Neurohormonal consequences of diuretics in different cardiovascular syndromes.

Diuretics have long been used to lower blood pressure in hypertensive patients or to control body fluid and electrolyte homeostasis in diseases such as congestive heart failure, chronic renal failure or cirrhosis. The initial response to diuretics is a negative sodium and fluid balance. The diuretic-induced loss of salt and water activates several hormonal systems such as vasopressin, the renin-angiotensin-aldosterone system or the sympathetic nervous system which tend to compensate for the changes in sodium and water balance. This neurohormonal response may have important clinical implication

Pathogenesis of sodium and water retention in liver disease.

The "Peripheral Arterial Vasodilation" hypothesis most completely explains the clinical spectrum of cirrhosis ranging from compensated to decompensated to the hepatorenal syndrome (Figure 15-1). As the systemic peripheral vasodilation increases, the neurohumoral responses to arterial underfilling are stimulated with resultant renal vasoconstriction, sodium and water retention. Hypoalbuminemia and portal hypertension, as well as local effects of vasodilation at the capillary level, also contribute to ascites formation and peripheral edema. The suppressed plasma renin activity and aldosterone co

Electroanalytical characteristics of the cardiotonics, enoximone and piroximone.

Enoximone and piroximone are cardiotonic agents for use in patients with congestive heart failure. Electroanalytical studies revealed that the dihydroimidazolone functionality was oxidizable and that this property can be analytically useful. The compounds undergo a two-electron, irreversible oxidation in neutral to acidic media, leading to two major products. Results in basic media show up to three electrons transferred per molecule and much higher oxidation potentials at the fresh carbon paste electrode than at identical electrodes preconditioned in supporting electrolyte at +1.2 V. Use of am

Renal tubular acidosis in childhood.

Nineteen children with clinical diagnoses of renal tubular acidosis were followed for periods ranging from 3 months to 20 years. Twelve patients had Type 1 renal tubular acidosis, five had Type 2, and two had Type 4. No sex predilection was found for any one of the types. Most patients had been diagnosed before 18 months of age, with failure to thrive the most common presentation. Tachypnea, polydipsia, polyuria, and vomiting were frequent symptoms. Some of these children had associated renal hypoplasia, vesicoureteral reflux, unilateral renal agenesis, glomerulocystic disease, adult polycysti

Pseudohyperkalaemia in thrombocythaemia.

Pseudohyperkalaemia in thrombocythaemia, in its primary form or associated with polycythaemia vera, can occur with platelet counts in excess of 600 X 10(9)/l, and it is not related to high leukocyte or thrombocyte counts. An increment of 0.15 mmol/l serum potassium was found for every 100 X 10(9)/l rise in platelet count with a correlation coefficient of 0.82. Potassium from platelets is not released during the aggregation phase but during the degranulation phase of the coagulation process.

[Post-traumatic pulmonary insufficiency: therapeutic measures].

The pathophysiology of posttraumatic pulmonary failure is today reasonably well known. Interaction of altered granulocytes with the pulmonary parenchyma in the form of an inflammatory reaction plays an important role. Gas exchange failure can be a major problem in these patients and influence the prognosis. No effective preventive treatment is known at present except for rapid and appropriate surgical management together with an adequate fluid resuscitation. In addition, the risk of infection and sepsis must be reduced as far as possible and dysfunction of other organ systems prevented if the

Plasma renin activity and aldosterone: correlations with moderate hypohydration.

Adult male test subjects (n = 16) were assigned to one of three clothing configurations (Army fatigues, fatigues with impermeable chemical protective garments, and fatigues with protective garments plus protective masks) prior to exercise (level treadmill, 1.11 m/s, 50 min/h, 6 h) in a moderate (wet bulb globe temperature, WBGT = 23 degrees C) environment with ad lib water consumption. When protective masks were worn, two through-mask drinking systems were evaluated: the current gravity-fed system for fluid delivery and a new system utilizing a small hydraulic pump (Fist-Flex). Age-related loss of nongoblet crypt cells parallels decreased secretion in rabbit descending colon.

The effects of aging on active electrolyte transport and epithelial cell morphology were studied in rabbit descending colon. The rate of electrolyte transport was determined using the voltage-clamp technique, while computerized morphometric techniques were used to describe age-related and secretion-induced changes in histology. Comparison of percent water content of fecal pellets between young (2-3 mo) and mature (3.5-5 yr) animals demonstrated that mature animals had significantly decreased percent stool water. In mature animals, the total number of crypt epithelial cells was significantly le

Hyperventilation with hypoproteinemia.

