oxygen-carrying capacity and nutritional status, while the red cell distribution width indicates the heterogeneity in the size of red blood cells. The ratio of these two indicators can comprehensively reflect the systemic metabolic status and inflammatory state of patients with diabetic ketoacidosis. OBJECTIVES: To study the association between the

Fly larvae as an alternative sample for the detection of highly pathogenic avian influenza A(H5N1) virus.

Altered carcasses present a diagnostic challenge to veterinary pathologists. Autolysis and putrefaction may render organs unrecognizable, and scavenging by vertebrates and insects may remove organs entirely. Sampling for viral or bacterial testing is greatly hindered and determination of an infectious cause of death is nearly impossible. An outbreak of highly pathogenic avian influenza (HPAI) in California condors in the spring of 2023 offered an opportunity to explore the utility of blow fly larvae (maggots) as an alternative test sample. An 18-y-old male condor that died during the outbreak

Medical decision analysis for personalized oncology at the patient bedside.

Medical decision analysis is a method to make rational clinical decisions under uncertainty, enabling a mathematical combination of probabilities and utilities (i.e. values assigned to outcomes under risk). Decision analysis is commonly used in health economics, but it is underexploited in the clinic. With a view to fostering the use of medical decision analysis at the cancer patient bedside, this paper provides basic templates for some typical clinical decisions in cancer treatment, namely affecting: the quantity/quality of life trade-offs in curable cancer; adjuvant/neoadjuvant treatments; c

An error-compensated immersion and invariance adaptive estimator for hammerstein neural networks with binary-valued observations.

Designing a parameter recovery algorithm using binary-valued observations that can handle the quantized identification problem of a nonlinear system is essential for various engineering processes including spectrum sensing to network communication, automatic control, and cyber-physical systems. This study addresses the parameter estimation problem based on binary-valued measurements for Hammerstein neural networks by constructing an error-compensated immersion and invariance (I&I)-based estimation method. First, based on a filter operator and half-decomposition technique, a filtered identifica

Neuromonitoring in the Decompressed Brain: Rethinking Physiology, Monitoring, and Thresholds After Decompressive Craniectomy.

Decompressive craniectomy (DC) is a life-saving intervention for refractory intracranial hypertension across traumatic, ischemic, and hemorrhagic etiologies. While its immediate hemodynamic benefits are well established, DC fundamentally disrupts intracranial physiology by abolishing the closed-box condition of the cranial vault, thereby altering the interpretation of all monitored parameters. Neuromonitoring (NM) after DC remains a largely unaddressed domain in current consensus statements and evidence-based guidelines. This viewpoint synthesizes available evidence to address five critical cl

Public deliberation on health gain measures.

Researchers and decision-makers use health gain measures to assess the value of health interventions. However, our current understanding of how these measures are understandable and accessible to the community is limited. This study examined a diverse group of stakeholders' attitudes and preferences for 9 commonly used health gain measures. We recruited 20 stakeholders, including patients, caregivers, pharmacists, allied health professionals, and citizens. We conducted 2 in-person deliberative meetings in which participants learned, discussed, deliberated on, and ranked 9 health gain measures.

Desire for a sense of power and religious suicide in psychobiographical research: combining personality and sociocultural theories.

The article presents two theoretical perspectives that provide a helpful framework in psychobiographical research, especially when psychobiographies concern religious suicide. The first is typical in contemporary psychology, a subjective analysis focused on the individual, looking at life course/lifetime in the light of personality psychology. The second one is represented by anthropological research on the concept of honour-shame and the sociological works of E. Durkheim. Contemporary psychobiography should consider sociocultural context and refer to social sciences (anthropology, sociology).
