Twenty five patients with vesicovaginal fistulae were managed at St. Gaspar Hospital, Itigi, Tanzania between February 1993 and March 1994. Most of them, 92% were repaired vaginally routinely using the Martius labial flap and 8% were repaired transabdominally because the fistulae were inaccessible vaginally as they were vesicouterine or vesicocervical fistulae. The success rate was 96% after the first operative attempt.

Examined the utility of a new parent-report measure designed specifically for pediatric inpatients, the Behavioral Upset in Medical Patients-Revised (BUMP-R). The BUMP-R was administered to 151 mothers of hospitalized children ages 4-12 years the day following the child's hospital admission. The health profile of two groups of Ethiopian immigrants in Israel was evaluated. Trichophytosis and active trachoma were observed in 5%-8.7% of prepubertal children. Among adults 13% had corneal opacities and 10.4% had cataracts. Goiter, spastic bronchitis, valvular heart disease, leprosy and onchocerciasis were prevalent. Malnutrition was common, with average body weights ranging from 72.4% to 85.2% of normal. Anemia was observed in 6.3% of young children and 70% of all immigrants. A recognized major postoperative complication of hepatic hydatid disease is accumulated hydrops leading to secondary infection in the residual cavity of the excised cyst. This study examined two methods of treating cystic hepatic hydatid disease in 43 children. A group of 22 children were treated by a new method of open drainage, 21 underwent capsulorrhaphy without drainage. There was no mortality or morbidity. The open method was associated with more rapid resolution of hydrops and quicker shrinkage of residual cavities, with efficacy over a median follow-up of 42 months.

Seventy-one cases of malaria were diagnosed in the hospital of Dunkerque from 1979 to 1990. Forty-five patients have been infected in the Comores islands. Among the 24 other imported cases, 17 (71%) have been contaminated in Western or Central Africa. One case of congenital malaria and one of malaria acquired by blood transfusion are also reported. Plasmodium falciparum was detected in 84.5% of the patients. The four human species were found among the Comorians. Two cases with abnormal forms of Plasmodium vivax were observed.

A study has been carried out to assess the pattern of child admissions to Tayside Hospitals as a result of accidents, poisoning and violence (hereafter referred to simply as 'accidents') during 1989, and to determine if any major changes have taken place since 1980. The length of stay resulting from each class of accident was used as an approximate indicator of severity and the consumption of Health Service resources. Since 1958 we have conducted a prospective study of anorexia/bulimia nervosa (A/BN) covering a total of 550 patients. This paper presents a new idea of the development of A/BN. For a variety of reasons, a predisposition for developing A/BN is triggered by a minor and non-specific loss of weight. Forty-nine hospital wards in the United States and Canada were surveyed during specified periods for one year to document assaults by inpatients on staff or other patients. Nine (5.7%) of the 158 assaults by adult psychiatric inpatients were sufficiently severe to require medical treatment. Four others occurred on the only children's service studied. Only one (2.6%) of 39 assaults on adult nonpsychiatric units required medical care. Injuries were rarely serious, with only four assaults in any setting leading to bedrest or missed workdays. From a sample of 850 adolescents, 377 were involved in 550 separate incidents which resulted in 636 injuries requiring medical attention. In a university teaching hospital, the charts of 150 psychiatric inpatients consecutively referred for EEG were reviewed to survey the clinical use of the EEG by psychiatrists. In a 750-bed community-teaching hospital with 3,200 employees, throat cultures were routinely done in hospital personnel complaining of a sore throat. During a 3-month period, 323 employees had throat cultures; only 20 (6.2%) of these throat cultures grew group A streptococcus. The prevalence of positive throat cultures was similarly low in employees (6.2%) and adult patients (7.3%). There was no evidence that employees either had higher prevalence of group A streptococcal pharyngitis or that they spread the infection to patients. Five elective and one emergency orthotopic heart transplantation were performed in six patients, 32 to 54 years of age. Postoperative complications included transient renal failure in four patients with need for dialysis in one, seizure in one, and herpes infections in four. Immunosuppression of the first three patients consisted of cyclosporine and prednisone. The last three received cyclosporine and azathioprine, with prednisone added later for control of rejection. All six patients are home and well.

Our experience with 119 cases of peritonsillar abscess supports the experience of others, that immediate tonsillectomy is a safe treatment which provides prompt, complete drainage of the abscess. There was one episode of immediate postoperative hemorrhage and one of delayed bleeding, but there were no anesthetic complications.
