The potential role of HLA-DRB1*11 in the development and outcome of haematopoietic stem cell transplantation-associated thrombotic microangiopathy.

Transplantation-associated thrombotic microangiopathy (TA-TMA) is a serious complication of allogeneic haematopoietic stem cell transplantation (allo-HSCT) with high mortality rate. We retrospectively studied the frequency, clinical and genetic associations and prognostic effect of TA-TMA, in a total of 425 consecutive adult patients, who underwent allo-HSCT for a malignant haematological condition between 2007 and 2013 at our single centre. TA-TMA developed in 19% of the patients. Unrelated donor type (P<0.001), acute GvHD grades II-IV (P<0.001), myeloablative conditioning regimens (P=0.003),

Prion-like Mechanism in Amyotrophic Lateral Sclerosis: are Protein Aggregates the Key?

ALS is a fatal adult-onset motor neuron disease. Motor neurons in the cortex, brain stem and spinal cord gradually degenerate in ALS patients, and most ALS patients die within 3~5 years of disease onset due to respiratory failure. The major pathological hallmark of ALS is abnormal accumulation of protein inclusions containing TDP-43, FUS or SOD1 protein. Moreover, the focality of clinical onset and regional spreading of neurodegeneration are typical features of ALS. These clinical data indicate that neurodegeneration in ALS is an orderly propagating process, which seems to share the signature

Non-neuronopathic lysosomal storage disorders: Disease spectrum and treatments.

Distinctive facial features, hepatosplenomegaly or cardiomyopathy with or without associated skeletal dysplasia are clinical manifestations that may be suggestive of an underlying lysosomal storage disorder (LSD), However, these features may not be evident in certain subtypes associated primarily with central nervous system involvement. Age at onset can be broad, ranging from infancy to adulthood. Diagnosis may be delayed, as manifestations may be slow to evolve (taking months to years), particularly in those with later (adult-)onset, and in isolated cases (i.e., those without a prior family h

Introduction to the alpha-proteobacteria: Wolbachia and Bartonella, Rickettsia, Brucella, Ehrlichia, and Anaplasma.

Wolbachia is an obligate intracellular endosymbiont and likely mutualist living within the heartworm Dirofilaria immitis and a number of other filarial nematodes in the family Onchocercidae. The bacterial infection is passed from worm to worm transovarially; the organisms are in ovarian cells, the developing microfilariae, and multiply and persist in all later developmental stages through the mosquito and into the next host. Besides being present in the ovaries of the adult worms, they also are present in large numbers within the hypodermal tissues of the nematode. It is now know that these ba

Clinical characteristics and outcomes of adults with Philadelphia chromosome positive and/or bcr-abl positive acute lymphoblastic leukemia: a single center study from China.

The study reviewed 389 adult patients with acute lymphoblastic leukemia (ALL) and 110 patients (28.3%) were diagnosed as Philadelphia chromosome positive (Ph-positive) and/or bcr-abl positive ALL. The special group had the same clinical characteristics as other studies, except for relatively young age and high incidence of both P190 and P210 fusion proteins expression. The complete remission (CR) rate in conventional chemotherapy (CT) group and in chemotherapy combined with imatinib (ICT) group was 84.7% and 96.0%, respectively. The 2-year disease-free survival (DFS) and overall survival (OS)

Outcome of HLA-matched related allogeneic hematopoietic stem cell transplantation for patients with acute leukemia in first complete remission treated in Eastern European centers. Better results in recent years.

The goal of this study was to analyze results and to determine factors affecting outcome of HLA-matched hematopoetic stem cells transplantation (MRD-HSCT) for patients with acute leukemia transplanted in first complete remission in Eastern European countries. Six hundred forty HSCT were performed between 1990 and 2006 for adults with acute myeloid (n = 459) and lymphoblastic (n = 181) leukemia. Two-year leukemia-free survival (LFS), nonrelapse mortality (NRM), and relapse incidence were 58 +/- 2%, 19 +/- 2%, and 23 +/- 2%, respectively. The cumulative incidence of NRM decreased from 22 +/- 2%

Endogenous stem cell activation and voluntary exercise differentially influence improvements in motor recovery after chronic spinal cord injury.

