We retrospectively evaluated neurological and other complications related to posterior lumbar interbody fusion (PLIF) performed by facet joint excision and pedicle screw fixation, in 148 consecutive patients with degenerative disorders. Barbiturates, etomidate and propofol decrease cerebral blood flow (CBF), mediated by a decrease in cerebral metabolism, thus decreasing intracranial pressure (ICP). We reviewed conventional scleral buckling and vitrectomy for uncomplicated rhegmatogenous retinal detachment due to posterior retina break. A comparison was made for a consecutive series of 21 cases undergoing scleral buckling procedure and 15 cases receiving vitrectomy as the initial surgery. The diagnostic and therapeutic influence of laparoscopy has been studied in 255 patients presenting with nontraumatic acute abdominal pain. Laparoscopy provided a correct diagnosis in 93% (236 of 255) of the cases, the others requiring a laparotomy. An erroneous preoperative diagnosis was corrected by laparoscopy in 50 patients (20%), which called for a change of treatment in 25 patients (10%). Trauma, primarily due to traffic accidents, is the most common cause of death in childhood. In the context of abdominal trauma, the probability of injuries to urinary organs in children is higher than in adults. Peritoneal lavage and use of the CRAMS scale have proved to be reliable methods for assessment and planning of therapy for traumatised children. When it comes to treatment for splenic rupture, efforts should be made to preserve the greatest possible amount of splenic tissue to prevent post-splenectomy sepsis.

Invoking unacceptable operative risks, many institutions have adopted a conservative policy toward carotid stenosis in patients who require cardiopulmonary bypass (CPB). We have continued to apply simultaneous carotid endarterectomy/coronary artery bypass grafting (CEA/CABG) in selected patients, and in order to place operative risk in perspective, our experience with CEA/CABG was reviewed and contrasted with both CEA and CABG performed as isolated procedures. Thirty-four nonambulatory patients with progressive neuromuscular spinal deformity were surgically managed using a 1/4" U-shaped double rod construct with segmental instrumentation from T2 to the pelvis accompanied by posterior spinal fusion.
