Advances in cell-penetrating peptide-based nose-to-brain drug delivery systems.

The incidence of brain disorders has gained worldwide attention and the presence of the blood-brain barrier prevents numerous drugs from reaching the targeted brain. The specific physiology of the nasal cavity and the brain provides the feasibility of direct nose-brain delivery, a system that bypasses the blood-brain barrier in a non-invasive manner for brain-targeted drug delivery via intracellular and extracellular mechanisms. The use of CPPs provides further feasibility for naso-brain drug delivery studies, and liposomes, nanopolymer particles, and gels modified with CPPs have demonstrated

Feasibility and acceptability of a virtual mindfulness intervention for Black adults with PTSD and depression: Randomized controlled trial.

Mindfulness-based cognitive therapy (MBCT) offers promise as a group-based intervention to alleviate posttraumatic stress disorder (PTSD) and depression symptoms in traumatized Black adults. Given the high level of barriers that exist for low-income Black adults, virtual delivery of MBCT may be helpful. This pilot randomized controlled trial assessed feasibility and acceptability of an adapted 8-week virtual MBCT group intervention for Black adults screening positive for PTSD and depression. Forty-six participants (89.3% women) recruited from an urban safety net hospital were randomized to MBC

Transcranial focused ultrasound for the treatment of tremor: A preliminary case series.

BACKGROUND: Essential tremor (ET) can be debilitating. Treatments for ET include beta-blockers and surgical interventions. Low-intensity focused ultrasound (LIFU) may offer an office-based non-invasive alternative. OBJECTIVE: This pilot open label clinical trial explores safety, feasibility, and potential efficacy of LIFU in treatment of ET. METHODS: We report outcomes from the first 10 participants in this IRB-approved trial of LIFU for treatment of ET. The ventral intermediate nucleus of the thalamus (Vim) was targeted using structural and functional MRI. Participants underwent eight 10-mi

Reporting Individual-Level Research Results from Neurocognitive and Psychological Research in People Living with HIV: Lessons from Dementia Research.

PURPOSE OF REVIEW: There are growing expectations for the return of individual-level research results (RoR), which promotes autonomy and potential clinical and personal benefits. There are ethical and practical challenges, however, that may be exacerbated in research that assesses neurocognitive and psychological outcomes, including HIV-associated neurocognitive disorder (HAND). This paper reviews central concepts for RoR and recent empirical and conceptual articles from Alzheimer's disorder (AD) as a model for HIV. RECENT FINDINGS: Data from AD studies indicate high participant interest and

[Initial experiences with sentinel lymph node biopsy in malignant melanoma].

Elective versus therapeutic lymph node dissection has been a controversial field of the surgical treatment of cutaneous malignant melanoma for more than two decades. The identification and biopsy of the sentinel lymph node in different solid malignancies has become feasible by the method described by Morton in 1992. The sentinel lymph node is the first tumor draining lymph node in the regional lymph node basin. If metastasis is not proven in the sentinel node by detailed histological study those are unlikely in other regional lymph nodes and formal lymph node dissection can be omitted. Patient

Human Papilloma Virus self-sampling performance in low- and middle-income countries.

BACKGROUND: Screening for HPV has led to significant reductions in cervical cancer deaths in high-income countries. However, the same results have not been achieved in low- and middle-income countries (LMICs). HPV self-sampling is a novel approach that could improve screening rates. METHODS: This study's objective is to summarize the recent literature on HPV self-sampling in LMICs, focusing on sensitivity/specificity, and feasibility/acceptability of self-sampling compared to traditional screening methods. We conducted a PubMed search for articles published in English within the last 10 years

Freeze-dried plasma for major trauma - Systematic review and meta-analysis.

BACKGROUND: Treatment of acute trauma coagulopathy has shifted toward rapid replacement of coagulation factors with frozen plasma (FP). There are logistic difficulties in providing FP. Freeze-dried plasma (FDP) may have logistical advantages including easier storage and rapid preparation time. This review assesses the feasibility, efficacy, and safety of FDP in trauma. STUDY DESIGN AND METHODS: Studies were searched from Medline, Embase, Cochrane Controlled Trials Register, ClinicalTrials.gov, and Google Scholar. Observational and randomized controlled trials (RCTs) assessing FDP use in traum

Gamified sensor-based upper extremity functional rehabilitation in individuals with spinal cord injury: a pilot clinical trial protocol.

BACKGROUND: Spinal cord injury (SCI) is a life-altering condition that impacts physical, functional, and psychosocial well-being. Regardless of the cause, restoring hand function is the first priority for tetraplegic people which tend to place more emphasis on independence and activity limitations in activities of daily living (ADL) in SCI. Traditional rehabilitation strategies often result in limited motor recovery, highlighting the need for adjunctive approaches. Emerging evidence suggests that gamified sensor-based interventions may augment conventional therapy by increasing patient engagem

TaIlored ManagEment of Sleep (TIMES) for people with dementia and mild cognitive impairment in primary care in England: protocol for a feasibility cluster-randomised controlled trial.