Hypoproteinemia by itself produces a metabolic alkalosis. It is not clear whether a respiratory compensation (hypercapnia) develops with this alkalosis; patients with liver cirrhosis, most of them with hypoproteinemia, are known to hyperventilate. We studied 23 clinically stable patients with hypoproteinemia, with very low albumin-to-globulin ratios (range 0.4 to 1.1), who had either liver cirrhosis (n = 12) or other medical conditions (n = 11). In both groups, there was marked hypocapnia, accompanied by alkalemia (PaCO2 values (mean +/- SD) 31 +/- 2 and 32 +/- 3 torr; pH (mean +/- SD) 7.45 +/

Acute alcohol intoxication in a two-month-old baby.

A 2-month-old, well developed, healthy boy, weighing 5.55 kg, was fed 200 ml of bottle-milk containing 65 ml of sake. So-called kanzamashi (sake boiled in the evening and remaining in a bottle overnight,) was mistaken for yuzamashi (water boiled and left to cool), and used to prepare a 15% formula milk. About 10 minutes later, the baby became flushed, began to breath hard, and lose consciousness, and an alcoholic odor was noticed. He was brought to our clinic, where gastric lavage and parenteral fluid therapy were started. On admission, his main physical signs were, whole body had become red,

Oral rehydration salts and diarrhoeal diseases: effects of changing inpatient management in Tonga.

Methods of inpatient management of children with diarrhoeal diseases (DD) in 1978 and in 1980 were compared to assess the use of oral rehydration salts (ORS) and a national antidiarrhoeal programme (NAP). Inpatient notes of 369 children admitted with DD were reviewed. The management of DD differed significantly but the clinical outcome was similar, with the exception of the case-fatality rate. All deaths occurred within the first 24 h of admission, indicating that this difference was attributable to improved management of DD in the community rather than changes in the inpatient treatment regim

Lung lymph flow during volume infusions.

We investigated the effect of intravenous isotonic crystalloid solution infusion on lung lymph flow. Tracheobronchial lung lymph vessels were cannulated in 13 anesthetized dogs. The lymph flow rate was measured 1) with the lymph flowing against atmospheric pressure (QL), and 2) with the pressure at the outflow end of the lymph cannula equal to systemic venous pressure (QLV). QL and QLV were measured alternately in each lymph vessel. In one group of nine dogs, the base-line QL and QLV were 18 +/- 9 and 13 +/- 6 (SD) microliter/min, respectively (P less than 0.05). QL increased by 4.8 +/- 1.4-fo

Colonic dysfunction during shigellosis.

To study the pathogenesis of shigella diarrhea, we evaluated 11 patients for colonic fluid and electrolyte flux. Methods used were subtraction of the fecal flow rate from the ileocecal flow rate and colonoscopic perfusion of the colon. The mean ileocecal flow rate was 0.57 ml/min compared with a mean fecal flow rate of 0.48 ml/min, and the mean of the differences showed a slight net colonic water absorption of +0.09 ml/min. With use of perfusion, these patients showed an overall mean of slight net fluid secretion by the colon of -0.04 ml/min. Seven patients restudied during convalescence showe

Magnesium, potassium and hormonal regulation.

Clinical and experimental data demonstrate that hormonal factors are involved in magnesium regulation. It is possible that the parathyroid hormone could play an important role in the maintenance of normal calcium and magnesium concentrations. The action of other hormones in magnesium metabolism appears to be an indirect response to factors such as calcium concentration or changes in volume. Alterations of the magnesium concentrations are important for the intracellular potassium, sodium and calcium content. Magnesium deficiency appears to lower intracellular potassium and to increase intracell

Effects of jaundice and phototherapy on gastric emptying in the newborn.

Gastric emptying rate was measured in term infants without jaundice, with mild jaundice, and with moderate jaundice before and after phototherapy. Gastric emptying was significantly delayed in the moderately jaundiced infants (serum unconjugated bilirubin 233-332 mumol/l) but returned to normal following phototherapy. Mild jaundice (serum unconjugated bilirubin 170-204 mumol/l) had no effect on gastric emptying. Jaundice appears to inhibit gastric emptying, and phototherapy influences emptying indirectly by lowering serum bilirubin concentration. This effect is likely to be of clinical signifi
