Evaluating the Impact of CYP1A2 Inhibition on the Pharmacokinetics of Savolitinib: A Phase 1 Open-Label Study.

Savolitinib is an oral, potent, selective CNS-penetrant MET-tyrosine kinase inhibitor that in vitro studies indicate is metabolized by cytochrome P450 (CYP) enzymes, including CYP1A2, and by non-CYP enzymes, such as aldehyde oxidase (AO). Savolitinib primary metabolites, M2 and M3, are formed mainly by CYP1A2 and AO, respectively. This Phase 1, open-label study evaluated the impact of CYP1A2 inhibition by fluvoxamine on the pharmacokinetics of savolitinib and its metabolites in 16 healthy male volunteers. Volunteers received a single oral dose of 300 mg savolitinib and, following a ≥ 10-day wa

Anti-vertigo drug prescribing for patients with vestibular symptoms in primary care: a retrospective observational cohort study.

BACKGROUND: There is limited evidence that anti-vertigo drugs (AVDs) are effective in patients with vestibular symptoms. Still, betahistine is one of the most frequently prescribed off-label drugs. GPs are likely to contribute substantially to these potentially inappropriate prescriptions. AIM: To evaluate the frequency of (long-term) AVD prescriptions in primary care and characteristics associated with long-term prescriptions. DESIGN & SETTING: We conducted a retrospective observational cohort study using anonymised routine primary care data from ≥1.2 million patients registered at 269 gene

FDA Approval Summary: Idecabtagene Vicleucel for the Treatment of Triple-Class-Exposed, Relapsed or Refractory Multiple Myeloma.

On April 4, 2024, the FDA approved idecabtagene vicleucel (ide-cel) for adults with relapsed or refractory multiple myeloma after two or more prior lines of therapy, including an immunomodulatory agent, a proteasome inhibitor, and an anti-CD38 monoclonal antibody. Approval was based on KarMMa-3, a randomized, open-label trial in 386 patients. The study compared a single infusion of ide-cel to standard-of-care treatment consisting of the investigator's choice of five anti-myeloma regimens. The primary endpoint was progression-free survival, which was significantly longer in the ide-cel arm [HR

Development and validation of an LC-MS/MS multiplex assay for the quantification of bedaquiline, n-desmethyl bedaquiline, linezolid, levofloxacin, and clofazimine in dried blood spots.

Dried blood spot (DBS) assays to quantify novel and repurposed drugs for the treatment of rifampicin-resistant tuberculosis (RR-TB) would facilitate pharmacokinetic studies and therapeutic drug monitoring in low-middle income settings, considering their ease of application and simple sample storage requirements. We describe a DBS method for the simultaneous quantification of bedaquiline and metabolite N-desmethyl bedaquiline, linezolid, levofloxacin, and clofazimine. The analytes were extracted from the matrix and isolated by solid-phase extraction. Two LC-MS/MS systems were used, optimized fo

Human nerve growth factor delivery to the retina: Quantitative methodology and mathematical modeling in preclinical settings.

Nerve growth factor (NGF) plays a critical neuroprotective role in retinal health, supporting neuronal survival and regeneration. Recombinant human NGF (rhNGF) holds promise for treating retinal degenerative diseases such as glaucoma, retinitis pigmentosa, and optic neuropathies. However, efficient retinal delivery of rhNGF remains a major challenge due to anatomical barriers and rapid clearance from conventional routes. Here, we integrate in vivo experimentation with mathematical modeling to identify and validate optimized delivery strategies for rhNGF. By using stable isotope-labeled rhNGF,

Real-World Effectiveness of Advanced Therapies in Microscopic Colitis.

BACKGROUND & AIMS: Microscopic colitis (MC) is a leading cause of chronic diarrhea, particularly in older adults. Although many patients respond to budesonide, refractory and dependent cases pose major therapeutic challenges. Although the off-label use of advanced inflammatory bowel disease therapies is expanding, robust real-world data remain scarce. METHODS: Through the ECCO CONFER network, we conducted a multinational, retrospective study in patients with MC treated with biologics or small molecules following budesonide failure or intolerance. We systematically analyzed clinical outcomes,

Establishment of a R-Sphere labelled immunoassay platform and its rapid combined quantitative detection of ketamine and morphine in serum.

BACKGROUND AND AIMS: Ketamine and morphine are among the most commonly abused drugs. Ketamine is classified as a new psychoactive substance, while morphine represents the opioid class. These two substances may exhibit synergistic or antagonistic effects in their pharmacological actions, and their combined abuse could lead to more severe health risks, such as respiratory depression. Therefore, simultaneous detection is crucial for comprehensive monitoring of drug abuse. This study developed a test strip capable of detecting both ketamine (KET) and morphine (MOP) in serum, utilizing Raman spectr

Personalized therapy with CFTR modulators: Response of p.Ile148Asn variant.

BACKGROUND: Elucidating the molecular and cellular effects caused by CFTR variants is crucial to understand Cystic Fibrosis (CF) disease pathophysiology, but also to predict disease severity, to provide genetic counselling, and to determine the most adequate therapeutic strategy for people with CF (pwCF). While the current CFTR modulator drugs (CFTRm) are approved mainly for pwCF with the most prevalent variant, p.Phe508del, pwCF carrying rare and/or uncharacterized CFTR variants are not eligible. However, previous studies have shown that such rare variants can be rescued by the approved CFTRm

Effect of Surface Properties of Chitosan-Based Nanoparticles in the Skin-Diffusion Rate.

Skin diseases may cause rash, inflammation, itchiness, and other important skin changes, including dysplasia. Some skin conditions may be due to genetic and lifestyle factors and immune-mediated factors. The current skin disease treatment can include oral medication, topical cream, or ointments. Nanotechnology is revolutionizing the drug delivery systems, increasing the time life of active therapeutic compounds and improving the treatment efficiency. This work hypothesizes that varying the surface properties of chitosan nanoparticles (Ch-NPs) can modulate their diffusion through dermal tissue.

Endo-chip laser-induced thrombus formation: a vessel-on-chip model for in vitro testing of antithrombotic agents.

Mouse models, such as the intravital laser-induced model of thrombus formation, are commonly used for mechanistic and preclinical studies in thrombosis. However, their translational value is limited by species differences. Few in vitro models incorporate laser-induced vascular injury. Here, we developed an endothelialized microfluidic device, the Endo-chip, to model thrombus formation in response to laser injury. Citrated blood was treated with IXA4, an endoplasmic reticulum stress inducer, or with antithrombotic agents including antitissue factor antibody (5G9), antiplatelet drugs (abciximab,
