Exclusion of patients with brain metastases from early-phase systemic therapy trials: a cross-sectional analysis of eligibility criteria.

PURPOSE: Patients with brain metastases (BrM) are frequently excluded from early-phase clinical trials, limiting evaluation of central nervous system (CNS) efficacy of novel therapies. We aimed to determine the proportion of phase I systemic therapy trials for patients with metastatic solid tumours that permit enrolment of patients with BrM and to characterise BrM-specific eligibility criteria. METHODS: ClinicalTrials.gov was searched in July 2024 for phase I systemic therapy trials for patients with advanced solid tumours and publicly available protocols. Trial characteristics and BrM eligib

Identification and prioritization of high-frequency biomarkers and therapeutic targets in gastric cancer trials.

BACKGROUND: Gastric cancer (GC) is the third leading cause of cancer-related deaths worldwide. An investigation of clinical trials and therapeutic targets for GC was conducted. METHODS: Using the Trialtrove database, we anlyzed global and Chinese clinical trials on GC. Subsequently, we investigated oxaliplatin, S-1, apatinib, nivolumab, and sintilimab, alongside associated oncogenic biomarkers and therapeutic targets. The safety of these targets was evaluated through a joint analysis of the GTEx-RNA, HPA-RNA, and HPA-Proteins datasets. Finally, we assessed their specificity and clinical prospe

A practical overview of the use of amyloid-PET Centiloid values in clinical trials and research.

The density of brain amyloid-beta neuritic plaque accumulation, a marker of Alzheimer's disease (AD), can be visualized and quantified using amyloid-positron emission tomography (PET). Amyloid-PET data can be obtained using different tracers and methodologies; therefore, comparison across studies can be difficult. The introduction of Centiloids in 2015 allowed for the transformation of amyloid-PET quantitative data to a common scale, enhancing comparability across studies and potentially enabling pooled analysis. Since then, Centiloid values have been used increasingly in research and clinical

Resveratrol's Multifaceted Potential in Alzheimer's Disease: Insights from Preclinical and Clinical Evidence.

Alzheimer's disease (AD) is a devastating neurodegenerative disorder characterized by amyloid-beta (Aβ) plaques, neurofibrillary tangles (NFTs), and neuroinflammation, leading to progressive cognitive decline. Research has increasingly emphasized the importance of a nutritious diet rich in phytochemicals that promotes brain health and potentially mitigates the risk or progression of neurodegenerative diseases, such as AD. Resveratrol (RSV), a natural polyphenol with known anti-inflammatory, antioxidant, antiapoptotic, and neuroprotective properties, has thus garnered significant attention. Thi

Proteostasis and Neuroinflammation in Alzheimer's Disease.

Alzheimer's disease (AD) is the most common cause of dementia in older adults, marked by a gradual and irreversible deterioration of cognitive abilities, including memory and thinking skills. AD is highly heterogeneous, with variations in amyloid and tau pathology, symptoms, proteostasis, neuroinflammation, and genetics. Dysregulated proteostasis and neuroinflammation, though usually protective, contribute significantly to disease progression. Proteostasis refers to the network that maintains the integrity of both intracellular and extracellular proteins, while neuroinflammation is the biologi

Catechins and Human Health: Breakthroughs from Clinical Trials.

Green tea, derived from the unoxidized leaves of Camellia sinensis (L.) Kuntze, is one of the least processed types of tea and is rich in antioxidants and polyphenols. Among these, catechins-particularly epigallocatechin gallate (EGCG)-play a key role in regulating cell signaling pathways associated with various chronic conditions, including cardiovascular diseases, neurodegenerative disorders, metabolic diseases, and cancer. This review presents a comprehensive analysis of recent clinical studies focused on the therapeutic benefits and potential risks of interventions involving green tea extr

Does adjunctive electro-convulsive therapy improve the speed of treatment response in schizophrenia? A systematic review and week-by-week meta-analyses of controlled clinical trials.

BACKGROUND: Electroconvulsive therapy (ECT) is sometimes recommended in patients with schizophrenia to hasten clinical response. However, there is limited research comparing the speed of response between ECT in combination with antipsychotic medication versus antipsychotics alone. We conducted a systematic review and meta-analyses of week-by-week clinical response to examine whether ECT augmentation leads to faster improvement in psychotic symptoms in patients with schizophrenia. METHODS: We searched PubMed/Medline and Cochrane Library for controlled trials which compared the combination of E

Cumulative Logit Ordinal Regression With Proportional Odds Under Nonignorable Missing Responses-Application to Phase III Trial.

