A case is presented of a 16-year-old girl with ectodermal dysplasia for whom dental surgery under general anaesthesia was planned. Following a priming dose of vecuronium, and immediately after injection of sodium thiopentone (5 mg.kg-1) pulmonary aspiration of gastric contents occurred. It is hypothesized that, because of the rapid speed of onset of neuromuscular blocking agents on the laryngeal muscles, that partial laryngeal paralysis was present at the time of induction of anaesthesia and that this was responsible in part for the episode of pulmonary aspiration.

In the past 2 years the major genes for inherited breast-ovarian cancer, bowel cancer and melanoma have been identified. Relatives of cancer patients frequently perceive themselves to be at high risk of cancer. Appropriate assessment of the family history permits the identification of those at particularly high risk who may benefit from special programs of prevention, screening and surveillance. In certain cases, genetic testing and prophylactic surgery may be indicated. This article looks at the general issues involved. A follow-up article next month will look at specific diseases.

We evaluated whether pituitary-adrenal response to surgical stress is modified in chronic schizophrenic patients. While routinely used in pediatric general surgery, the extraperitoneal paramedian approach to the spine and retroperitoneum has been employed infrequently in the adult population. We have utilized this technique successfully in 15 adult patients with pathologic conditions of the spine requiring radical vertebral body resection, dura decompression, and/or spine reconstruction. A total of 10,299 new cases of eye diseases were seen in the past three years at Jimma Eye Unit. Conjunctivitis, trachoma, cataract, and refractive errors, were the leading causes of ocular morbidity accounting for 35.5%, 16.8%, 10.6% and 9.7% of the cases respectively. 28.3% of the trachoma cases had trichiasis and/or entropion. Increased activity in primary eye care in general and, provision of entropion surgery, availability of low-cost eye glasses and the expansion and improvement of the eye unit are recommended to effect a better ophthalmic service in the region.

In 34 patients undergoing major surgery, the inspired isoflurane concentration was adjusted by a control system designed to maintain systolic arterial pressure at a predetermined value. A nasal foreign body was discovered radiologically on an initial dental examination of a young child. The child was a candidate for dental rehabilitation, using nasotracheal intubation under general anesthesia. The parents were unaware of the object's presence, and the child had no nasal symptoms. Reported in this paper is the surgical applicability, as established from 35 operations, of the Type TLS 61 laser scalpel of Tungsram Rt, 60 Watt in output. Useful experience has been recorded from surgical approaches to thorax cavity as well as from general, gastro-enterological, vascular, and dermatological surgery. Minimised bleeding and sterile incisions are benefits of laser operations. The above model has proved to be applicable to skin, muscles, lung, stomach, intestines, bones, and other tissues. Wound healing was absolutely okay. After-bleeding or other complications did not occur.

Xanthogranulomatous pyelonephritis is a specific form of a chronically destructive inflammation of the kidney. In addition to our own case of the extremely rare coincidence of xanthogranulomatous pyelonephritis and a transitional cell carcinoma of the renal pelvis our report also refers to seven further cases of xanthogranulomatous pyelonephritis. A patient with the Eisenmenger syndrome presented for removal of a carotid body tumour under general anaesthesia. The presentation and management of the case are described and some of the anaesthetic problems associated with these two uncommon conditions are discussed. The case is reported because of its rarity and because some of the complications of carotid body surgery may be particularly hazardous in the presence of the Eisenmenger syndrome. The importance of monitoring, of maintaining a stable heart rate and blood volume, and the danger of paradoxical systemic air embolism are stressed.

This is a case of a 22-year-old female patient with idiopathic thrombosis of the hepatic veins in whom side-to-side portacaval shunt was followed by the disappearance of a hitherto intractable ascites and a marked improvement in her general condition. The patient remained well for 1 1/2 years after surgery; however, liver cirrhosis, which was present before shunt, persisted. From this case and from the few published cases, the authors concluded that side-to-side portacaval shunt might be beneficial to patients with thrombosis of the hepatic veins.

A double-blind investigation has been carried out using three different doses (20, 35 and 50 mg) of cocaine which were sprayed onto the nasal mucous membranes of forty-five patients for nasal surgery. The cocaine was administered after induction of general anaesthesia in all cases. The 20 mg dose of cocaine dissolved in 2 ml of saline without adrenaline was found to be safe and produced adequate nasal decongestion in the absence of other predominant factors. The efficacy of cocaine and its safety with regard to systemic absorption are assessed.
