Care of patients with long-term indwelling urinary catheters.

The complications and management of long-term indwelling catheters used for urinary retention and incontinence were reviewed. Research evidence from 1992 - 2002 was located through searches of CINAHL (38 articles), and Medline (89 articles). Fifty studies were critiqued for this review. The most common complications of long-term indwelling catheters are bacteriuria, encrustation, and blockage. Less common is the prevalence of bacteremia and renal disease. Risk factors for bacteriuria include female gender, older age, and long-term indwelling catheter use. Urinary white blood cells are the best

Outbreak of Clostridium difficile-associated disease in a small animal veterinary teaching hospital.

An apparent outbreak of enteric disease occurred in dogs and cats at a veterinary teaching hospital. Clostridium difficile Toxin A or B or both were identified in 1 or more fecal samples from 48 of 93 (52%) dogs over a 5-month period, 30 of which were identified in the 1st 26 days, after which strict infection control measures, including closure to elective cases, were implemented. Affected animals included in-patients, out-patients that were housed temporarily in the wards, and resident blood donor dogs. Infection control measures, including partial depopulation, isolation, hospital and yard

Urinary tract infections. How to manage nursing home patients with or without chronic catheterization.

Urinary tract infections (UTIs)--including cystitis, pyelonephritis, and catheter-associated infections--are among the most common nursing home-acquired infections. Asymptomatic bacteriuria can be identified in 20 to 50% of nursing home residents who do not have bladder catheters and in 100% of those who do. Diagnostic tests for nursing home patients with suspected UTI include urinalysis, urine culture, and sensitivity testing. Treatment of cystitis can usually be managed in the nursing home with oral antibiotics. Initial therapy with a parenteral agent is often recommended in nursing home-acq

Nosocomial infections.

Nosocomial infections and antimicrobial resistance are topics that have been intensely studied in human medicine because of their significant impact on human health. In recent years, concerns have been raised that the use of antibiotics in veterinary medicine, animal husbandry, and agriculture may be contributing to the development of resistance in common bacterial species affecting human beings. Although there is inadequate proof at this time that the resistance is transmitted from animals to people, if antibiotics continue to be used indiscriminately in veterinary medicine, veterinarians may

Respiratory tract pathogens isolated from patients hospitalized with suspected pneumonia: frequency of occurrence and antimicrobial susceptibility patterns from the SENTRY Antimicrobial Surveillance Program (United States and Canada, 1997).

Thirty-seven sentinel hospitals (29 in the United States [US]; eight in Canada) collected bacterial isolates from hospitalized patients with a diagnosis of pneumonia. The antimicrobial susceptibility patterns of these pathogens were determined to more than 60 agents (40 reported) using the reference broth microdilution method described by the National Committee for Clinical Laboratory Standards. The five most frequently recorded species among the 2757 isolates collected during the study were (no. tested/%): Staphylococcus aureus (632/22.9%), Pseudomonas aeruginosa (498/18. 1%), Haemophilus inf

Incidence and susceptibility of pathogenic bacteria vary between intensive care units within a single hospital: implications for empiric antibiotic strategies.

BACKGROUND: The purpose of this study was to determine whether the incidence of recovery and patterns of antibiotic susceptibility of pathogenic bacteria vary between intensive care units (ICUs) in a single teaching hospital. METHODS: Culture and susceptibility results were collected prospectively for a 3-month period (April through June 1999) in each of the surgical, trauma, and medical ICUs. The number of unique isolates and susceptibility patterns were determined. Susceptibility of isolates among ICUs was compared with chi2. RESULTS: Statistically significant differences between ICUs in s

[Evaluation of Mycobacterium kansasii isolates from a clinical laboratory in a city hospital].

Fifteen isolates of Mycobacterium kansasii in Showa University Fujigaoka Hospital between 1982 and 1995 were investigated. Comparing by gender, 13 were isolated from male patients and only two were isolated from female patients. The average of cases was 48 years old and 14 out of 15 cases (93%) were isolated from respiratory tract specimens. The rate of the smear- and culture-positives was 64%, which was significantly higher than that (26%) of M. avium complex (p < 0.01 by chi 2 test). All 4 isolates were susceptible to rifampicin (10 micrograms/ml) by drug susceptibility testing using Ogawa e

Delayed presentation of false abdominal aortic aneurysm following umbilical artery catheterisation.

An eight-month-old male child presented with a nonpulsatile abdominal mass, which was detected during a routine follow-up examination. After ultrasound examination a tentative diagnosis of pseudoaneurysm of the abdominal aorta was made. An umbilical artery catheterisation had been performed for procuring arterial blood gases after birth for treatment of birth hypoxia. There was a history of fever subsequent to the umbilical artery catheterisation with positive blood and catheter tip cultures for coagulase-positive Staphylococci and Klebsiella pneumoniae. He had also suffered from infectious ar

[Infectious diseases in patients with solid tumors].

