Self-neglect: Implications for prosecutors.

Historically, the view of elder mistreatment in the United States has been similar to that of child abuse and has been detrimental to the understanding of elder mistreatment. Until recently, there was little understanding of the complexity of elder mistreatment and the legal system lacked appropriate laws with which to handle this growing problem. Over the last two decades, legislation, laws, and criminal codes of conduct have been developed to tackle the problem of elder mistreatment. However, little knowledge or understanding of the public health problems and legal implications related to th

How is the diagnosis of asthma in older people established in clinical practice?

WHAT WE NEED TO KNOW: How is breathlessness perceived and defined in older people? What impact does breathlessness have on quality of life? Can history taking and physical examination be tailored to efficiently cover all the organ systems associated with breathlessness? Can a self- or carer-rated questionnaire be used to identify asthma in patients with breathlessness? WHAT WE NEED TO DO: Develop self- and carer-rated questionnaires that measure change in function and quality of life before and after treatment. Validate objective measures of physical function and airflow that are sufficiently

[The dependence medical index (DMI): validation and comparison with the activity daily living and the instrumental activity daily living].

AIM: The aim of this study was the validation of the dependence medical index (DMI), a disability medical assessment tool in the elderly. Skin diseases of the lower extremities in the elderly.

Routine care of the legs and feet in the elderly is important in helping to prevent infections, malignancies, and further deterioration. The lower extremities are of particular interest because of the increasing amount of diseases and disorders involving the legs and feet. Foot disorders in the elderly are associated with poor choices in footwear, structural changes, brought on by aging, and inadequate knowledge about prevention and treatment. Conservative treatments along with gait modification provide positive long-term results, although sometimes surgery is necessary for severe foot disorde

Community-acquired pneumonia in older people: the need for a broader perspective.

Community-acquired pneumonia (CAP) poses a substantial threat to the health of older adults. The incidence of this infection and mortality associated with it increase with age. Despite the considerable effect of CAP on older adults, little is known about the effect of socioeconomic and environmental factors on CAP in older people. This paper argues that broader determinants, including socioeconomic status (SES), nutrition, and factors in the physical environment such as exposure to tobacco smoke and air pollution need to be evaluated as potential risk factors for CAP in older adults. Data sugg

Economic evaluation is essential in healthcare for the elderly. A viewpoint.

The aging population is considered to be one of the major factors driving the cost of healthcare upward in industrialised countries. However, several analyses show that expenditure has increased mainly because of other factors. Expenditure is expected to increase when an aging population is combined with technical progress. In addition, the growing proportion of the population who are elderly means that there is an increasing proportion of people who do not work, creating further problems in the financing of healthcare. These problems make it imperative to provide medical care to the elderly i

Relationship between odor perception and depression in the Japanese elderly.

Odor perception has been studied in patients with various mental disorders; however, no consensus has been reached as to its detection, identification, or pleasantness/unpleasantness of odors especially in patients with depression. One hundred and nineteen normal elderly individuals living at home were exposed to odors of rose, perfume, white ginger, Indian ink, cigarette smoke, milk, feces and orange scent using the scratch and sniff method. They were asked to rate the strength of each odor, its pleasantness or unpleasantness, their liking for it, and their familiarity with it. They were also

[Inappropriate psychopharmacological treatment of the elderly. A 1-year study].

The psychogeriatric department in the county of Arhus is in a unique position to collect cases of old people treated with psychotropics in various inappropriate ways in hospital wards or by general practitioners. In 1991 the department was in contact with 768 elderly people mostly on an outpatient basis among whom 40 patients exhibited accumulated errors of psychopharmacologic treatment. The treatment episodes were analysed. The patients were very old (mean 80.8 years) with severe somatic morbidity. A majority were demented or delirious. 75% were living in nursing homes. The treatment episodes

The myth of senility. Diagnosing nonspecific major illness in the elderly.

In the elderly, physical and mental concemitants of chronic and explainable ill health are often masked by the diagnosis of senility. The presentation of major illness in the elderly is commonly nonspecific, and a full physical examination and tests with special attention to infections and to the signs of endocrine and metabolic disorders should be carried out. Symptoms such as confusion and memory loss, hostile or difficult behavior, and dementia point to different probable causes, and the physician should carefully search for the underlying pathology.
