High AIDS awareness may cause tuberculosis patient delay: results from an HIV epidemic area, Thailand.

OBJECTIVE: To elicit community perceptions about tuberculosis (TB) and the behaviour of TB patients in an area where significant public health attention has been focused on AIDS. SETTING: Chiang Rai, Thailand, the epicentre of HIV/AIDS in this country. DESIGN: Eleven focus group sessions with a cross-section of the population. PARTICIPANTS: Health centre staff, community members, persons having HIV, TB patients (with and without HIV infection), and male injecting drug users; five female groups and six male groups. RESULT: People had good knowledge of AIDS but they knew little about TB. Onl

[Epidemic outbreak of meningitis and meningoencephalitis, caused by West Nile virus, in the Krasnodar territory and Volgograd region (preliminary report)].

Sera from 102 inpatients from the Volgograd region (64) and Krasnodar region (38) were tested for antibodies to West Nile (WN) virus in hemagglutination inhibition (HI) test and for IgM and IgG antibodies in enzyme immunoassay (EIA). Diseases etiologically associated with WN virus were diagnosed in 81 patients: in 50 out of 64 in the Volgograd region and in 31 out of 38 in the Krasnodar region, which makes 79.4%. Specificity of antibodies to WN virus was confirmed in HI and EIA with WN antigens, related flaviviruses (Japanese encephalitis and yellow fever), and Sindbis alfavirus. A considerabl

Analysis of 42 cases of septicemia caused by an epidemic strain of methicillin-resistant Staphylococcus aureus: evidence of resistance to vancomycin.

Recent case reports of vancomycin treatment failures in the United States, Japan, and France have prompted a retrospective analysis of 42 cases of septicemia caused by epidemic methicillin-resistant Staphylococcus aureus strain 15 (EMRSA-15), which is the most prevalent epidemic strain of methicillin-resistant S. aureus in the United Kingdom; all cases occurred in a teaching hospital in Manchester, United Kingdom, between 1994 and 1998. Mortality was lowest (4%) in patients with rifampin-susceptible isolates treated with vancomycin and rifampin. It rose to 38% in patients who were treated with

Cost-effectiveness of oral cholera vaccine in a stable refugee population at risk for epidemic cholera and in a population with endemic cholera.

Recent large epidemics of cholera with high incidence and associated mortality among refugees have raised the question of whether oral cholera vaccines should be considered as an additional preventive measure in high-risk populations. The potential impact of oral cholera vaccines on populations prone to seasonal endemic cholera has also been questioned. This article reviews the potential cost-effectiveness of B-subunit, killed whole-cell (BS-WC) oral cholera vaccine in a stable refugee population and in a population with endemic cholera. In the population at risk for endemic cholera, mass vacc…

[Ciprofloxacin in the treatment of dysentery caused by type 1 Shigella dysenteriae during an epidemic in Rwandan refugees in Goma in 1994].

There was an outbreak of dysentery caused by type I Shigella dysenteriae among Rwandan refugees at Goma, in what was then Zaire, in 1994. The causal organism was resistant to all antibiotics available from the public health authorities. The only effective antimicrobial agent available was ciprofloxacin. It was given to 326 inpatients at the Médecins Sans Frontières (MSF) Center, mostly children under the age of 5, pregnant women and immunocompromised patients. A standard dose schedule was used: 1 g ciprofloxacin per day in two doses for 5 days for adults, 250 mg for children weighing less than

AIDS epidemic in San Francisco among men who report sex with men: successes and challenges of HIV prevention.

San Francisco's gay male community has been hit harder by the HIV/AIDS epidemic than any other community in the world. By 1995, 20,530 of the estimated 58,000 gay men had been diagnosed with AIDS. However, the epidemic has also been more effectively contained within San Francisco than anywhere else, as evidenced by rapidly dropping AIDS incidence and mortality rates. This article reviews the epidemiology of HIV/AIDS among men who report sex with men (MSM) in San Francisco and the successful prevention efforts in this community. Also cited are areas in which the San Francisco prevention model h

An epidemic of sylvatic yellow fever in the southeast region of Maranhao State, Brazil, 1993-1994: epidemiologic and entomologic findings.

