INTRODUCTION: Antimicrobial resistance is a major global public health challenge with particular implications in pediatrics. Antimicrobial stewardship programs (ASPs) require structured tools to assess prescribing quality, especially in complex clinical scenarios such as bloodstream infections and multidrug-resistant organism infections. The aim of this study was to design and implement a structured support program for managing these infections in a pediatric hospital, including an evaluation tool, a standard operating procedure, and its integration into the electronic health record.

Multiple myeloma (MM) is a complex hematologic malignancy characterized by the abnormal proliferation of plasma cells in the bone marrow. Despite advances in therapy, resistance remains a significant obstacle to curing the disease. Long non-coding RNAs (lncRNAs), a broad family of regulatory RNA molecules, have recently been recognized as important players in MM biology, influencing disease progression and treatment response. This review explores current insights into the diverse roles of lncRNAs in MM, focusing on their involvement in key molecular processes, including miRNA sponging, epigene

Candida auris has garnered substantial clinical and public health attention for its widespread antifungal resistance. Most isolates are resistant to fluconazole, and many, to other drug classes, with acquired resistance to all clinically available antifungal drugs reported. Antifungal resistance is rising alongside increasing case counts, threatening a sparse antifungal toolbox with multidrug and pan-resistant isolates that may cause untreatable infections. In this minireview, we examine the recent literature investigating the mechanisms and evolutionary patterns of resistance in clinical isol

Strong biofilm-forming multidrug-resistant (MDR) Escherichia coli (E. coli) can persist on food-contact surfaces and contribute to contamination along the food production chain. In this study, we investigated the occurrence, genomic features, and plasmid transferability of strong biofilm-forming E. coli from food-producing animals and associated farm environments, and evaluated an approach for controlling their surface-associated biofilms. Among 536 isolates, 42 were classified as strong biofilm producers (7.84%, 42/536). The highest proportion was observed in pig-derived isolates (10.11%, 36/

The rapid spread of multidrug-resistant (MDR) Escherichia coli poses a severe global health risk and demands the development of new antibacterial strategies. Biologically manufactured metal oxide nanocomposites have gained popularity due to their superior antibacterial activity and environmentally sustainable synthesis. This study uniquely integrates fungal metabolite profiling, green synthesis of a CuO/ZnO heterojunction nanocomposite, and molecular evaluation of anti-virulence activity against clinically isolated MDR Escherichia coli. The CuO/ZnO system was selected due to its synergistic he

The escalating crisis of multidrug-resistant bacteria necessitates innovative antibiotic discovery platforms. Conventional antimicrobial peptide (AMP) mining often relies on complete biosynthetic gene clusters (BGCs), leaving fragmented genomic resources underexplored. Here, we present an evolution-inspired approach to reconstruct and predict AMPs from partial BGCs. Applying this strategy to 954 Paenibacillus genomes identifies five polymyxin-like peptides, NP001-NP005, with broad in vitro activity. Crucially, in murine models of polymyxin-resistant infection, NP001 reduced bacterial burdens b

Acinetobacter baumannii is a leading cause of healthcare-associated infections and is classified among the highest-priority antimicrobial-resistant pathogens. Its clinical success reflects the convergence of antimicrobial resistance (AMR) and biological traits that promote environmental persistence and transmission. Acinetobacter baumannii has undergone a remarkable transformation over the past few decades, evolving from a relatively obscure environmental bacterium into a globally recognized multidrug-resistant pathogen. Its prevalence in healthcare settings, particularly intensive care units,

Acinetobacter baumannii is a Gram-negative opportunistic pathogen and a major cause of healthcare-associated infections, including ventilator-associated pneumonia, bacteraemia, meningitis, and urinary tract infections. Its persistence in hospital environments is due to its ability to survive desiccation, resist disinfectants, and colonize both biotic and abiotic surfaces. Virulence in A. baumannii is largely associated with structures such as Csu pili, biofilm-associated protein (Bap), and outer membrane protein A (OmpA), which enable surface attachment, biofilm formation, and host cell damage

Antimicrobial resistance (AMR) has become a major concern in the treatment of bacterial infections, and bacteriophage therapy has emerged as a promising alternative to antibiotics. Bacteriophages are highly specific to their bacterial hosts; hence, isolating phages indigenous to a specific region offers a significant advantage against various pathogen strains. We have isolated a cocktail of bacteriophages against pathogenic E. coli from sewage water at a primary healthcare centre. Characterisation of the isolated phages demonstrated their stability across a broad pH and temperature range, stro

Pseudomonas aeruginosa is a major opportunistic pathogen responsible for severe human infections and is increasingly associated with multidrug resistance and limited treatment options. In this study, we isolated and characterized bacteriophages targeting drug-resistant P. aeruginosa strains and evaluated their capacity to disrupt biofilms and modulate biofilm-associated gene expression. Phages recovered from sewage showed broad lytic activity against P. aeruginosa and were classified morphologically into the families Podoviridae and Myoviridae. Two phages, A2 and A4, exhibited the highest lyti

UNLABELLED: Multidrug-resistant (MDR) Pseudomonas aeruginosa poses a significant global health threat, necessitating alternative therapeutic strategies. Bacteriophage therapy shows promise, but single-phage use is prone to resistance. Bacteriophage cocktails can address this issue, but they are typically formulated empirically without a systematic evaluation of constituent phage interactions. We employed a comprehensive combinatorial approach to optimize three-phage cocktails against clinical P. aeruginosa isolates. From 25 candidate bacteriophages, 5 were selected based on a broad host range

The increasing prevalence of multidrug-resistant (MDR) Vibrio parahaemolyticus poses a growing threat to public health and aquaculture, underscoring the need for alternative therapeutic strategies. In this study, a clinically derived MDR Vibrio parahaemolyticus strain (Vp1018) was characterized; its genome encodes multiple antimicrobial resistance determinants and exhibits clear pathogenicity despite lacking the canonical virulence genes tdh and trh. Using Vp1018 as the host, a lytic phage, vB-Vp-ZX1018, was isolated and identified as a Siphoviridae (genus Mardecavirus) double-stranded DNA pha

Carbapenem resistance in Acinetobacter baumannii, driven largely by class D, along with class A and class B β-lactamases, has severely compromised the utility of these last resort antibiotics. As a result, infections caused by such pathogens are characterized by extremely high mortality rates. Here we describe the antimicrobial activity of the novel C5 methyl-substituted carbapenem JDB/PQ-1-219 against multidrug resistant A. baumannii and the mechanism of its interaction with its major carbapenemase, OXA-23. JDB/PQ-1-219 exhibits potent antimicrobial activity against A. baumannii producing var

Urological infections are among the most frequent bacterial diseases and result in a correspondingly high consumption of antibiotics with the associated known increased risk for the development of multidrug-resistant pathogens. For this reason, the measures summarized under the term antibiotic stewardship (ABS) are becoming increasingly more important for a rational use of antibiotics, which are explicitly underlined in the current European and German guidelines on urological infections. Despite these clear guideline recommendations the use of antibiotics in the urological practice is often su