In malignant diseases the prognosis of tumour and the patient's quality of life are essentially influenced by the kind and severity of the immunodeficiency. The high rate of infection of up to 40% is effected by both the basic disease and the different methods of tumour therapy as well as by numerous other endogenous and exogenous factors. Especially the myelosuppressive effect of cytostatics favours the lowered defense mechanism. Hence severe infections stand in causal relation to almost 2/3 of all deaths caused by carcinoma. Resulting from these facts it is necessary to have a tight antimicr

[An evaluation of the sensitivity to antiseptic preparations of clinical strains of microorganisms in the family Enterobacteriaceae].

Classical enterobacterial strains are sensitive to the working concentrations of pervomur, dioxydine, resorcinol, sodium sulfacyl, iodopyrone, chlorhexidine and boric acid, resistant to the action of cetyplyridinium chloride, rivanol, roccal and ethonium. In enterobacterial populations strains with acquired resistance to chloramine B, iodopyrone, chlorhexidine and resorcinol are present. Hospital strains of enterobacteria are characterized by higher, in comparison with extrahospital strains, resistance to ethonium, sodium sulfacyl, lodopyrone, chloramine B and resorcinol.

[Urinary Corynebacterium group D2 infections. A hospital epidemic and antibiotic sensitivity].

An outbreak of Corynebacterium group D2 urinary tract infections was recently observed at Nice CHR. These infections were characterized by alkaline urine. The five first cases were reported in the same department and the ten other cases were latter observed in different departments. Age average was 79 and all patients had either an intra-vesical catheter of a urinary collector. Among samples of material tested, only one gave a positive culture. The 15 isolates showed a similar identification and antibiotic phenotype. These strains presented a multiple antibiotic resistance: resistance to beta-

Salmonella bacteremia in systemic lupus erythematosus. Eight-year experience at a municipal hospital.

Non-endemic Salmonella bacteremia tends to occur in patients with chronic disease. We reviewed all cases of Salmonella infection documented in adults at Bellevue Hospital during the years 1975-1982. Unexpectedly, the most frequent underlying disease found among bacteremic patients was systemic lupus erythematosus (SLE). Patients with SLE accounted for 6 of 30 Salmonella bacteremias as compared with 13 of 2,388 non-Salmonella gram-negative bacteremias. Salmonella was the single most frequent gram-negative isolate from the blood of SLE patients. All lupus patients with Salmonella infection were

Enterococcal bacteremia without endocarditis.

Seventy-four cases of enterococcal bacteremia without endocarditis were reviewed retrospectively for the years 1963 through 1977. Thirty-nine patients had hospital-acquired infection, 27 had serious underlying disease, and 18 had polymicrobial bacteremia. The organisms isolated from the primary source of infection were similar in patients with pure enterococcal or polymicrobial bacteremia. The most common source was the urinary tract, followed by the abdomen, infected burns, and soft-tissue infections other than burns. The overall mortality was 34%, with significantly higher mortality in immun

Staphylococcal infection in subdivided general surgical wards.

The spread of Staphylococcus aureus was studied in three general surgical wards of identical design which consisted of a number of separate rooms, and the results were compared with those in the same unit previously studied, in subdivided wards elsewhere and in large open wards. The nasal carrier rate of Staph. aureus by patients rose during their stay in the ward, but its peak rate was comparable with minimum rates reported in other subdivided wards and was lower than in open wards.Staphylococcal sepsis rates were lower than in most open wards and were also much lower than those found previous

The light chain of the L9 antibody is critical for binding circumsporozoite protein minor repeats and preventing malaria.

L9 is a potent human monoclonal antibody (mAb) that preferentially binds two adjacent NVDP minor repeats and cross-reacts with NANP major repeats of the Plasmodium falciparum circumsporozoite protein (PfCSP) on malaria-infective sporozoites. Understanding this mAb's ontogeny and mechanisms of binding PfCSP will facilitate vaccine development. Here, we isolate mAbs clonally related to L9 and show that this B cell lineage has baseline NVDP affinity and evolves to acquire NANP reactivity. Pairing the L9 kappa light chain (L9κ) with clonally related heavy chains results in chimeric mAbs that cross

Indirect hemagglutination test of Rickettsia orientalis: its specificity for reference strains, Karp, Gilliam and Kato.

Amberlite XE-64 and bovine serum albumin-treated rickettsia suspension, which was prepared from the chicken yolk sacs infected with each of 3 reference strains of Rickettsia orientalis, Karp, Gilliam and Kato, was sonicated at 10 KC for 15 min at 4 degrees C and centrifuged at 10,000 rpm for 40 min at 0 degrees C. The supernatant was used to sensitize the formalinized and tanned sheep red blood cells (SFTSRC). This antigen (2.5%) could be preserved for at least 1 wk at 4-8 degrees C and at least a month, if merthiolate (1:10,000) was added. Each SFTSRCP of 3 reference strains was found to be s
