The double-edged sword of goal engagement: consequences of goal pursuit in bipolar disorder.

A series of studies suggest that bipolar disorder is related to high sensitivity to incentives and that incentive sensitivity (or sensitivity of the approach system) can predict the course of mania. Incentive sensitivity in bipolar disorder seems to be related to two processes: a tendency to invest in difficult-to-attain goals and an over-reactivity to cues of goal progress versus thwarting. Both of those processes appear relevant to symptom generation. Hence, bipolar disorder seems related to a greater emphasis on reaching goals and also a problematic reactivity to reaching those highly desir

A randomized controlled trial of mindfulness-based cognitive therapy for bipolar disorder.

OBJECTIVE: To compare the efficacy of mindfulness-based cognitive therapy (MBCT) plus treatment as usual (TAU) to TAU alone for patients with bipolar disorder over a 12-month follow-up period. METHOD: Participants with a DSM-IV diagnosis of bipolar disorder were randomly allocated to either MBCT plus TAU or TAU alone. Primary outcome measures were time to recurrence of a DSM-IV major depressive, hypomanic or manic episode; the Montgomery-Åsberg Depression Rating Scale (MADRS); and Young Mania Rating Scale (YMRS). Secondary outcome measures were number of recurrences, the Depression Anxiety St

Comparison of attention training and cognitive therapy in the treatment of social phobia: a preliminary investigation.

BACKGROUND: Prominent models of social phobia highlight the role played by attentional factors, such as self-focused attention, in the development and maintenance of social phobia. Elevated self-focused attention is associated with increases in self-rated anxiety. Treatments that aim to modify and change attentional processes, specifically self-focused attention, will have a direct effect on social phobia symptoms. Thus, Attention Training targets attentional focus. AIM: The present study aimed to investigate the efficacy of Attention Training in comparison to an established treatment for soc

Who benefits most from guided self-help for binge eating? An investigation into the clinical features of completers and non-completers.

Guided self-help (GSH) is a recommended first step in treatment for bulimia nervosa (BN) and binge eating disorder (BED) (NICE, 2004). It remains unclear what makes some individuals more likely to respond to this form of treatment than others. Forty-eight patients participated in this study using a GSH programme for binge eating. Profiles of treatment completers and non-completers are compared, and reasons for non-completion explored. Completion of treatment was associated with significant improvements in mood, general functioning and on measures of dietary restraint, frequency of objective bi

Does avoidant personality disorder impact on the outcome of treatment for bulimia nervosa?

OBJECTIVE: To examine the impact of avoidant personality disorder (AVPD) on bulimia nervosa (BN) treatment outcome over 3 years. METHOD: Women with BN were participating in a randomized treatment trial. The sample was split into: AVPD (n = 37), other PD (n = 37), and no PD (n = 60). Eating disorder symptomatology, depressive symptoms and psychosocial functioning were examined at pretreatment and follow-up. Multiple regression was conducted to control for high axis I comorbidity. RESULTS: There were no significant differences across the groups at pretreatment or follow-up on eating disorder s

Young people's perceptions of and reasons for accessing a web-based cognitive behavioural intervention for bulimia nervosa.

OBJECTIVE: The aim of the present study was to explore young people's experience of accessing and utilising a web-based cognitive behavioural intervention for bulimic disorders. METHOD: Young people (N = 11) with bulimia nervosa or atypical bulimia nervosa completed a qualitative interview. Interviews were analysed and themes were identified. RESULTS: Young people liked the programme for its accessibility, flexibility, support and information. Some participants used the intervention as a stepping stone to further treatment. CONCLUSION: Web-based interventions have the potential to provide a

Pushed and pulled: the role of affect and cognition in shaping CAM attitudes and behavior among men treated for prostate cancer.

We used Self-Regulation Theory (SRT) to elaborate and describe cognitive and affective representations presumed to underlie CAM decisions in a sample of 55 men who were using CAM after having received conventional treatment for early-stage, localized prostate cancer. Positive representations of CAM were the strongest predictors of beliefs that CAM was necessary for one's health (a 'pulling' effect toward CAM). Nonetheless, negative representations of conventional medicine made an important, though somewhat lesser contribution (a ;pushing' effect away from conventional medicine and towards CAM)

Treating eating disorders in primary care.

Binge-eating disorder, bulimia nervosa, and anorexia nervosa are potentially life-threatening disorders that involve complex psychosocial issues. A strong therapeutic relationship between the physician and patient is necessary for assessing the psychosocial and medical factors used to determine the appropriate level of care. Most patients can be effectively treated in the outpatient setting by a health care team that includes a physician, a registered dietitian, and a therapist. Psychiatric consultation may be beneficial. Patients may require inpatient care if they are suicidal or have life-th

Long-term goals in the management of acute and chronic anxiety disorders.

