Middle-third clavicle fracture with associated type IV acromioclavicular separation: case report and literature review.

Review of available English literature suggests that combined injuries involving a clavicle shaft fracture and an acromioclavicular (AC) separation are rare. The force dissipation after the occurrence of either a midshaft clavicle fracture or an AC separation typically renders the injuries mutually exclusive. This article presents a review of literature on this combined injury pattern. A variety of treatment approaches have been put forth, including nonoperative, operative, and hybrid management of the two distinct injuries. The most appropriate treatment rendered depends on the degree of AC j

[Is routine implant removal after trauma surgery sensible?].

We present a literature review about implant removal after operative extremity and spine fracture treatment. The indication for implant removal procedures has become less frequent in recent years, but is still more common in Europe than for example in North America. The time required to perform a implant removal can easily exceed the planned amount. Implant removal can result in significant complications like soft tissue damage, fractures, infections, and other problems. Not only because of these problems, the decision on whether or not to remove the implant should be made with great care. The

[A boy with a painful upper leg].

A 13-year-old boy came to the Emergency Department with a Salter Harris type 1 epiphysiolysis of the distal femur. These fractures can be treated with a closed reduction and possible fixation. They cause a growth disturbance in 65 % of dislocated cases and therefore should be clinically evaluated until skeletal maturity.

Radial nerve injury during double plating of a displaced intercondylar fracture.

Double-plate fixation is a popular treatment method for intercondylar humeral fractures. Ulnar nerve complications are emphasized, but radial nerve complications are rarely mentioned. We present a case of iatrogenic radial nerve palsy following open reduction and double plating of a supracondylar/intercondylar fracture of the humerus. Before surgery, only a sensory deficit in the radial nerve territory was present, but after surgery, there was a complete motor deficit of the wrist and finger extensors. On exploration, a segment of nerve was found crushed within the reduced lateral condyle frac

[Medial transposition of the radial nerve in humeral shaft fractures fixation].

OBJECTIVE: To summarize clinical outcomes of using medial transposition of the radial nerve in humeral shaft fracture fixation. METHODS: From January 2005 to December 2009, 16 patients with humeral shaft fractures were treated with medial transposition of the radial nerve during open reduction and anterolateral plate fixation, included 12 males and 4 females ranging in age from 26 to 49 years, with a mean of 36 years. There were 7 fractures in the right and 9 in the left. According to AO classification, 6 fractures were type A3.2, 5 fractures were type A2.2, 2 fractures were type A1.2 and 3 f

Modified "2-portal" kocher-langenbeck approach: a minimally-invasive procedure protecting the short external rotator muscles.

Acetabular fractures rank among the most challenging reconstruction procedures in trauma surgery. Thereby, substantial muscle dissection can result in functional limitations. In this study, a muscle-protecting modification of the standard Kocher-Langenbeck approach is described. A prospective clinical analysis was performed on 18 patients who underwent surgery for acetabular fractures. Nine patients were treated by the presented modified approach and another 9 patients by a standard Kocher-Langenbeck approach. The modification, which is described in this study, protects the small rotator muscl

Transverse incision for calcaneal tuberosity avulsion fractures.

A number of incision options are available to surgeons approaching the posterior aspect of the calcaneus for repair of fractures of the posterosuperior aspect of the body of the calcaneus. In this brief communication, we depict our preference for the use of a transverse posterior calcaneal incision for reduction and fixation of avulsion fractures of the calcaneus. The advantages of this particular incisional approach include adequate exposure to the underlying target structures, orientation of the scar in line with relaxed skin tension lines, which minimizes scar formation, and avoidance of di

Minimally invasive reduction technique in split depression type tibial pilon fractures.

Tibial pilon fractures usually result from high energy trauma and present as a challenge to the orthopedic surgeon. Accurate reduction of the joint with meticulous care for the surrounding soft tissues is mandatory. We present a case report in which an anterior cruciate ligament targeting device is used with a minimally invasive technique under arthroscopic and fluoroscopic guidance for Orthopaedic Trauma Association 43-B2.3 type pilon fracture treatment.

[A 80-year-old woman with delirium after intertrochanteric fracture of femur].

The aim of this study is to present a case of delirium -the state occurring frequently in elderly patients. A 80-year-old woman was hospitalized in the department of internal diseases for internal medical examinations before scheduled operation of femoral osteosynthesis. In ward, she was given analgesic medicines from the group of non-steroidal anti-inflammatory drugs (NSAID), paracetamole and opioids influencing central nervous system. On the second day after modifying the painkilling treatment (single doses of paracetamole were increased and oral opioid medication was introduced) a quickly p

Use of articular wrist views to assess intra-articular screw penetration in surgical fixation of distal radius fractures.

The use of volar locking plates for fixation of distal radius fractures is increasing in modern orthopedic practice. They have the advantage of limited dissection, early mobilization, and improved function over previous fixation devices. Standard anteroposterior and lateral radiographs would normally be requested by the orthopedic team, but these may not give an accurate representation of the articular anatomy of the fracture fixation. This study evaluated the effectiveness of a series of radiographs, the articular wrist series, which accurately represent the wrist joint and clarify doubt rega

[Reconstruction of fractures of the anterior wall and the anterior column of the acetabulum using an ilioinguinal approach].

OBJECTIVE: Restoration of the congruence of the hip joint. Correction of gaps or steps in the articular surface, especially in the main weight-bearing area of the acetabular dome. Correction of femoral head subluxation. Restoration of joint stability in order to enable early postoperative mobilization. INDICATIONS: Fractures of the anterior wall and/or column that are characterized by intraarticular gaps or steps of > 1 mm in the area of the main weight-bearing dome of the acetabulum. Fractures complicated by subluxation or dislocation of the femoral head. CONTRAINDICATIONS: Poor general phy

Distal tibial epiphyseal fracture (Tillaux) and capsular interposition.

Arthroscopic-assisted reduction is one recommended technique for pediatric distal tibial epiphyseal fractures. A 15-year-old male patient with a triplane distal tibial epiphyseal fracture (Tillaux type) was scheduled for arthroscopic reduction and pinning, which failed to provide adequate fracture reduction. Open surgery was subsequently performed, which revealed a capsular interposition in the fracture line attributable to an anterior capsular tear of the ankle. The possibility of capsular interposition should always be taken into consideration when problems such as those described in this ca

Management of fractures through the angle of the mandible.

Fracture through the angle of the mandible is one of the most common maxillofacial injuries sustained in modern societies. Among issues related to the treatment of maxillofacial injuries, those concerning angle fractures are the most hotly debated, with the exception perhaps of those concerning the condylar process of the mandible. There are several reasons for this controversy about treatment of angle fractures, a controversy too often made up of arguments founded on emotion rather than on scientific information. This article discusses some of the controversies in the management of such fract
