Antigen-antibody reactions in allergic human tissues. II. Study by fluorescence technique of the localization of reagins in human skin and their relation to globulins.

The skin of atopic patients contains specific reaginic antibodies in all epidermal cells, in unchallenged as well as antigen-challenged areas. The reagins are also present in the epithelial cells of sebaceous glands, sweat glands, hair follicles and also in macrophages and pericytes. Judged by staining characteristics, their immunologic reaction in tissues with antigen is identical with that of rabbit antibodies. Comparison of tissues stained with conjugated reagins and conjugated anti-human globulin antibodies demonstrates the close relationship of reagins and globulins. In antigen-challenged

Mini-review: occupational health topics series on the effects of chemicals. Occupational cholangiocarcinoma incident.

In 2012, a social issue arose concerning a high incidence of cholangiocarcinoma (bile duct cancer) among printing workers. The cause was prolonged exposure to high concentrations of 1,2-dichloropropane that was included in the ink cleaning agent. Until then, it was not known that this substance could cause cancer in humans. The author was involved in this incident from the beginning. This article describes the course from the start of the investigation to the recognition of cholangiocarcinoma as an occupational disease. Furthermore, the article presents the pathological characteristics of occu

Establishing Air-Liquid Interface (ALI) Airway Culture Models for Infectious Disease Research.

Air-liquid interface (ALI) airway culture models serve as a powerful tool to emulate the characteristic features of the respiratory tract in vitro. These models are particularly valuable for studying emerging respiratory viral and bacterial infections. Here, we describe an optimized protocol to obtain the ALI airway culture models using normal human bronchial epithelial cells (NHBECs). The protocol outlined below enables the generation of differentiated mucociliary airway epithelial cultures by day 28 following exposure to air.

Neurogenic arthrogryposis and the power of phenotyping.

In this article we review the commonest cause of neurogenic arthrogryposis, termed Spinal Muscular Atrophy Lower Extremity Dominant (SMALED), due to variants in DYNC1H1 and BICD2. We discuss the characteristic clinical and radiological phenotype of this disorder and how this has facilitated the identification of the genetic cause of SMALED2. We also review the similarities and differences between the human SMALED phenotype and mouse models and how this has informed our understanding of the potential mechanisms governing motor neuron loss in these disorders.

Prosthetic Joint Infections Due to Histoplasma capsulatum: A Report of 3 Cases.

Histoplasma capsulatum causes pneumonia and multisystemic disease in humans. Musculoskeletal involvement in histoplasmosis is most often tenosynovitis and rarely septic arthritis. Even more uncommon is the involvement of prosthetic joints. Here, we report a series of 3 cases of prosthetic joint failures caused by infection due to H capsulatum. Together with a review of 4 previously reported cases, we summarize host characteristics, clinical presentation, surgical approaches, antifungal management, and outcomes of this rare orthopedic joint infection.

A Rare Variant of Penile Squamous Cell Carcinoma in a Man with Paraplegia.

Verrucous carcinoma (VC) is a rare variant of squamous cell carcinoma (SCC). It is described as a low grade, slow growing, locally infiltrative neoplasm that accounts for 3%-8% of penile SCCs. Here we report a case of destructive VC of the glans penis in a paraplegic man resulting in a hypospadias from the tip of the glans to the corona. Histology demonstrated exophytic squamous epithelial proliferation with characteristic round, pushing borders. In situ hybridization was positive for both low-risk and high-risk strains of human papillomavirus.

Frequency of decompression illness among recent and extinct mammals and "reptiles": a review.

The frequency of decompression illness was high among the extinct marine "reptiles" and very low among the marine mammals. Signs of decompression illness are still found among turtles but whales and seals are unaffected. In humans, the risk of decompression illness is five times increased in individuals with Patent Foramen Ovale; this condition allows blood shunting from the venous circuit to the systemic circuit. This right-left shunt is characteristic of the "reptile" heart, and it is suggested that this could contribute to the high frequency of decompression illness in the extinct reptiles.

Norovirus infection in immunocompromised hosts.

Acute gastroenteritis caused by noroviruses often has a duration of 2-3 days and is characteristically self-limiting. In contrast, chronic infection caused by noroviruses in immunocompromised individuals can last from weeks to years, making clinical management difficult. The mechanisms by which noroviruses establish persistent infection, and the role of immunocompromised hosts as a reservoir for noroviruses in the general human population, are not known. However, study of this patient cohort may lead to new insights into norovirus biology and approaches to treatment.

The cytolethal distending toxin of Haemophilus ducreyi inhibits endothelial cell proliferation.

Haemophilus ducreyi, the etiologic agent of the sexually transmitted disease chancroid, produces a cytolethal distending toxin (HdCDT) that inhibits mammalian cell proliferation. We investigated the effects of HdCDT on normal human endothelial cells and on tubule formation in an in vitro model of angiogenesis. Endothelial cells were arrested in the G2 phase of the cell cycle, and tubule formation was inhibited in a dose-dependent manner. The antiproliferative activities of HdCDT on endothelial cells might contribute to the characteristic slow healing and persistence of chancroid ulcers.

Alcohol-Related Thiamine Deficiency: Impact on Cognitive and Memory Functioning.

