At present, according to the unanimously accepted data, cholecystoses are noninflammatory, nonlithiasic, gallbladder diseases. However the authors' experience has proved that the inflammatory process is much more frequent than it is believed and often associated also with lithiasis, a fact which, in the authors' opinion, would justify a reconsideration of this group of diseases. This study, based on histopathologic examination, was carried out in 1,630 gallbladder specimens, surgically removed. Gallstone ileus is a rare case of mechanical intestinal obstruction observed in patients with history of cholelithiasis or cholecystitis. Its diagnosis is difficult and it is characterized by high mortality rate. Diagnostic Imaging plays an important role in the management of patients with suspected gallstone ileus because an early diagnosis could reduce the mortality. Abdominal Computed Tomography (CT) is the preferred modality because of its rapid diagnosis. Surgery remains the gold standard treatment.

A 54-year-old man was admitted to our surgery department complaining of right upper quadrant abdominal pain. The working diagnosis of acute cholecystitis was based on the clinical picture, laboratory tests, and ultrasound findings. Because of the uncommon course of the disease, computed abdominal tomography was performed, showing multiple stones in the anterior abdominal wall. On surgery, the fistula, containing stones and pus connecting the gallbladder and the abdominal wall muscles, was found.

The systemic inflammatory response, induced by the lipopolysaccharides of the normoflora of the operated patients was studied on the example of the plasma changes in 107 patients. All patients were divided in 4 groups, depending on the level of lipopolysaccharides in plasma. Thus, patients with the complicated course of the acute cholecystitis, demonstrated the increased lipopolysaccharides levels. An obese patient with cholelithiasis and acute cholecystitis was scheduled for an emergency laparoscopic cholecystectomy. On preoperative workup, a 5-year-old echocardiogram showed a large patent foramen ovale (PFO) with a right-to-left shunt that was open at rest. Noting both the benefits of laparoscopic surgery and the substantial incidence of PFO in the general population (10% to 30%), the attending and consulting surgeons reasoned that the benefits of laparoscopy greatly exceeded an immeasurably small risk of paradoxical emboli.

In an 83-year-old woman presenting with gallstones and a cancer in the sigmoid colon, resection was performed through a median incision, after which the wound was extended, the stones were crushed, and the gallbladder was infolded and sutured (reefed). Even in elderly patients, some treatment for bile stones should be done at abdominal section to avoid future cholecystitis or complications. Reefing is a useful technique that can be done easily from a comparatively narrow field of view.

A 46-year-old man was operated on for cholelithiasis with chronic cholecystitis. On gross inspection of the resected gallbladder, a slight thickening in the body wall, in an area measuring about 1 x 0.5 cm, was noted. On light microscopic and immunohistochemical examinations, the lesion was diagnosed to be a rhabdoid tumor. After a thorough review of the literature we failed to find any reference to such a lesion in the gallbladder. This is the first known case report of a rhabdoid tumor of the gallbladder.

A retrospective study of all records of patients operated on for biliary lithiasis during the years 1995, 1996 and 1997 was performed in a general hospital setting to compare the costs of management according to presentation. This study shows that acute and chronic cholecystitis have statistically significantly different costs. The reimbursement plans, base on diagnostic related groups do not take this into account. This system disadvantages those centers which preferentially treat acute pathologies.

The value of magnetic resonance imaging (MRI) and magnetic resonance cholangiopancreatography (MRCP) is demonstrated in a case of a 22-year-old female with a small choledochocele clinically presenting with acute cholecystitis and pancreatitis. Previous abdominal sonography and computed tomography were not diagnostic. MRI and MRCP showed a cystic dilatation of the distal common bile duct, intramurally located in the second portion of the duodenum and protruding into the duodenal lumen. These findings allowed a confident diagnosis of a choledochocele.

The anatomical position of the gallbladder makes it readily accessible to ultrasonographic visualization. Gallstones, gravel and sludge present no diagnostic problems. Acute cholecystitis, empyema and perforation may be more difficult to detect. In the presence of large or numerous stones in the gallbladder, there is a danger of overlooking a carcinoma of the gallbladder. Bile duct carcinomas are difficult to detect by ultrasonography. While bile duct dilatation and the site of an occlusion are readily detected, the cause of the occlusion is usually not revealed by ultrasonography.

Generalized pustular psoriasis in siblings, 31-year-old male and 26-year-old female, is presented. In both cases, pustular lesions appeared in childhood and typical eruption of psoriasis vulgaris was not observed during their clinical course. Cholecystitis and chronic tonsillitis of the brother and periodontitis and chronic tonsillitis of the sister were considered to be possible provocative factors. HLA-A24, Bw52-, DR2, as the common HLA haplotype in our cases, was estimated.

Granular cell tumors are benign lesions that usually appear in the skin, oral activity, and mucous membrane. Occasionally, they will appear in visceral organs, and cause signs and symptoms similar to those of other organ-related diseases. We studied a patient with granular cell tumors of the common bile duct whose clinical appearance mimicked that of cholecystitis with cholelithiasis. Recognition of granular cell tumors on frozen section will allow surgical resection with probable cure.

Three neonates with spontaneous perforation of the common bile duct have been described. All were treated by laparotomy in the seventh week, with attempted closure of the perforation and a simple biliary diversion by cholecyst-jejunostomy with jejunojejunostomy below it. They have done well and represent 100% salvage of our cases to date. One is now 12 years of age. As far as we are aware, this method of dealing with the condition has not been described before.
