Neurophysiologic intraoperative monitoring in pediatrics.

Neurophysiologic intraoperative monitoring, using somatosensory, brainstem auditory, and visual evoked potentials, transcranial electric motor stimulation, and electromyography, is typically used during complex surgeries involving the motor and sensory cortex, brainstem, cranial nerves, spinal cord, nerve root, peripheral roots, brachial plexus, lumbar plexus, and peripheral nerves. The particular type of surgery and the neurologic structures at risk determine the type of monitoring chosen. Although many methods are the same in adult and pediatric patients, some differences in the pediatric po…

The feedback-related negativity (FRN) in adolescents.

This study examined age-related differences in the ERP correlates of external feedback processing (i.e., the feedback-related negativity [FRN]) in adolescent and young adult males, using a simple gambling task involving unpredictable monetary losses and gains of low and high magnitude. The FRN was larger after losses than gains, and was modulated by the magnitude of gains, but not the magnitude of losses, for all participants regardless of age. FRN amplitude was larger in adolescents than adults and also discriminated relatively less strongly between gains and losses in adolescents. In additio

[Characteristics of auditory brain stem response in neonatal hyperbilirubinemia induced by different causes].

OBJECTIVE: To explore the characteristics of auditory brainstem response (ABR) in neonatal hyperbilirubinemia induced by different causes. METHODS: A total of 88 neonates (176 ears) with hyperbilirubinemia were divided into several groups according to the causes and followed up after 42 d, and 15 normal neonatus (30 ears) were measured ABR and analyzed the results. RESULTS: The thresholds of ABR in glucose-6-phosphate dehydrogenase were the highest in all the groups and had the longest incidence rate. The wave III, V latencies and III-V, I-V interwave intervals of the ABR were significantly

Physiologic effects on the vestibular system in Meniere's disease.

Ménière syndrome is an inner ear disorder characterized by spontaneous attacks of vertigo, fluctuating low-frequency sensorineural hearing loss, aural fullness and tinnitus. When the syndrome is idiopathic and cannot be attributed to any other cause (eg, syphilis, immune-mediated inner ear disease, surgical trauma), it is referred to as Ménière disease. This article reviews the physiologic effects of Ménière disease on vestibular function, as measured by caloric, head impulse, and vestibular-evoked myogenic potential testing.

Transcranial magnetic stimulation in Gilles de la Tourette syndrome.

The cause of Gilles de la Tourette syndrome (GTS), a chronic motor and vocal tic disorder of childhood onset, remains unknown. Abnormalities … otor thresholds are similar in GTS and healthy subjects; in the resting state, recruitment of motor evoked potentials (MEPs) above threshold is more gradual in patients than controls. In contrast, recruitment of MEPs during preactivation is similar in both groups, as is the duration of the cortical silent period. This suggests that the distribution of excitability …

ERP correlates of complex human decision making in a gambling paradigm: detection and resolution of conflict.

The present event-related potential study investigated the correlates of decision making in relation to the amount of response conflict. In a gambling paradigm, response conflict was introduced by giving participants the option to either gamble or pass. Second, the odds and gains in each trial were manipulated to make the decision to gamble or pass determined or underdetermined. Underdetermined trials included an extra conflict. The N2 was modulated by the mere presence of conflict. In contrast to both conflict monitoring and inhibition theories for N2, these results suggest that an enhancemen

A VEP measure of the binocular fusion of horizontal and vertical disparities.

PURPOSE: Because of the lateral separation of the orbits, the retinal images differ in the two eyes. These differences are reconciled into a single image through sensory and motor fusional mechanisms. This study demonstrates electrophysiologically the effects that normal horizontal and vertical fusional processes have on the processing of monocular position signals. METHODS: VEPs were recorded in 16 healthy adults in response to a vernier onset-offset target presented to one eye. The vernier offsets appeared and disappeared at 2 Hz and were introduced into bar targets that were oriented eithe

Auditory cortical tuning to statistical regularities in phonology.