Many anxiety disorders are not treated to remission (symptom-free state); however, this should be the minimum goal of therapy. Antidepressant therapies have shown significant beneficial effects in the management of anxiety disorders, with some variability in results in specific disorders. In social anxiety disorder, selective serotonin reuptake inhibitors and venlafaxine extended release (XR) have demonstrated efficacy, with response rates varying between 40% and 68%. Monoamine oxidase inhibitors and cognitive-behavioural therapies are also effective. In patients with generalized anxiety disor

Prediction of outcome in bulimia nervosa by early change in treatment.

OBJECTIVE: The authors' goal was to identify predictors of treatment response in bulimia nervosa and, in particular, to attempt to replicate and extend the observation that early change predicts outcome. METHOD: Predictors of response at the end of treatment and 8-month follow-up were sought from a group of 220 women treated with either cognitive behavior therapy or interpersonal psychotherapy. RESULTS: Early change in frequency of purging was the best predictor of response at the end of treatment and at 8-month follow-up. CONCLUSIONS: Early change in treatment is a robust and potent predic

Cognitive treatment of pathological gambling.

This study evaluated the efficacy of a cognitive treatment package for pathological gambling. Sixty-six gamblers, meeting DSM-IV criteria for pathological gambling, were randomly assigned to treatment or wait-list control conditions. Cognitive correction techniques were used first to target gamblers' erroneous perceptions about randomness and then to address issues of relapse prevention. The dependent measures used were the South Oaks Gambling Screen, the number of DSM-IV criteria for pathological gambling met by participants, as well as gamblers' perception of control, frequency of gambling,

New developments in panic disorder.

Panic disorder is a distressing and disabling disorder characterized by recurring panic attacks; anticipatory anxiety about having additional attacks; and agoraphobic fear and avoidance of situations in which attacks have occurred, ready escape is difficult, or help is unavailable in the event of an attack. It is typically associated with impairment in social, emotional, and vocational functioning, as well as excessive utilization of the medical care system. A number of pharmacologic agents and cognitive-behavioral treatments have proven effective for the treatment of panic disorder. Selective

Emotional processing, three modes of mind and the prevention of relapse in depression.

Rachman's (1980) analysis [Rachman, S. (1980). Emotional processing. Behaviour Research and Therapy, 18, 51-60] of emotional processing is extended and applied to the prevention of relapse and recurrence in depression. It is proposed that effective emotional processing leads to changes in the ability of triggering cues to reactivate depressogenic processing cycles at times of potential relapse. Available evidence supports the usefulness of the probe methodology of the emotional processing framework as a way to investigate processes mediating relapse prevention by cognitive therapy. It is propo

The modification of breathing behavior. Pavlovian and operant control in emotion and cognition.

The purpose of this article is to bring attention to breathing as a behavior that can be modified by means of Pavlovian and operant principles of control. With this aim in mind, this paper (a) reviews a selection of early and recent conditioning studies (Pavlovian and operant paradigms) in respiratory psychophysiology, (b) discusses the bidirectional relationship between breathing and emotion/cognition, and (c) discusses theoretical and applied implications that point to new directions for research in the laboratory and clinic. Emphasis is placed on dyspnea/suffocation fear and the acquisition

Long-term outcome of panic disorder and social phobia.

The question is addressed as to whether cognitive-behavior treatment delivered as a routine service in a specialist clinic is effective in the long term. Of 124 consecutive patients completing treatment for panic disorder or social phobia, 93 were assessed an average of 2 years following treatment. The treatment produced significant improvement in measures of symptoms, avoidance, and disablement during treatment and further significant improvement during the follow-up. A quarter of participants no longer met diagnostic criteria, had not sought further treatment, and their anxiety had not troub

Overprediction of fear in panic disorder patients with agoraphobia: does the (mis)match model generalize to exposure in vivo therapy?

The purpose of this study was to test the (mis)match model of Rachman and co-workers during real life exposure therapy in panic disorder patients with agoraphobic avoidance. The results showed that although the patients tended to overpredict their expected fear before the exposure sessions, their predictions did not show a tendency to become more accurate in the course of treatment. After an overprediction predicted fear tended to decrease and self-efficacy tended to increase; after a correct match both predicted fear and self-efficacy remained unchanged. Patients reports of fear tended to dec

Carbon dioxide hypersensitivity, hyperventilation, and panic disorder.

OBJECTIVE: The purpose of this article is to offer a comprehensive, data-based explanation of the relationship between hyperventilation and panic disorder linking CO2 hypersensitivity, cognitive/behavioral factors, and the respiratory effects of antipanic pharmacologic and psychological treatments. METHOD: The authors conducted a computerized search of MEDLINE for relevant articles. RESULTS: Some panic patients have a chronic, subtle respiratory disturbance. Acute hyperventilation is neither necessary nor sufficient for panic to occur. Respiratory abnormalities in panic patients may adaptive
