Tarlov cysts: a controversial lesion of the sacral spine.

The primary aim of our study was to provide a comprehensive review of the clinical, imaging, and histopathological features of Tarlov cysts (TCs) and to report operative and nonoperative management strategies in patients with sacral TCs. A literature review was performed to identify articles that reported surgical and nonsurgical management of TCs over the last 10 years. Tarlov cysts are often incidental lesions found in the spine and do not require surgical intervention in the great majority of cases. When TCs are symptomatic, the typical clinical presentation includes back pain, coccyx pain,

Impact of perioperative dexamethasone on postoperative complications in diabetic patients undergoing spine surgery.

BACKGROUND: Dexamethasone is commonly used perioperatively for analgesic and antiemetic benefit, however, the risk of postoperative hyperglycemia is concerning, particularly among diabetic patients undergoing spine surgery, where extensive tissue exposure and hardware implantation may increase risk of wound complications. The influence of baseline glycemic control and surgical approach on dexamethasone-associated risk remains poorly characterized. METHODS: A retrospective cohort study using the TriNetX United States Collaborative Network, a nationally representative database comprised of over

Hip Arthroscopy After Total Hip Arthroplasty: A Current Review of Indications, Techniques, and Outcomes.

PURPOSE OF REVIEW: Persistent pain after total hip arthroplasty (THA) is a common complication requiring extensive diagnostic effort and is often associated with potentially invasive and morbid treatment options. With THA volume expected to steadily increase there is a similarly growing need for creative and effective diagnostic and therapeutic options for these clinically challenging patients. Hip arthroscopy has emerged as a promising tool in the setting of persistent pain after THA with expanding indications and promising outcomes. The purpose of this article was to provide a review of the

P2X7 receptor antagonism with Brilliant Blue G promotes functional recovery in spontaneous canine spinal cord injury: a preliminary study in a large animal model.

BACKGROUND: Secondary injury cascades critically influence neurological outcomes after severe spinal cord injury (SCI), yet effective disease-modifying pharmacological interventions remain lacking. Extracellular ATP-mediated activation of the P2X7 receptor is a key driver of post-traumatic neuroinflammation, glial activation, and progressive tissue loss. While P2X7 receptor antagonism improves functional recovery in experimental rodent models of SCI, its translational relevance in naturally occurring large-animal injury has not been established. METHODS: We conducted a prospective, uncontroll

The symptom spectrum: preoperative symptom-related indicators of surgical outcomes for adult type 1 chiari malformation.

Chiari I malformation (C1M) may necessitate surgical decompression in adulthood due to the emergence of debilitating neurological symptoms. The outcomes of surgical management for C1M in the adult population are variable and predictive value of individual symptoms has been less extensively studied. A retrospective cohort study was conducted on adult patients (≥ 18 years) who underwent posterior fossa decompression surgery (PFDS) for C1M at our institution between 2004 and 2024. Univariate and multivariate analyses were performed to evaluate the predictive value of preoperative C1M-related symp

Development and validation of a Bayesian network predicting neurosurgical intervention after injury in children and adolescents.

BACKGROUND: Timely surgical decompression improves functional outcomes and survival among children with traumatic brain injury and increased intracranial pressure. Previous scoring systems for identifying the need for surgical decompression after traumatic brain injury in children and adults have had several barriers to use. These barriers include the inability to generate a score with missing data, a requirement for radiographic imaging that may not be immediately available, and limited accuracy. To address these limitations, we developed a Bayesian network to predict the probability of neuro

Esophageal stenting with minimally-invasive surgical intervention for delayed spontaneous esophageal perforation.

BACKGROUND: Spontaneous esophageal perforation is a challenging surgical emergency with significant morbidity and mortality, and timely primary repair carries good outcomes. However, direct repair for a delayed spontaneous esophageal perforation is not always feasible and is associated with high mortality. Esophageal stenting can provide therapeutic benefits in the management of esophageal perforations. In this study, we review our experience with placing esophageal stents in combination with minimally-invasive surgical drainage to treat delayed spontaneous esophageal perforations. METHODS: W

Common trunk anomaly of the anterior and posterior inferior cerebellar artery in hemifacial spasm.