OBJECTIVE: Ample behavioral evidence suggests that distributional properties of the language environment influence the processing of speech. Yet, how these characteristics are reflected in neural processes remains largely unknown. The present ERP study investigates neurophysiological correlates of phonotactic probability: the distributional frequency of phoneme combinations. METHODS: We employed an ERP measure indicative of experience-dependent auditory memory traces, the mismatch negativity (MMN). We presented pairs of non-words that differed by the degree of phonotactic probability in a mod

Corollary discharge dysfunction in schizophrenia: can it explain auditory hallucinations?

Failure of corollary discharge, a mechanism for distinguishing self-generated from externally generated percepts, has been posited to underlie certain positive symptoms of schizophrenia, including auditory hallucinations. Although originally described in the visual system, corollary discharge may exist in the auditory system, whereby signals from motor speech commands prepare auditory cortex for self-generated speech. While associated with sensorimotor systems, it might also apply to inner speech or thought, regarded as our most complex motor act. In this paper, we describe the results of a se

[Intraoperative monitoring of nerve roots in spinal surgery].

INTRODUCTION: The use of intraoperative monitoring to determine the functional integrity of the nervous system has grown steadily over the last 15 years. The development of new instrumentation systems in spinal surgery and a greater incidence of complications have led to the need to introduce new methods of monitoring. AIMS AND DEVELOPMENT: The purpose of this study is to analyse the neurophysiological techniques that are currently available and applied in order to prevent injury to nerve roots and the fibres of the cauda equina, to assess their indications and to determine their limitations,

Auditory brainstem response detects early bilirubin neurotoxicity at low indirect bilirubin values.

When sedation, prematurity or other disease processes mask symptoms in the clinically ill newborn, serum bilirubin concentration is monitored as the sole indicator of kernicterus risk. This case emphasizes the value of auditory brainstem responses for the management of indirect hyperbilirubinemia complicated by prematurity, hemolytic anemia, asphyxia, and direct hyperbilirubinemia.

Neuroanatomy and neurophysiology in schizophrenia.

Schizophrenia is a major mental disorder, characterized by their set of symptoms, including hallucinatory-delusional symptoms, thought disorder, emotional flattening, and social withdrawal. Since 1980s, advances in neuroimaging and neurophysiological techniques have provided tremendous merits for investigations into schizophrenia as a brain disorder. In this article, we first overviewed neuroanatomical studies using structural magnetic resonance imaging (s-MRI), MR spectroscopy (MRS), and postmortem brains, followed by neurophysiological studies using event-related potentials (ERPs) and magnet

Perceptual awareness and its loss in unilateral neglect and extinction.

We review recent evidence from studies of patients with unilateral neglect and/or extinction, who suffer from a loss of awareness for stimuli towards the affected side of space. We contrast their deficit with the effects of damage to primary sensory areas, noting that such areas can remain structurally intact in neglect, with lesions typically centred on the right inferior parietal lobe. In keeping with preservation of initial sensory pathways, many recent studies have shown that considerable residual processing can still take place for neglected or extinguished stimuli, yet without reaching t

State of the art neonatal hearing screening with auditory brainstem response.

The purpose of this paper is to briefly review some of the aspects of the auditory brainstem response (ABR) that are important in its use as a method of screening and assessing hearing in the neonate. The paper starts by considering the technical limitations of click ABR and explores the alternative electrophysiological methods. It then considers where ABR is required in the screening of neonates. The role of bone conduction ABR in estimating the conductive component of any hearing loss is discussed. Finally, the ability of the neonatal ABR to predict the long-term audiometric outcome of perma

Analysis of the dynamics of interneuronal functional connections during conditioned reflex activity.

The dynamics of interneuronal functional connections were studied in the prefrontal cortex of dogs performing a task consisting of unforeseen remodeling of movement conditioned reflexes. An original method was used to find and classify the temporal patterns of linked spikes coming from several simultaneously recorded neurons. This procedure showed that in 33 pairs of neurons (87% of the total number of pairs showing interneuronal functional connections), parts of the conditioned reflex program were associated with behaviorally significant changes in the functional relationship between the neur