BACKGROUND: People living with dementia (PLWD) and mild cognitive impairment (MCI), and their family carers, often experience sleep disturbance which can impair daily living and care. There are limited options for effective long-term pharmacological management of sleep disturbance, yet recent advances in non-pharmacological approaches offer promising alternatives. TIMES is a novel, complex intervention, which aims to improve wellbeing for PLWD/MCI and their carers in primary care, by developing whole-person, tailored care plans that optimise management of sleep disturbance in context. METHODS

A brief digital screening tool to address parental and adolescent tobacco and electronic cigarette use in paediatric medical care in Canada: a pilot randomized controlled trial.

BACKGROUND: Tobacco use is a strong risk factor for premature mortality and morbidity. We assessed the feasibility of screening and recruiting participants for a randomized controlled trial (RCT) of a facilitated referral intervention to promote smoking and vaping cessation among parents and children (CanCEASE). METHODS: We conducted a pilot RCT of CanCEASE in five outpatient pediatric clinics in Montreal, Canada. We recruited parents (≥ 18 years) and adolescents (14-17 years) who reported smoking or vaping in the 7 days prior to recruitment. Our primary outcome was the number of participants

Pilot study protocol to increase resistance to peer pressure in tobacco use among nursing students (T-RESIST).

BACKGROUND: In Mexico, tobacco use represents one of the leading threats to public health, being associated with thousands of preventable deaths and diseases. Social factors, such as peer pressure, play a significant role in the initiation of tobacco use among young people, who seek acceptance and identity. Although intervention studies exist, few address peer pressure comprehensively and from a theoretical framework. Therefore, a psychoeducational intervention is proposed for university students, with the aim of assessing its acceptability and feasibility, as well as exploring changes in self

Feasibility of the Promoting Pupils' Physical Literacy (3PL) intervention: Does it work? Will it work?

BACKGROUND: The rise in physical inactivity among children poses serious health risks impacting physical, mental, and social well-being. Physical literacy (PL) has been identified as an effective approach to encourage long-term physical engagement. Schools and communities, particularly through physical education (PE) programs, play a critical role in promoting PL. However, while there are several PL-focused PE interventions worldwide, no such programs have been specifically tailored to Nordic contexts, making feasibility testing essential for effective implementation. This study investigated t

The feasibility and acceptability of conducting a randomised controlled trial evaluating a healthcare professional-supported, self-management intervention for people suffering from fatigue after critical illness.

BACKGROUND: Fatigue is a common problem that significantly affects intensive care unit (ICU) survivors' physical, psychological, and social functioning. ICU survivors often experience a loss of self-worth and identity, struggle to return to their normal roles, and face ongoing challenges with cognitive and emotional recovery. Despite its profound impact, there are limited rehabilitation interventions targeting this population. This research aims to evaluate the acceptability of implementing the Fatigue After CriTical illness (FACT) self-management intervention into usual care for patients expe

Optimization of care and recovery in older colorectal cancer surgery patients: a hybrid effectiveness-implementation pilot study protocol.

BACKGROUND: The exponential rise in colorectal cancer surgeries among older adults combined with 30-day readmission rates as high as 25% highlights the crucial need to optimize post-discharge care. While geriatrics surgery programs such as the Optimization of Senior Care and Recovery (OSCAR) model improve outcomes, they do not address the post-discharge transition. This study will address this gap by systematically integrating the OSCAR program with an evidence-based Care Transitions Intervention (CTI), using implementation science frameworks to guide adaptation. The resulting integrated model

Pilot study of the DeStress for Health Program: a community-engaged stress reduction and cancer prevention intervention for rural communities.

BACKGROUND: Stress related to personal, social, and environmental factors has been linked to engaging in unhealthy behaviors. These behaviors may increase cancer risk. Most current prevention programs do not address stress related to social factors and related stresses that are driving unhealthy behaviors. The purpose of this study was to pilot the DeStress for Heath Program. METHODS: We conducted a pilot randomized controlled trial. Community members from two rural counties were randomized to the DeStress for Health Program group or a waitlist control group. The DeStress for Health Program,

A pragmatic adaptive trial of hope-focused mentoring to improve mental health and social outcomes for young women who are not in education, employment or training in deprived coastal areas (The Looking Forward Project): feasibility trial stage protocol.

BACKGROUND: A large number of young women are not in education, employment, or training (NEET) in England. This group experiences poorer mental health and social outcomes than young women who work and study, and NEET young men. Gender disparities are compounded in deprived and coastal areas, where young women especially lack self-agency and aspirations. Research suggests that greater hope lowers risks of staying NEET and developing mental health problems. We co-developed a hope-focused intervention (HOPEFUL) for this group. HOPEFUL is a flexible modular programme, designed to help develop a mo