The ependymal cell layer lining the central canal of the spinal cord forms a key endogenous neural stem cell niche in adult mammals, and enhancing its activity may promote repair following spinal cord injury (SCI). We investigated the independent and combined effects of voluntary free wheel running and PNU-120596 (PNU), a positive allosteric modulator of α7 nicotinic acetylcholine receptors, on functional recovery and endogenous plasticity after chronic thoracic contusion SCI. Four weeks post-injury, female rats received PNU or vehicle, with or without access to running wheels, for six weeks.

Curative treatment for severe sickle cell disease: allogeneic hematopoietic cell transplant or gene therapy.

Sickle cell disease (SCD) is characterized by chronic hemolysis, ischemia-reperfusion injury, and progressive end organ damage. Although survival for children with SCD in the United States (US) is excellent the life expectancy for adults is approximately two decades shorter compared to those without SCD. Consequently, those with severe SCD, estimated to be about 20% of individuals with SCD in the US deserve to be offered treatment choices with the potential for cure. Two treatment options with curative intent are available. The first, hematopoietic cell transplantation is limited by donor avai

Anti-Leukemic Profiling of Oxazole-Linked Oxadiazole Derivatives: A Computational and Kinetic Approach.

Background/Objectives: Leukemia is a common cancer that arises in both children and adults when bone marrow's hematopoietic stem cells proliferate unrestrained because of anomalies in normal cell regulatory systems. The present study focused on biological evaluation of oxazole-based oxadiazole scaffolds to evaluate the anti-proliferative effect on leukemic cancer cell lines. Methods: All novel oxazole-based oxadiazole scaffolds were synthesized and structurally characterized via 13C NMR, 1H NMR, and HREI-MS. In order to identify an efficient anti-leukemia agent, the biological profiles of each

Role of Ovarian Stem Cells in Postnatal Oogenesis and Implications in Fertility Preservation.

It is a long-held dogma in reproductive biology that females are born with a set nonrenewable number of oocytes in the ovary. While some animal studies challenged this dogma by demonstrating allegedly postnatal oogenesis and the presence of certain stem cell factor expression germ cell markers, their biological roles are yet to be defined. In this chapter, we revisited this issue revisit this dogma in light of most recent data. The chapter outlines the characteristics of these putative OSCs, including their morphological features, expression of germline markers (e.g., DDX4, DAZL), telomerase a

Borneol-modified docetaxel plus tetrandrine micelles for treatment of drug-resistant brain glioma.

OBJECTIVE: Glioma is the most common and deadly primary malignant tumor in adults. Treatment outcomes are ungratified due to the presence of blood-brain barrier (BBB), glioma stem cells (GSCs) and multidrug resistance (MDR). Docetaxel (DTX) is considered as a potential drug for the treatment of brain tumor, but its effectiveness is limited by its low bioavailability and drug resistance. Tetrandrine (TET) reverses the resistance of tumor cells to chemotherapy drugs. Borneol (BO) modified in micelles has been shown to promote DTX plus TET to cross the BBB, allowing the drug to better act on tumo

Zidovudine and Interferon Alfa based regimens for the treatment of adult T-cell leukemia/lymphoma (ATLL): a systematic review and meta-analysis.

BACKGROUND: ATLL (Adult T-Cell Leukemia/Lymphoma) is an aggressive hematological malignancy. This T-cell non-Hodgkin lymphoma, caused by the human T-cell leukemia virus type 1 (HTLV-1), is challenging to treat. There is no known treatment for ATLL as of yet. However, it is recommended to use Zidovudine and Interferon Alfa-based regimens (AZT/IFN), chemotherapy, and stem cell transplant. This study aims to review the outcome of patients with different subtypes of ATLL treated with Zidovudine and Interferon Alfa-based regimens. METHODS: A systematic search was carried out for articles evaluatin

Donor HLA-E Status Associates with Disease-Free Survival and Transplant-Related Mortality after Non In Vivo T Cell-Depleted HSCT for Acute Leukemia.