Missing data are inevitable in clinical trials, and trials that produce categorical ordinal responses are not exempted from this. Typically, missing values in the data occur due to different missing mechanisms, such as missing completely at random, missing at random, and missing not at random. Under a specific missing data regime, when the conditional distribution of the missing data is dependent on the ordinal response variable itself along with other predictor variables, then the missing data mechanism is called nonignorable. In this article, we propose an expectation maximization based algo

A Bayesian approach towards the identification of latent subgroups.

In clinical trials, it is often of interest to know whether treatment works differently for some groups than others, known as heterogeneity of treatment effect. Such subgroup analysis is complicated to conduct because trials are typically not powered to find subgroups. Furthermore, it is difficult to identify characteristics of patients pertaining to such subgroups. In this article, we propose a semiparametric mixture model to identify subgroups with time-to-event outcomes. Specifically, we assume a proportional hazards model with subgroup-specific piecewise constant baseline hazards, where th

Consensus position statements for the standardized application of histological grading and staging systems in MASH clinical trials.

BACKGROUND & AIMS: Assessment of histologic disease activity (grading) and fibrosis (staging) is a prerequisite for patient selection and evaluation of treatment response in clinical trials of metabolic dysfunction-associated steatohepatitis (MASH). The lack of universally accepted definitions of histological components required for metabolic dysfunction-associated steatotic liver disease (MASLD) grading and staging, inconsistent histologic interpretations, and the absence of guidelines on how to apply currently used scoring systems, contribute to high interobserver variation and influence sup

Exploring the Efficacy and Safety of Ketamine for Managing Acute Renal Colic in Emergency Departments: A Systematic Review of Recent Clinical Trials.

Kidney stones typically present as renal colic in emergency departments (EDs), where patients experience severe pain and often require parenteral therapy for symptom management. The economic burden associated with managing kidney stones exceeds USD 5 billion annually in the US and accounts for more than a million visits to EDs each year. There is clear evidence emphasizing the need for innovative and alternative pain control options for patients with renal colic. Recent randomized controlled trials suggest that intranasal (IN) and intravenous (IV) ketamine are as effective as parenteral NSAIDs

Intentional heterogeneity in autologous cell-based gene therapies: strategic considerations for first-in-human trials.

Cell-based gene therapies, including chimeric antigen receptor-T, T-cell receptor-T, and tumor-infiltrating lymphocyte therapies, have transformed the treatment landscape for certain cancers, yet their efficacy in solid tumors remains limited. Next-generation therapies aim to overcome biological barriers, enhance potency and safety, and streamline development timelines through innovative approaches. Recent advances in genome editing technologies have identified hundreds of gene edits that improve T-cell functionality in preclinical models. However, the limited direct translatability of these f

Documentation of patient withdrawals, retention strategies, and postwithdrawal data practices in cancer clinical trials.

BACKGROUND: The withdrawal of patients from cancer clinical trials with unspecified rationale introduces selection bias, compromises study validity, and reduces generalizability. Excluding these data can lead to informative censoring, masking treatment toxicity, or inflating efficacy estimates. Whereas regulatory agencies emphasize documenting reasons for withdrawal and retaining data after withdrawal, adherence to these guidelines is unclear, raising concerns about trial integrity. The objectives of this study were to determine the prevalence of withdrawal with unspecified rationale, evaluate

Investigating the Latest Evidence from Phase III Trials Supporting Treatment Options for De novo Clinically Lymph Node-Positive Hormone-Sensitive Prostate Cancer.

PURPOSE OF REVIEW: The introduction of PSMA-PET/CT scans is expected to increase the incidence of clinically lymph node-positive metastatic hormone-sensitive prostate cancer (mHSPC). The 8th AJCC-TNM classify disease with metastasis limited to pelvic nodes (cN1M0) and nonregional lymph nodes (M1a) as stage IV. To date, there is limited prospective evidence for management of this subgroup. Additionally, no specific recommendations currently exist for managing M1a as a distinct condition but as a part of CHAARTED low volume disease (LVD). Our review examines relevant results from phase III trial
