Complement receptor 3 (CR3) is expressed on cells of the reticuloendothelial system and involved in the clearance of immune complexes. In this article a patient with a deficiency of the C3bi binding site of this receptor is described. Clinically this patient exhibited predominantly cutaneous manifestations of a systemic lupus erythematosus with an immune vasculitis and panniculitis. The deficiency of the CR3 epitope was demonstrated using flow cytometry. The functional relevance of this defect was demonstrated in a rosetting assay with C3bi-loaded erythrocytes. C3bi binding was found to be sig…

The Churg-Strauss syndrome is part of the necrotizing vasculitis group of diseases. Its clinical diagnosis rests on the occurrence of a severe asthma followed, more or less closely, by systemic manifestations that are predominantly neurological, cutaneous, gastrointestinal and cardiovascular. To these must be added pulmonary infiltrates, eosinophilia and increase in serum IgE's. Three histological criteria (necrotizing vasculitis of the small vessels, perivascular infiltration with numerous eosinophils and extravascular granulomas) enable this anatomico-clinical syndrome to be identified, but …

A group of patients has been identified with cutaneous necrotizing venulitis (vasculitis). These patients, some with concomitant connective tissue disorders, have skin lesions that separate them from the arteritis commonly described as rheumatoid vasculitis. HLA typing has been performed on 31 of these unrelated patients with cutaneous necrotizing venulitis, including 19 with associated chronic disorders. The antigen pair A11, BW35 was found in 5 of these 19 patients and in 11 of 346 controls. This difference in frequency is statistically significant. Because HLA genes appear to be linked to i…

Hansen's disease, also known as leprosy, is often termed "the great imitator" due to its diverse clinical presentations that can mimic various rheumatologic disorders. We present the case of a 34-year-old female who developed extensive purpuric rashes, initially raising suspicion of vasculitis. Laboratory investigations revealed triple-positive antiphospholipid antibodies. However, skin smears and histopathological examination confirmed a diagnosis of diffuse lepromatous leprosy complicated by Lucio phenomenon. This case highlights the importance of considering infectious etiologies, such as l…

Granulomatosis with polyangiitis (GPA) is a subtype of anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) that commonly requires aggressive immunosuppression to achieve remission. We present a case of a young Malay lady with recurrent episodes of ANCA-positive nodular anterior scleritis who responded poorly to topical and systemic corticosteroids and relapsed while on methotrexate. A year later, she had epistaxis, and a sino-nasal biopsy confirmed granulomatous vasculitis. While receiving cyclophosphamide, she developed proptosis with optic neuropathy, which resolved with …

Immunoglobulin A (IgA) nephropathy, mesangial deposition of IgA in renal parenchyma, typically presents with hematuria and proteinuria. Leukocytoclastic vasculitis (LCV), a small-vessel vasculitis, can present secondary to IgA. We will discuss a case of secondary IgA nephropathy with concomitant LCV in a patient with reactivated hepatitis C. A 55-year-old male with decompensated alcoholic cirrhosis presented for a bilateral lower-extremity rash. The patient was diagnosed with IgA nephropathy, by kidney biopsy, and skin biopsy showing LCV. Further investigation revealed hepatitis C viral load w…

A 55-year-old healthy man presented with redness and pain in both eyes and was diagnosed with bilateral scleritis. A year later impairment of ocular movement and cervical adenopathy appeared. Laboratory tests revealed signs of inflammation: increased erythrocyte sedimentation rate, lymphocytosis, lactate dehydrogenase, S-ACE converting enzyme and interleukin-2-antibody. An orbital MR-scan revealed inflammation of the extra-ocular muscles. PET-CT showed vascular changes consistent with vasculitis. Lymph node biopsy confirmed the diagnosis chronic lymphatic leukaemia (CLL). This is a rare case -…

Polyarteritis nodosa (PN) is vasculitis of small- to medium-sized arteries. A 57-year-old woman with PN developed aortic valve regurgitation. Aortic valve replacement (AVR) was performed. We found inflammatory change of the ascending aorta which was rarely involved in PN. Abnormal thickness of the aortic wall was recognized during operation, which had not been detected by preoperative computed tomography. Perioperative course was uneventful, and the patient underwent anti-inflammatory therapy after the operation. No perivalvular leakage has been detected for 3 years since AVR. We should consid…

