Trials of pacing to control ventricular rate during atrial fibrillation.

Pharmacologic therapy to achieve rate control in patients with atrial fibrillation is often difficult and inadequate. For this reason, ventricular pacing strategies have been developed as an alternative to drug therapy to alleviate symptoms due to rapid and irregular ventricular rates. Ventricular pacing in combination with AV junctional ablation provides palliative improvement in a wide range of clinical outcomes. Because of the irreversible complete AV block associated with this procedure, strategies to control the ventricular response to atrial fibrillation by ventricular pacing alone have

Unilateral limitation of abduction of the hip. A valuable clinical sign for DDH?

Between 1992 and 1997, we undertook a prospective, targeted clinical and ultrasonographic hip screening programme to assess the relationship between ultrasonographic abnormalities of the hip and clinical limitation of hip abduction. A total of 5.9% (2 of 34) of neonatal dislocatable hips and 87.5% (7 of 8) of 'late' dislocated hips seen after the age of six months, presented with unilateral limitation of hip abduction. All major (Graf type III) and 44.5% of minor (Graf type II) dysplastic hips presented with this sign. Statistically, bilateral limitation of hip abduction was not a useful clini

The clinical diagnosis of genital ulcer disease in men.

We report the sensitivity and specificity of physical examination findings for diagnosing primary syphilis, chancroid, and genital herpes. The physical features of genital ulcers in 446 men were measured in accordance with a quantitative scale. Two hundred-twenty of these men had an established, single microbiological diagnosis. Forty-five (20%) had primary syphilis, 118 (54%) had chancroid, and 57 (26%) had genital herpes. There was considerable overlap in the clinical presentation of these three diseases. The classic clinical sign complex attributed to primary syphilis (painless, indurated,

Serodiagnosis of candidiasis, aspergillosis, and cryptococcosis.

During the past decade, substantial progress has been made in the development of new approaches and methods for the serological diagnosis of the mycoses. Clinically relevant antigens have been adapted for use in immunoassays for the detection of specific antibodies, and methods that detect fungal antigens in body fluids have been progressively refined. Nevertheless, few of the novel serological techniques have as yet found widespread clinical use. Reasons for this apparent discrepancy include the paucity of clinical data that substantiate improved predictive value for disease of antibody tests

[Etiologic classification of cerebral infarct. Experience from a prospective data register].

Most classifications of stroke include clinically heterogeneous subgroups and therefore are of limited value for comparative studies or clinical protocols. The view is held that a classification according to stroke etiology is clinically more reasonable and more consistent for therapeutic strategies. In order to determine the frequency of various etiological subgroups in a series of stroke cases, the results of the Klosterneuburger Schlaganfall-Datenbank (KSDB) are reported. This stroke registry has prospectively recorded over 300 items on all stroke cases referred to one center since March 19

Skull Base Reconstruction in Challenging Pediatric Patients.

Skull base reconstruction is a challenging task following trauma or surgical resection in the pediatric population. The reason is the region's complexity and the need to restore both the anatomic integrity and the functional aspects of the cranial base. This brief clinical case series presents the management of pediatric patients with skull base defects. These surgeries were successfully reconstructed skull bases, using calvarial bone grafts in 2 patients and a titanium mesh in the third. All patients showed excellent postoperative recovery with no complications. Skull base defects can be effe

Protocol for Development of an Evidence-Based Rapid Readiness Screen.

In combat and sport, there is an urgent need to identify readiness for performance to decide on removal from and return to activity. Cognitive readiness, including attention, can be impaired for many potentially coexisting reasons, including concussive or subconcussive injury, insufficient sleep/fatigue, and intoxication. Standard tests of concussion, sleep deprivation, and intoxication are often lengthy or require specialized skill or equipment to conduct. The ideal readiness screen should predict readiness or be associated with surrogate measures of readiness, be sensitive to multiple condit

Iatrogenic nerve injury in upper limb surgery excepting shoulder.

Upper limb nerve injuries, although uncommon, represent a major source of morbidity and medicolegal issues. They may occur in various contexts: during patient positioning, regional anesthesia, or the surgical procedure itself. This article provides a synthesis of the mechanisms of injury, the nerves at risk according to anatomical regions (from the arm to the hand), and simple preventive measures to be adopted in each situation. We emphasize the importance of knowledge of anatomical landmarks, safety zones, and the clinical settings most likely to cause injury. A reasoned diagnostic and therap

Large Granular Lymphocytic Leukemia: Clinical Features, Molecular Pathogenesis, Diagnosis and Treatment.

Large granular lymphocytic (LGL) leukemia is a lymphoproliferative disorder characterized by persistent clonal expansion of mature T- or natural killer cells in the blood via chronic antigenic stimulation. LGL leukemia is associated with specific immunophenotypic and molecular features, particularly STAT3 and STAT5 mutations and activation of the JAK-STAT3, Fas/Fas-L and NF-κB signaling pathways. Disease-related deaths are mainly due to recurrent infections linked to severe neutropenia. The current treatment is based on immunosuppressive therapies, which frequently produce unsatisfactory long-

Left Bundle Branch Area Pacing to Overcome Coronary Sinus Anatomy-Related Technical Problems Encountered during Implantation of Biventricular CRT-A Case Report.

The results of clinical trials show that up to one-third of patients who are eligible for cardiac resynchronization therapy (CRT) do not benefit from biventricular pacing. The reasons vary, including technical problems related to left ventricle pacing lead placement in the appropriate branch of the coronary sinus. Herein, we present a case report of a patient with heart failure with reduced ejection fraction and left bundle branch block, in whom a poor coronary sinus bed made implantation of classic biventricular CRT impossible, but in whom, alternatively, rescue-performed left bundle branch a

[A case report of acquired factor ⅩⅢ deficiency following hip replacement surgery induced by sodium valproate and a literature review].

