Left atrial appendage closure versus direct oral anticoagulants after pulmonary vein isolation for atrial fibrillation: protocol for a multicentre, prospective, randomised, non-inferiority trial (PROMOTE study).

INTRODUCTION: Atrial fibrillation (AF), with a prevalence of 1-2%, is the most common cardiac arrhythmia. AF is associated with a fivefold increased risk of cardioembolic events; approximately 20% of all strokes are caused by AF. Pulmonary vein isolation (PVI) has become the first-line treatment for AF. However, PVI cannot eliminate the residual stroke risk. Current guidelines recommend that anticoagulation be continued in this specific group of patients, regardless of the presence or absence of AF. In this large AF population post-PVI, who are considered to be in an earlier stage of AF, it is

Letter to the editor on "Topical antibiotic powder reduces fracture-related infections in pelvic and lower limb fractures: a systematic review and meta-analysis": the denominator matters.

PURPOSE: To clarify the interpretation of the reported increase in Gram-negative organisms after prophylactic topical antibiotic powder in a recent systematic review and meta-analysis. METHODS: We examined the published report and its supplementary bacterial-profile forest plot, compared the denominators used for microbiological outcomes with the full treatment-cohort denominators, and evaluated the resulting estimand in light of potential index-event bias and contemporary randomized evidence. RESULTS: Supplementary Material S3 used denominators of 56 powder recipients and 181 controls, alth

Efficacy and tolerance of early administration of tranexamic acid in patients with cirrhosis presenting with acute upper gastrointestinal bleeding: a study protocol for a multicentre, randomised, double-blind, placebo-controlled trial (the EXARHOSE study).

INTRODUCTION: The management of acute upper gastrointestinal bleeding (UGIB) is challenging in patients with cirrhosis, as it is responsible for severe complications and high mortality rates. Tranexamic acid (TXA) may help control the bleeding by counterbalancing cirrhosis-related hyperfibrinolysis. Still, there is a lack of unbiased data to conclude on its efficacy. The aim of this study is to evaluate the efficacy of TXA in the early treatment of acute UGIB in patients with cirrhosis. METHODS AND ANALYSIS: This study is a multicentre, randomised, double-blind, placebo-controlled trial, for

Why are ototopical aminoglycosides still first-line therapy for chronic suppurative otitis media? A systematic review and discussion of aminoglycosides versus quinolones.

OBJECTIVE: This systematic review aimed to establish that quinolones are as effective as aminoglycosides when used to treat chronic suppurative otitis media. METHOD: The review included good quality, randomised, controlled trials on human subjects, published in English, that compared topical aminoglycosides with topical quinolones for the treatment of chronic suppurative otitis media. RESULTS: Nine trials met the criteria. Two studies showed a higher clinical cure rate in the quinolone group (93 per cent vs 71 per cent, p = 0.04, and 76 per cent vs 52 per cent, p = 0.009). Four studies showe

A combined analysis of 2 randomized clinical studies of tretinoin gel 0.05% for the treatment of acne.

Acne vulgaris is a widely prevalent skin disorder primarily treated with retinoids, which have been shown to cause skin irritation. This report describes the combined analysis of 2 similar phase 3 studies designed to evaluate the efficacy and safety of an aqueous gel formulation of tretinoin relative to its vehicle (both studies) and a marketed microsphere formulation of tretinoin (one study) for once-daily topical treatment of acne. Randomized participants 10 years and older with mild to moderate acne (N=1537) received tretinoin gel 0.05% (n=674), tretinoin gel microsphere 0.1% (n=376), or ve

Progestogen supplementation to prevent miscarriage: Evidence and controversies.

Progesterone plays a key role in the establishment and maintenance of pregnancy and its deficiency has been associated with early pregnancy loss. Progestogen supplementation has therefore been widely investigated as a preventive strategy against miscarriage, particularly in women with recurrent pregnancy loss or threatened miscarriage or after assisted reproductive technologies. However, the overall evidence remains inconclusive. Meta-analyses of randomized trials, including the large PROMISE and PRISM studies, have not demonstrated a significant overall benefit in live birth rates, although a

Effect of different formulations of heparin analogues on radiotherapy skin markers: A cross-sectional study.

This observational study evaluated the effects of different topical heparinoid formulations on the visibility of Inkbox skin markers used in radiotherapy (RT). Skin markers are essential for accurate patient positioning and precise radiation delivery during RT. Conventional skin markers often suffer from diminished visibility due to sweating, friction, or the use of topical agents. Inkbox®, a plant-based, long-lasting alternative that penetrates the stratum corneum, provides durable and reliable skin markings. Heparinoid preparations, commonly prescribed to prevent radiation-induced dermatitis

Virtual Screening-Guided Discovery of a Selective TRPV1 Pentapeptide Inhibitor with Topical Anti-Allergic Efficacy.

Transient receptor potential vanilloid 1 (TRPV1) channels are critical mediators of cutaneous allergic inflammation, contributing to pruritus, erythema, and hypersensitivity in allergic skin disorders. Despite their therapeutic potential, clinically available TRPV1 inhibitors remain limited, leaving effective treatment options lacking. Here, we focused on a self-constructed virtual pentapeptide library and identified a highly selective TRPV1 inhibitor that demonstrated pronounced anti-allergic effects in human skin assays. Through structure-based virtual screening of approximately 200,000 pept

4-aminopyridine-loaded topical gel for promoting skin regeneration in burn injuries.

Burn wounds are a common traumatic injury that impair cellular function and hinder the healing process, often resulting in significant skin loss. While autologous skin grafting is considered the gold standard for treating burns, its widespread use is limited due to donor site morbidity and the requirement for large amounts of tissue. Traditional wound dressings and treatments often fail to ensure complete recovery. Being initially FDA-approved to treat multiple sclerosis, 4-aminopyridine (4-AP) has also been shown to accelerate burn wound closure by modulating keratinocytes and fibroblasts whe

Application of polymeric surfactants in research and development of drugs applied topically to the skin and mucous membranes.

Polymeric surfactants play an important role in the research and development of drugs applied topically to the skin and mucous membranes. Their versatile properties include the ability to lower surface tension, thereby favorably contributing to the energetic balance of the emulsification process during the preparation of various dosage forms. In addition, they offer important structural advantages that enhance the stability of the resulting pharmaceutical or cosmetic products through electrostatic repulsion and steric effects. The influence of viscosity and density should also be taken into ac