In a 47-year-old man presenting with chronic bronchitis since 1978, a weight loss of 11 kg in 9 months and deteriorated general condition, the x-ray film of the thorax revealed numerous pulmonary nodules of up to 4 cm size. Laboratory findings were characterised by enhanced ESR, blood eosinophilia of 7% and a lot of eosinophilic granulocytes in the sputum. The findings could not be clarified despite many additional examinations including bronchoscopy with transbronchial biopsy. Thoracotomy was finally considered imperative. Pulmonary biopsy revealed necrotising vasculitis, eosinophilic infiltr…

BACKGROUND: Dysthyroid optic neuropathy is the most commonly suspected diagnosis of optic neuropathy in Graves' patients; however, other causes need to be ruled out. We present a unique case of optic neuropathy secondary to hypertrophic pachymeningitis with antineutrophil cytoplasmic antibody-associated vasculitis, which was suspected to be antithyroid drug related.
CASE PRESENTATION: A 79-year-old Japanese male presented with acute visual loss in the left eye. He had a 24-year history of Graves' disease and was taking methimazole. Best-corrected visual acuity was 0.8 in the right eye and lig…

PURPOSE: To examine histological changes within nasal and lacrimal sac mucosa harvested from patients with granulomatous polyangiitis (GPA) who required lacrimal drainage surgery. As these patients were considered quiescent on clinical and serological grounds, particular attention was paid to the presence of active vasculitis.
DESIGN: Retrospective noncomparative case series.
PARTICIPANTS: Patients with GPA who had open lacrimal drainage surgery under the care of a single surgeon between 1991 and 2017. All patients had paired biopsies of both nasal and ipsilateral lacrimal sac mucosa.

Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis is a systemic autoimmune disease characterized by necrotizing inflammation of the small blood vessels. ANCA-associated vasculitis is subclassified into three variants: granulomatosis with polyangiitis, eosinophilic granulomatosis with polyangiitis, and microscopic polyangiitis (MPA). Myeloperoxidase (MPO) ANCA is a marker antibody for MPA. Interstitial pneumonia (IP) is occasionally complicated with MPA. However, only a few cases of idiopathic IP develop MPO-ANCA-positive conversion and MPA. Therefore, we present a case of a 70-y…

We encountered 2 cases of microscopic polyangitis (MPA) with preceding interstitial pneumonia. The patients were elderly men, and radiological findings showed a UIP pattern in both cases. Furthermore, ANCA-associated vasculitis had been suspected since the first admission in case 1, because MPO-ANCA tests were positive, and the surgical lung biopsy revealed vasculitis in a honeycomb lesion. A diagnosis of idiopathic pulmonary fibrosis was made in case 2 because no findings of vasculitis were pathologically confirmed, and MPO-ANCA was not examined. Both patients were followed up without any tre…

Vasculitis can occur either as a primary condition or secondary to CTDs, infection, medication or malignancy. This article reviews the clinical presentation and management of vascular disease associated with SLE and SS, as well as the primary necrotizing vasculitides. Although pulmonary arterial hypertension (PAH) has traditionally been considered a rare complication of SLE, estimates of its prevalence range from 0.5% to 14% and it has a significant impact on prognosis. In contrast to PAH associated with other CTDs, patients with SLE respond well to immunosuppressive agents (cyclophosphamide i…

Wegener's granulomatosis (WG) is a unique clinicopathological disease characterized by necrotizing granulomatous vasculitis of the respiratory tract, pauci-immune necrotizing glomerulonephritis and small-vessel vasculitis. Owing to its wide range of clinical manifestations, WG has a broad spectrum of severity that includes the potential for alveolar hemorrhage or rapidly progressive glomerulonephritis, which are immediately life threatening. WG is associated with the presence of circulating antineutrophil cytoplasm antibodies (c-ANCAs). The most widely accepted pathogenetic model suggests that…

We report a rare case of Churg-Strauss syndrome (CSS) in a 41-year-old Japanese man with a history of middle-age onset of bronchial asthma who had severe abdominal pain. He presented with ileus caused by an annular ulcer of the ileum, attributable to mucosal ischemia resulting from necrotizing vasculitis of the mesenteric artery. He also had marked hypereosinophilia (51.5%), elevated serum IgE levels (34040 IU/ml), and generalized enlargement of the superficial cervical lymph nodes, containing eosinophilic granulomas. A stenotic lesion caused by an annular ulcer in the ileum was found and rese…