BACKGROUND: The common trunk anomaly of the anterior and posterior inferior cerebellar artery (APC) is a variant artery that causes a hemifacial spasm (HFS). The anatomical characteristics include a large diameter of the trunk and the existence of the bifurcation near the facial nerve root entry zone (REZ). Despite APC being encountered at a constant rate in microvascular decompression (MVD), the anatomical and technical issues of transposing APC have not been entirely focused on yet. METHODS: We reviewed our 68 cases with APC involvement. Patient background, radiological findings, and operat

A Rare Presentation: Cauda Equina Compression Secondary to an L1 Burst Fracture in Osteoporosis.

Cauda equina syndrome (CES) rarely occurs in upper lumbar spinal pathologies above L2. Osteoporosis is a consideration in determining the operative approach. We report a case of CES as a result of an L1 burst fracture in an osteoporotic lady with schizophrenia. A 74-year-old schizophrenic lady presented with traumatic lower back pain with no neurological deficit. Due to her psychiatric condition, the clinical assessment was challenging. On day 3 of admission, there was an acute total loss of motor function over bilateral L2-L3 myotomes to MRC grade 0/5, progressively involving bilateral L2-S1

Artificial disc replacement and adjacent-segment pathology: 10-year outcomes of a randomized trial.

OBJECTIVE: Artificial disc replacement (ADR) is designed to preserve motion and thus protect against adjacent-segment pathology (ASP) and act as an alternative treatment to fusion surgery. The question remains, how well do ADR devices perform after 10 years of follow-up compared with fusion surgery in terms of patient satisfaction, sustainability, and protection against ASP? METHODS: This was the 10-year follow-up study of 153 participants who underwent ADR or fusion surgery after anterior decompression due to cervical degenerative radiculopathy (ISRCTN registration no. 44347115). Scores on t

Effectiveness of endolymphatic duct blockage versus endolymphatic sac decompression in patients with intractable Ménière's disease: study protocol for a double-blinded, randomised controlled trial.

INTRODUCTION: Outcomes of surgery for Ménière's disease (MD) remain discordant. Recently, a new surgical procedure in which the endolymphatic duct is clipped was proposed. To date, only one prospective trial assessing this technique was published, yielding promising results. This protocol describes a prospective, double-blinded, randomised controlled trial that will be carried out to assess the effectiveness of this surgical intervention. METHODS: Eighty-four patients with intractable MD will be recruited from 13 hospitals in the Netherlands. Intraoperatively, randomisation will determine whe

Surgical management of Chiari malformation type II.

BACKGROUND: Chiari malformation type II is present in almost all patients with myelomeningocele but usually remains asymptomatic. Symptoms are generally more severe in neonates, who have the worst prognosis. The association symptoms/hydrocephalus is well known, and first treatment usually consists of ensuring adequate ventricular drainage. Craniovertebral decompression may be required in patients who do not improve after drainage. However, mechanisms of symptom development are not yet completely understood, timing and techniques of surgery are not codified, long-term evolution is poorly report

Acute upper limb compartment syndrome in children: special focus on nerve recovery.

BACKGROUND: Many studies have discussed acute compartment syndrome in children associated with or without fractures and have given their visible perspectives. Little is known about the nerve involvement and the factors associated with recovery patterns in these patients. We intend to propose that ischemic nerve and muscles tend to regenerate after surgical decompression but in a different pattern and the given circumstances. METHODS: Twenty-four children with acute compartment syndrome in the upper limb were analyzed between 2009 and 2015. Data included demographic features of these patients,

Complications following posterior cervical decompression and fusion: a review of incidence, risk factors, and prevention strategies.

Posterior cervical decompression and fusion (PCF) is a common surgical technique used to treat various cervical spine pathologies. However, there are various complications associated with PCF that can negatively impact patient outcome. We performed a comprehensive literature review to identify the most common complications following PCF using PubMed, Cochrane Database of Systematic Reviews, and Google Scholar. The overall complication rates of PCF are estimated to range from about 15% to 25% in the current literature. The most common immediate complications include acute blood loss anemia, sur
