INTRODUCTION: Timely antiviral therapy in chronic hepatitis B (CHB) significantly reduces risk of cirrhosis, hepatocellular carcinoma, and mortality. Gaps in CHB have been reported, but reasons contributing to these persistently low treatment rates are unclear. We aim to evaluate gaps and disparities in the CHB cascade of care spanning from clinical and laboratory monitoring, assessment of treatment eligibility, and receipt of antiviral therapy.
METHODS: Adult Veterans with CHB receiving care within Veteran Affairs healthcare facilities were identified from 1/1/2010 to 6/30/2024 (with follow-…

Chronic hepatitis B (CHB) remains incurable due to the immune system's tolerance toward the hepatitis B virus (HBV) surface antigen (HBsAg). This study aimed to achieve a functional cure by breaking HBV tolerance through immunotherapy. CHB patients were treated with either standard nucleotide analog (NA) therapy (Adefovir Dipivoxil, ADV) (Cohort 1) or ADV combined with interferon-alpha (IFN-α) (Cohort 2). Additionally, a third cohort received the THRIL-GM-Vac regimen: three low-dose GM-CSF injections followed by one dose of the HBV vaccine, alongside standard treatment. THRIL-GM-Vac treatment …

Background and objective Chronic hepatitis B virus (HBV) and chronic hepatitis C virus (HCV) infections are significant global public health challenges, affecting millions of individuals worldwide and contributing substantially to morbidity and mortality due to chronic liver disease and its complications. In Pakistan, the burden of viral hepatitis remains considerable, with HBV and HCV infections continuing to affect vulnerable populations and placing an additional healthcare burden on patients with chronic diseases, including chronic kidney disease (CKD). This study aimed to determine the fre…

BACKGROUND: Over recent decades, treatment for human immunodeficiency virus (HIV)/hepatitis C virus (HCV) co-infection has significantly advanced. HIV is known to accelerate liver disease progression and increase liver-related mortality in patients with HCV infection.
AIM: To reassess the effectiveness of HCV treatments by comparing outcomes between HIV/HCV co-infected and HCV mono-infected patients.
METHODS: We retrospectively included patients with HCV mono-infection or HIV/HCV co-infection at 12 tertiary referral centers from January 2009 to December 2020. The primary endpoint was overall…

To close gaps in implementing care of people with hepatitis B virus (HBV) infection in resource-limited settings, we evaluated the accuracy of a novel liver point-of-care ultrasound (POCUS) protocol for hepatitis (PUSH) among front-line non-radiology healthcare workers in Zambia. At University Teaching Hospital in Lusaka, Zambia, from March to June 2024, we trained four nurses and six physicians with experience in HBV management, but not in ultrasound (US), in the PUSH protocol, which includes visualizing the liver in three windows (epigastric, subcostal and right transcostal) to identify cirr…