Coagulation factor ⅩⅢ (FⅩⅢ) deficiency is a rare disease that is easily overlooked because routine coagulation tests do not reflect its deficiency. FⅩⅢ deficiency is divided into hereditary FⅩⅢ deficiency and acquired FⅩⅢ deficiency. The diagnosis of FⅩⅢ deficiency requires testing for FⅩⅢ activity and antigen, with confirmation through genetic testing. Due to the instability of fibrin clots caused by FⅩⅢ deficiency, the clinical features of FⅩⅢ deficiency include umbilical cord bleeding, soft tissue hematoma, recurrent miscarriage, cerebral hemorrhage, and delayed wound healing. Acquired FⅩⅢ

Severe Hypertensive Response to Atropine Therapy for Bradycardia Associated with Dexmedetomidine: Case Report and Literature Review.

Dexmedetomidine is a selective and potent α2-adrenoceptor agonist used for sedation, analgesia, and anxiolysis, with minimal respiratory depression; therefore, it is widely used in clinical practice. Transient hypertension has been reported to be an indication for the use of dexmedetomidine. The authors report three female patients who experienced hypertensive crisis when used atropine to treat bradycardia caused by dexmedetomidine. The transient hypertension is a relatively common side effect of dexmedetomidine, hypertensive crisis seen with coadministration of atropine is much less frequentl

Pheochromocytoma Diagnosed During First Trimester of Pregnancy.

Pheochromocytomas are rare catecholamine-secreting tumors that occur in 0.002% of pregnancies. These tumors result in high maternal and fetal morbidity and mortality unless diagnosed in early stages of development, because excess levels of catecholamines cause vasoconstriction of both maternal and uteroplacental vasculature. Paroxysmal hypertension is the most common manifestation, but its variability in presentation and similarity to other pregnancy-related conditions often make diagnosis of pheochromocytoma difficult. Thus, it is essential to consider underlying pathological causes of hypert

Why percutaneous revascularisation might not reduce the risk of myocardial infarction and mortality in patients with stable CAD?

Percutaneous coronary intervention (PCI) is widely adopted to treat chronic coronary artery disease. Numerous randomised trials have been conducted to test whether PCI may provide any prognostic advantage over oral medical therapy (OMT) alone, without definitive results. This has maintained the paradigm of OMT as the first-line standard of care for patients, reserving PCI for symptom control. In this review, we discuss the current evidence in favour and against PCI in stable coronary syndromes and highlight the pitfalls of the available studies. We offer a critical appraisal of the possible re

Metastatic Uterine Leiomyosarcoma as a Rare and Sinister Cause of Respiratory Distress: A Case Report and Literature Review.

Uterine leiomyosarcomas are an extremely rare subtype of uterine malignancy. This is a case report of a 47-year-old woman whose underlying uterine leiomyosarcoma manifested as acute respiratory distress secondary to pulmonary metastases. We highlight that a combination of suggestive imaging features and elevated lactate dehydrogenase (LDH) may prompt its diagnosis, notwithstanding that histological examination of a tissue sample is mandatory for its confirmation. The diagnosis of this condition is arduous for a multitude of reasons, including the insidious clinical course, aggressive nature, a

Postoperative agitation or delirium in paediatric patients. What we know and how to avoid it.

Emergence delirium or postoperative agitation is the name given to the state of altered consciousness that occurs after surgery and especially affects pediatric patients. Its incidence is not negligible, reaching 80% of cases in certain studies. It is frequently confused with other clinical entities, for which reason a scale has been validated to facilitate its diagnosis. Risk factors include age under 5 years, the presence of pain after surgery and especially intense preoperative anxiety. Pediatric emergence delirium presents as an adverse event after surgery and influences patient safety by

[Spontaneous otoliquorrhea, complicated by recurrent secondary meningitis, in a 6-year-old child with an abnormality of the inner ear].

Congenital anomalies in the inner ear structures development are one of the reasons for unilateral hearing loss in children. Unilateral hearing loss is predominantly congenital, thus children with this pathology are initially deprived of the ability to hear with both ears, which leads to the formation of a specific «individual hearing norm». Due to this, children do not complain of hearing loss, and unsuspecting parents can only guess why their child is poorly studying and lagging behind his peers. Unfortunately, in some situations the detection of malformations of inner ear structures occurs

A 20-Year-Old Gossypiboma Causing Small Bowel Obstruction.

Retained foreign bodies including gossypiboma could be silent for years. However, in some cases, it can lead to major complications. Gossypiboma is not frequently reported for multiple reasons, including nonspecific presentation clinically and radiologically, and ethical issues. We present a case of a gossypiboma that was retained for more than 20 years causing a severe intestinal obstruction for an elderly female. The intestinal obstruction was initially thought to be adhesive in nature and was managed initially conservatively, but with failure to improve, the patient was taken for explorator

Multiple Dyselectrolytemia in a Chronic Alcohol Abuser: A Case Report.

Electrolyte disorders in alcohol-dependent patients can be due to a multitude of reasons. We discuss a patient with diabetes mellitus, seizure disorder, and alcoholism who presented with seizure episodes and vomiting following a binge alcohol intake. The evaluation showed life-threatening metabolic derangements that included hyponatremia, hypokalemia, hypomagnesemia, hypocalcemia, hypochloremia, hypophosphatemia with elevated blood glucose, and metabolic alkalosis with a normal anion gap. Subsequently, a detailed urinary analysis revealed a urinary loss of electrolytes. We emphasize that alcoh