Chronic alcohol abuse is associated with several neurological disorders, including Wernicke-Korsakoff syndrome (WKS). Deficiency of thiamine-a vitamin essential for the metabolism and function of brain cells-is thought to be one factor contributing to the cognitive deficits and brain pathology characteristic of WKS. Excessive alcohol consumption may contribute to thiamine deficiency in several ways. Studies in human patients and animal models of WKS are attempting to identify brain structures affected by thiamine deficiency and to correlate them with the cognitive deficits observed in WKS pati

Anxiety and stigmatizing aspects of HIV infection.

While it is not yet known what role anxiety may play in causing or exacerbating the immunologic aberration characteristic of acquired immune deficiency syndrome (AIDS), human immunodeficiency virus (HIV) infection is associated with a high incidence of psychologic and neuropsychiatric complications, including anxiety and adjustment disorders. The anxiety may be exacerbated by the isolation and fear patients may experience as a result of the stigma attached to the HIV diagnosis. The author addresses the consequences of anxiety related to the AIDS stigma and discusses appropriate psychiatric int

Hodgkin's disease in a killer whale (Orcinus orca).

Hodgkin's disease (HD) is reported in a killer whale (Orcinus orca). Generalized lymphadenopathy and splenomegaly were noted at necropsy and histopathological examination revealed diffuse proliferation of atypical large cells with huge eosinophilic nucleoli. They were often multinucleated, and binucleated forms characteristic of Sternberg-Reed cells of human HD were occasionally present. There was no cytological atypia in the background lymphocytes. Thus, the lesion was identified as analogous to HD in man. To our knowledge, this is the first recorded case of HD in the killer whale.

Diseases with DNA damage-processing defects.

Xeroderma pigmentosum, Cockayne's syndrome, ataxia telangiectasia, Fanconi anemia, and Bloom's syndrome are autosomal recessive diseases with cellular defects in the ability to process DNA damage. Although these diseases are rare, they are seen occasionally in practice and provide insight into the mechanisms of DNA repair and replication in humans. The authors will review the clinical and cytological presentation of each disease, the genetic heterogeneity, as inferred by complementation analysis, and the differentiating characteristics of each. The authors will conclude with a discussion of th

Creutzfeldt-Jakob disease following pituitary-derived human growth hormone therapy: a new American case.

A fourth histologically-confirmed American case of Creutzfeldt-Jakob disease (CJD) related to human growth hormone (hGH) therapy is reported. Like kuru, the illness was dominated by cerebellar signs and relatively little mental deterioration. The diagnosis was strongly supported premortem by the presence of two abnormal 30 kDa proteins in the CSF that are seen almost exclusively in CJD. The characteristic clinical picture coupled with such biochemical data allow a reasonably accurate premortem diagnosis of hGH-related iatrogenic CJD to be made.

Topographical anatomy of the ciliary sulcus.

The topography of the ciliary sulcus area in humans was examined by slitlamp biomicroscopy and scanning electron microscopy. Characteristics of this area included the following: the sulcus was angulated anteriorly; the ciliary processes were of unequal length; the zonules did not insert on the tips of the ciliary processes but, instead, inserted slightly posteriorly; the contour of the sulcus area was irregular; bands stretched from the base of the ciliary processes to the posterior surface of the iris, making the sulcus a potential space in some areas of the eye. Effects of this topography on

[Study of the electrophoretic and kinetic properties of erythrocyte glucose-6-phosphate dehydrogenase in hereditary enzyme deficiencies. Description of a new variant of glucose-6-phosphate dehydrogenase: the Gd Kremenchug].

A study of the activity of human erythrocyte G-6-PD has been completed on a great number of normal controls, coming from different regions of the USSR. This study has led to the discovery of a new variant of erythrocytic Gd, which was named "Kremenchug" from the origin of the patient. This new Gd, has a very low Km for G-6-P, and has a greater affinity for 2-deoxyglucose-6-phosphate. The maximal activity versus the pH of the medium is also characteristic of this new Gd.

Shock treatment, brain damage, and memory loss: a neurological perspective.

The author reviews reports of neuropathology resulting from electroconvulsive therapy in experimental animals and humans. Although findings of petechial hemorrhage, gliosis, and neuronal loss were well established in the decade following the introduction of ECT, they have been generally ignored since then. ECT produces characteristic EEG changes and severe retrograde amnesia, as well as other more subtle effects on memory and learning. The author concludes that ECT results in brain disease and questions whether doctors should offer brain damage to their patients.

Parallel studies on collagen hydroxyproline and hydroxylysine in human skin biopsies.

Studies on hydroxyproline and hydroxylysine, the two amino acids characteristic of collagen and related glycoproteins, were undertaken on biopsies of pathologic and clinically normal human skin. No statistically significant differences between clinically normal skin of mamma, thorax, axilla, femur, hip and sacral area were found. A decreased collagen content was seen in chronic pemphigus (bullous pemphigoid), amyloidosis, scleromyxedema and the edge of a leg ulcer. Determination of both amino acids is considered necessary to characterize alterations of collagen.

Mixed IgG-IgA cryoglobulinemia in human serum sickness. Evidence for Equine IgG in the cryoprecipitate.

Serum sickness followed the administration of anti-lymphocyte globulin to a patient with multiple sclerosis. In addition to other characteristic features of this syndrome, there was hypocomplementemia and transient renal dysfunction similar to that observed in the 'one-shot' experimental model of serum sickness. Cryoglobulinemia was transiently demonstrable at the height of the inflammatory response. Analysis of the purified cryoprecipitate revealed the presence of human IgG and IgA, and, in addition, equine IgG. This demonstration of a well-defined exagenous antigen in the cryoprecipitate pro
