Characteristics of sagittal spinopelvic alignment in patients with Parkinson's disease presenting with dropped head syndrome: a case series study.

BACKGROUND: Dropped head syndrome (DHS) is a rare specific abnormal posture known to develop in Parkinson's disease (PD). This case series study aimed to characterize DHS by analyzing the characteristics of sagittal spinopelvic alignment in patients with PD/DHS. METHODS: The study included eight patients with PD/DHS (men = 3, women = 5; mean age, 68.1 ± 6.4 years). Sagittal spinopelvic alignment was evaluated using 10 parameters on whole-spine lateral radiographs. RESULTS: The time from the onset of PD to that of DHS varied among the patients from 0 to 15.3 years. In three patients, DHS appe

Adjacent segment mobility after ACDF considering fusion status at the implant insertion site.

PURPOSE: This paper sets out to analyse mobility changes in segments adjacent to the operated segment. Additionally, it investigates the relationship between the degree of fusion in the operated disc space and mobility changes in the adjacent segments. METHODS: In total, 170 disc spaces were operated on in 104 consecutive patients qualified for one- or two-level surgery. The degree of mobility of segments directly above and below the implant insertion site was calculated. Measurements were performed the day before the surgery and 12 months post-surgery. Functional (flexion and extension) radi

Outcomes Following 2-Level Cervical Interventions with Cage-and-Plate, Zero-Profile, or Arthroplasty Constructs.

BACKGROUND: Though cage-and-plate constructs are widely used for disk height restoration in surgery for cervical disc disease, concerns over range of motion limitations and adjacent disc space violations have fueled the development of artificial disc and zero-profile constructs. This study investigated the outcomes of patients undergoing two-level cervical interventions via arthroplasty, cage-and-plate, or zero-profile constructs. METHODS: Patients undergoing two-level anterior cervical procedures between 2010 and 2020 were identified using an all-payer claims database. Logistic regression mo

Diagnosis and radiation therapy of an extensive myxoma in the retropharyngeal region infiltrating the cranial cervical vertebral canal in a dog.

An 8-year-old, intact Rottweiler-female dog presented due to an acute onset of lethargy, abnormal gait, and wheezing. Physical examination revealed stridor, cervical pain, and ambulatory tetraparesis. Magnetic resonance imaging-examination displayed a lobulated, fluid-filled mass extending from the sphenoid bone to C5, infiltrating the cranial vertebral canal causing extradural compression of the spinal cord and narrowing of the pharynx. An emergency debulking-surgery around the pharynx was performed. Histopathological findings were consistent with a myxoma. The remaining tumor was irradiated

CT diagnosis of occipital condyle fracture in a dog presented for severe cervical hyperesthesia.

A 9-month-old male entire Doberman Pinscher presented with acute onset of severe cervical hyperesthesia after a fall. Neurological examination revealed a normal gait with low head carriage and severe cervical hyperesthesia. A CT scan of the cervical vertebral column revealed the presence of a comminuted fracture at the dorsomedial aspect of the right occipital condyle and sclerosis of the underlying bone. Medical management was initiated consisting of an external bandage, strict rest, and pain medication. Due to the lack of clinical improvement, the dog was euthanized 2 months after diagnosis.

Incidence of dysphagia following posterior cervical spine surgery.

Abundant literature exists describing the incidence of dysphagia following anterior cervical surgery; however, there is a paucity of literature detailing the incidence of dysphagia following posterior cervical procedures. Further characterization of this complication is important for guiding clinical prevention and management. Patients ≥ 18 years of age underwent posterior cervical fusion with laminectomy or laminoplasty between C1-T1. Pre- and post-operative dysphagia was assessed by a speech language pathologist. The patient cohort was categorized by approach: Laminectomy + Fusion (LF) and L

Utility of Repeat MRI in Patients With Degenerative Cervical Disease.

Existing guidelines regarding indications for initial cervical spine magnetic resonance imaging (MRI) do not indicate when to perform repeat MRI in patients with previously documented degenerative disease. This study evaluates the efficacy of repeat MRI in patients with previously diagnosed degenerative cervical disease. Between 2013 and 2018, 153 patients (102 women, 51 men; mean age, 55 years; range, 19-81 years) without a history of trauma or surgery underwent cervical spine MRI 2 or more times at our institution indicated for symptoms of neck pain with or without radiculopathy. The MRI rep

Ossification of the Posterior Longitudinal Ligament: Pathophysiology, Diagnosis, and Management.

Ossification of the posterior longitudinal ligament (OPLL) occurs as heterotopic bone forms in the posterior longitudinal ligament, resulting in neural compression, myelopathy, and radiculopathy. OPLL is most commonly observed in East Asian populations, with prevalence rates of 1.9% to 4.3% reported in Japan. OPLL rates are lower in North American and European patients, with reported prevalence of 0.1% to 1.7%. Patients typically develop symptoms due to OPLL in their cervical spines. The etiology of OPLL is multifactorial, including genetic, metabolic, and anatomic factors. Asymptomatic or sym

Crimping technique to treat iatrogenic vertebral artery injury during spinal fusion.

Iatrogenic arterial injuries may occur during neurosurgical procedures. Particularly, the vertebral artery may be injured in a high-level cervical spinal fusion case, either during the initial exposure or when placing screws.1- 3 If such an injury occurs, obtaining hemostatic control and repairing the laceration are of paramount importance.4, 5 In this technical video, we describe the case of a patient who was undergoing a posterior C1-C2 cervical fusion when the right vertebral artery was injured due to variant anatomy. Using sutures to repair the injury was unsuccessful. Thus, we employed a

Assessing Postoperative Pseudarthrosis in Anterior Cervical Discectomy and Fusion (ACDF) on Dynamic Radiographs Using Novel Angular Measurements.

STUDY DESIGN: A retrospective review of operative patients at a single institution. OBJECTIVE: The aim was to validate a novel method of detecting pseudarthrosis on dynamic radiographs. SUMMARY OF BACKGROUND DATA: A common complication after anterior cervical discectomy and fusion is pseudarthrosis. A previously published method for detecting pseudarthrosis identifies a 1 mm difference in interspinous motion (ISM), which requires calibration of images and relies on anatomic landmarks difficult to visualize. An alternative is to use angles between spinous processes, which does not require cal