Yellow fever virus transmission was very active in Maranhao State in Brazil in 1993 and 1994. An investigation was carried out to evaluate the magnitude of the epidemic. In 1993, a total of 932 people was examined for yellow fever from Maranhao: 70 were positive serologically, histopathologically, and/or by virus isolation, and another four cases were diagnosed clinically and epidemiologically. In Mirador (17,565 inhabitants), the incidence was 3.5 per 1,000 people (case fatality rate [number of deaths/number of cases diagnosed] = 16.4%), while in a rural yellow fever risk area (14,659 inhabit

A community trial of the impact of improved sexually transmitted disease treatment on the HIV epidemic in rural Tanzania: 1. Design.

OBJECTIVE: To describe the rationale and design of a randomized trial of the impact of improved services for the treatment of sexually transmitted diseases (STD) on the incidence of HIV infection in Mwanza Region, Tanzania. METHODS: The likely impact of improved STD treatment services on HIV incidence, and the need for empirical information on the effectiveness of this intervention strategy, are discussed. The rationale and design of such an intervention programme in Mwanza Region, and of a community-randomized trial to measure the impact of the programme on HIV and other STD, are presented.

Epidemic dysentery caused by Shigella dysenteriae type 1: a sentinel site surveillance of antimicrobial resistance patterns in Burundi.

Annual epidemics of bacillary dysentery have been a public health problem in Burundi for the last 14 years. Recent civil unrest, resulting in the displacement of large numbers of people into refugee settlements, has aggravated the situation. We report the results of a nationwide, health-centre based, sentinel site survey to check the drug resistance of Shigella dysenteriae type 1 (Sd1), the causal organism of such epidemics. Shigella spp. (of which 97% were Sd1) were isolated from 73% of the 126 specimens collected from six main sites around the country. There was no difference in culture resu…

[Antibiotic sensitivity to epidemic strains of Vibrio cholerae and Shigella dysenteriae 1 isolated in Rwandan refugee camps in Zaire].

Multiresistance or epidemic enteric bacteria to antibiotics greatly complicates treatment, and in some cases prophylaxis, of severe invasive gastroenteritis. During the summer of 1994, two epidemics of diarrhea, one due to Vibrio cholerae and the other to Shigella dysenteriae 1 isolated from the Goma and Bukavu camps was determined by measurement of the Agar Minimal Inhibitory Concentration. Multiresistance to tetracyclins, aminopenicillins, trimethoprimsulfamethoxazole, and nifuroxazide was observed. After intensive treatment mutant forms of both bacteria resistant to nalidixic acid rapidly a…

Epidemic visceral leishmaniasis in Sudan: a randomized trial of aminosidine plus sodium stibogluconate versus sodium stibogluconate alone.

In a comparative trial of treatment in southern Sudan, visceral leishmaniasis was diagnosed by the following symptoms: fever for > 1 month, splenomegaly, and antileishmanial direct agglutination test (DAT) titer of > or = 1:25,600. Patients (200) were randomized to receive sodium stibogluconate (Sbv) at 20 mg/kg/day for 30 days (groups S, n = 99) or Sbv at 20 mg/kg/day plus aminosidine at 15 mg/kg/day for 17 days (group AS, n = 101). Of 192 patients who had spleens or lymph nodes aspirated at entry, 134 (70%) were positive for parasites. During treatment, 7% in group S and 4% in group AS died.

Unusual association of a plasmid with nalidixic acid resistance in an epidemic strain of Shigella dysenteriae type 1 from Asia.

The association of a 20-MDa plasmid with nalidixic acid resistant (Nalr) strains of Shigella dysenteriae 1 has been examined. The plasmid, which is readily transferable, does not itself code for nalidixic acid resistance but offers a survival advantage to its host under nalidixic acid stress. The plasmid-containing cultures of S. dynsenteriae 1 produced Nalr mutants in vitro at a frequency 1000-fold higher than their plasmidless parent strains, after two exposures to nalidixic acid. Using a similar procedure, mutants resistant to other antibiotics such as chloramphenicol, tetracycline, or cipr

[Analysis of the possible mechanisms of the action of socioeconomic factors on the epidemic process whose biological base is an open parasitic system].

The mechanism of action of socioeconomic factors on the epidemic process have been analyzed on the basis of the study of the influence, exerted on the biological properties of infective agents by environmental factors resulting from human activities, and the theory of the self-regulation of the parasitic system. Such mechanisms can be subdivided into the mechanisms affecting the epidemiological triad (the source of infection, the mechanism of transfer, and the susceptibility of the population) and the mechanisms facilitating the formation of infective agents with selective advantages (decrease