Previous studies have suggested that HLA-E may have a significant role in the outcome of matched unrelated hematopoietic stem cell transplantation (HSCT), especially for patients with acute leukemia. We used Center for International Blood and Marrow Transplant Research data and samples of 1840 adult patients with acute leukemia and their 10/10 HLA-matched unrelated donors to investigate the impact of HLA-E matching status as well as of donor/recipient (D/R) HLA-E genotype on post-HSCT outcome. Both patients and donors were HLA-E genotyped by next-generation sequencing. All patients received th

Pancreatic metastasis from nephroblastoma: an unusual entity.

CONTEXT: Pancreatic metastasis from renal cell carcinoma is a well-known entity. When metastatic disease is limited to the pancreas, pancreatic resection is the optimal treatment. A nephroblastoma is a frequent childhood cancer but can also occur in adults. A metastatic nephroblastoma mainly affects the lung and the liver. Pancreatic metastases from a nephroblastoma are very rare. CASE REPORT: We report an extremely rare case of pancreatic metastases in a 20-year-old man who had a right nephroblastoma resected at 9 years of age and liver metastases treated by right hepatectomy at 18 years of

Mesenchymal stem cells cultured on a collagen scaffold: In vitro osteogenic differentiation.

OBJECTIVE: Management of periodontal defects has always been a challenge in clinical periodontics. Recently mesenchymal stem cells (MSC) have been proposed for tissue regeneration in periodontal disease and repair of large bone defects. Bone regeneration has to be supported by a scaffold which has to be biocompatible, biodegradable, and able to support cell growth and differentiation. The aim of this study was to evaluate osteogenic differentiation of MSC seeded on a collagen scaffold. DESIGN: MSC were obtained from adult rat bone marrow, expanded and cultured in plastic dishes or seeded in a

TdT+/nonlymphoid antigen+ acute leukemias: immunologic and karyotypic monitoring during therapy and at relapse suggests the transformation of a bipotential stem cell.

Immunophenotype and karyotype were monitored in 19 adult acute leukemia patients with blast cell populations expressing terminal transferase (TdT) and nonlymphoid antigens either at presentation or at relapse. Three patterns of immunophenotypic course were observed when following the patients through at least one, sometimes two (six patients), or three relapses (one patient). Induction chemotherapy induced predominantly TdT+ leukemias with a minor monoblastic component to become TdT-negative, purely monoblastic without clinical response or change in karyotype in five patients (group 1). Central nervous system.

Several different types of tumors, benign and malignant, have been identified in the central nervous system (CNS). The prognoses for these tumors are related to several factors, such as the age of the patient and the location and histology of the tumor. In adults, about half of all CNS tumors are malignant, whereas in pediatric patients, more than 75% are malignant. For most benign CNS tumors that require treatment, neurosurgeons can offer curative resections or at least provide significant relief from mass effect. Unfortunately, we still lack effective treatments for most primary and secondar

The impact of pretransplant consolidation chemotherapy on clinical outcomes in adult patients with Philadelphia chromosome-negative acute lymphoblastic leukemia: a single-center retrospective study.

Acute lymphoblastic leukemia (ALL) is a malignant hematological disease that accounts for approximately 20% of adult leukemia. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is currently recognized as one of potentially curative treatments for ALL. Though pretransplant consolidation chemotherapy is generally considered essential, there is limited research on the optimal number of consolidation cycles, which leaves the ideal timing for transplant unclear. In this study, we retrospectively analyzed the clinical outcomes of 40 adult patients diagnosed with Philadelphia chromosome-

Clinical outcomes of hypomethylating agents and venetoclax in newly diagnosed unfit and relapsed/refractory paediatric, adolescent and young adult acute myeloid leukaemia patients.

Venetoclax (VEN) combined with hypomethylating agents (HMA) decitabine or azacitidine is used for adult acute myeloid leukaemia (AML), but its application in paediatric, adolescent and young adult (AYA) AML lacks prospective studies. We performed a retrospective chart review of paediatric and AYA AML patients treated with HMA + VEN at Cincinnati Children's Hospital Medical Centre. Twenty-seven patients received 30 HMA + VEN treatment courses for relapsed/refractory (R/R, n = 21) or newly diagnosed (n = 9) AML due to ineligibility for intensive chemotherapy.
