A case of fatal intracranial hemorrhage is reported in an eleven year old girl with acute idiopathic thrombocytopenic purpura following a viral infection. The patient was randomized to the IgG-arm of the ITP therapy study. Immunoglobulin administration was not followed by a raise of the thrombocyte count. Neither the IgG therapy nor intensive therapeutic measurements were able to prevent the fatal course of cerebral hemorrhage in this case. Hemolytic disease in the newborn, as a cause of early jaundice, is not uncommon. This is mostly due to Rh (D), ABO incompatibility and rarely due to other minor blood group incompatibility. The authors report two cases of Rh anti c isoimmunization presenting as significant early neonatal jaundice within the 20 h of life. Both the babies were treated with intensive phototherapy. One baby underwent exchange transfusion and the other required packed cell transfusion for anemia.

The infection with parvovirus B19 during pregnancy causes in 1/3 of the cases an aplastic crisis in the fetus and consecutively generalized fetal hydrops as a result of severe anemia. Some cases of fetal hydrops and intrauterine death are reported. In our two cases, the fetal therapy by intrauterine intravasal transfusion was successful and the children developed normal. The techniques and indications for fetal blood sampling and the method of intrauterine intravasal transfusion are explained.

Fetomaternal hemorrhage (FMH) is a common obstetrical occurrence most often associated with small volumes of blood transferred across the placenta. Fetomaternal hemorrhage leads to alloimmunization of Rh D-negative mothers, resulting in an increased risk of hemolytic disease of the newborn. Massive FMH involving volumes of blood greater than 30 mL can cause substantial fetal morbidity and mortality. High dose dexamethasone combined with irradiation to the base of the brain achieved a dramatic beneficial effect in two terminal cases of widespread metastatic carcinoma of the prostate involving cranial nerves and the entire skeleton. Pain requiring very large doses of analgesics and anemia requiring blood transfusions every 3-4 weeks were improved rapidly. No further transfusions were needed.

Intravascular fetal transfusions are occasionally complicated by extravascular deposition of transfused blood or by fetal bradycardia on penetration of the needle into the umbilical cord. It is desirable therefore to continuously monitor the intravascular location of the needle and the fetal heart rate. This can be achieved by Doppler velocimetry of the umbilical vein and artery during the procedure. The technique is easily performed, does not require moving of the ultrasonography transducer, and appears to be time efficient.

In almost every pregnancy feto-maternal transfusions occur. From the 6th week of pregnancy, blood containing Rhesus antigens may be infused into the maternal circulation and cause sensitization. In case of miscarriage, abortion, ectopic pregnancy and cystic mole the chance of fetomaternal transfusion followed by sensitization of the mother is significantly increased. Therefore immunoprophylaxis with anti-D-immunoglobins should be performed in all these cases.
