The 2023 European Respiratory Society Congress took place on a hybrid platform, with participants joining online and in-person in Milan, Italy. The congress welcomed over 20 000 attendees, bringing together exciting updates in respiratory science and medicine from around the world. In this article, early career members of Assembly 10 (Respiratory Infections) summarise a selection of sessions across a broad range of topics, including presentations on bronchiectasis, nontuberculous mycobacteria, tuberculosis, cystic fibrosis and coronavirus disease 2019.

Information on prevalence of special needs in internationally adopted children (IAC) is incomplete. We reviewed data from 422 IAC screened at a single Centre in Italy in 2015-16. Prevalence of special needs reached 17.1% (n = 72). Among these children, the most frequent conditions were fetal alcohol spectrum disorders (FASD; n = 30; 7.1%), cleft lip palate (n = 8; 1.9%) and other congenital malformations (n = 20; 4.7%). Worrisomely, 25 out of 52 (48.1%) Russian children presented with FASD.

Quadricuspid aortic valve is a rare occurrence, usually presenting with insufficiency rather than stenosis. We report an aortic valve replacement in a septuagenarian with a stenotic quadricuspid aortic valve, achieving a good result with a Perceval (Sorin Biomedica Cardio Srl, Sallugia, Italy) sutureless bioprosthesis. We emphasize the particular technique of implantation and possible pitfalls. Sutureless bioprosthetic valves present a feasible option for aortic valve replacement in patients with uncommon valve morphologies, aiming to reduce operative time.

The rates of severe side reactions observed in Stockholm I and Italian Trials are reported. Children vaccinated with the same DTPwc in both trials experienced high fever and persistent crying more frequently than in any other group, while similar rates for all the severe side reactions were observed among DTaPs and DT vaccines. Recipients of different DTaP vaccines showed similar rates of severe reactions, which did not differ from recipients of DT vaccines, either in Sweden or Italy.

An 81-year-old man, previously a shepherd in Italy, presented with recurrent Escherichia coli empyema over an 8-month period. His empyema was caused by an infected, nonviable hepatic Echinococcus cyst that eroded the diaphragm and led to intermittent spillage and pleural seeding. This case demonstrates that when dealing with Escherichia coli empyema, a subdiaphragmatic source ought to be suspected, and among immigrants from areas with prevalent hydatid disease, infected hepatic Echinococcus cyst might rarely be the cause.

Leprosy is currently uncommon in Europe: the diagnosed cases are almost all imported from endemic areas. We report on an autochthonous case of borderline lepromatous leprosy in a 71-year-old Portuguese woman. The case was complicated by a reversal reaction and then by erythema nodosum leprosum. A literature review identified 18 reported cases of European autochthonous leprosy since 2000; all but one were observed in Mediterranean countries. Therefore, active clusters of leprosy persist in Europe, particularly in Spain, Greece, Portugal, and Italy.

Aortic root replacement with porcine xenograft is a valuable treatment option in acute aortic dissection, but conduits are often prone to degeneration. Reoperation is still associated with high operative mortality, and it usually requires root removal and repetition of the Bentall procedure, or a less radical option limited to valve replacement. We describe two cases of Freestyle root degeneration in patients with chronic aortic dissection, in whom we performed a valve-in-valve procedure with the Perceval S prosthesis (Sorin Group, Saluggia, Italy).

We report a case of rash caused by crushing of a male of Oryctes nasicornis (Linnaeus 1758) (Coleoptera, "http:// it. wikipedia. org/ wiki/ Scarabaeidae" \o "Scarabaeidae" Scarabaeidae), popularly known as "European rhinoceros beetle", on the skin of an Italian tourist who developed the reaction during a trip to Turkey. The rash appeared one hour after the crushing of the insect on the skin. The patient was observed one day later, when she returned to Italy. To our knowledge, no similar cases have been reported in the literature.

Occupational cancer is an important public health concern in Italy and in many industrialized countries. The difficulties in monitoring and the complexity in retrieving occupational cancer cases have required the enrolment of a national epidemiologic sureveillance system at national scale with active search methods. In November 2011, the Medicine's Italian Agency published the Pediatric Working Group's recommendation; that is do not use the quadruple vaccine against measles, rubella, mumps and varicella as first dosage in children from 12 to 23 months, because it involves an increase of febrile convulsions risks. It caused a big discussion between international and national Scientific and Institutional Bodies. In Italy, varicella is the most common vaccine preventable disease and preventive measures are not uniform. Clear and universally accepted indication is needed.

In some premature newborns, 7 months old and with less than 1 kg of body wheight, total parenteral nutrition is used for weeks, so that good antiseptics can cooperate to the prevention of nosocomial infections, associating the best biocide effects to the best topical tolerability. Details are reported on the biocide cutaneous properties of some chloro-derivates, as sodium hypochlorite and NaDCC, of ethyl and propyl alcohols, of chlorhexidine, of iodophors and also of triclosan and octenidine (even if these latter biocids are not normally available in Italy as cutaneous antiseptics).

We report the case of a 36-year-old Senegalese male with non-scarring alopecia of the scalp, including nodules and pustules, diagnosed as tinea capitis caused by Trichophyton soudanense. This dermatophyte is endemic in Central Africa and is becoming more frequent in Europe because of immigration. It has seldom been isolated in Italy. Tinea capitis is common in childhood and it is rare in adults, in which female sex is preferred. In adults, alopecic patches have to be distinguished from those due to other dermatoses inducing alopecia.

A case of mediastinal lymphoma in a free-living, adult male European badger (Meles meles), from Trento Province (northern Italy) is described. A yellow-red mass delimited by a thick fibrous capsule completely occupied the anterior mediastinum. Based on the histologic and immunohistochemical findings, a diagnosis of mediastinal lymphocytic lymphoma was made. Last September the Italian National Commission on Bioethics issued an opinion on the use of electro-shock (or electroconvulsive) therapy. It has now been made available in English translation and is reprinted here. The commission had been asked for an opinion by the leader of the Green Party group on Rome City Council. The request specifically asked whether it was now advisable to suspend the practice of this particular therapy. In producing its opinion, the commission's members seem to have consulted widely within Italy but not much outside.

Visceral leishmaniasis (VL) in pregnancy is rare in Mediterranean countries. We report here 2 cases of VL in pregnant women who acquired the infection in Italy. In the first case, the disease was diagnosed and treated with liposomal amphotericin B during the pregnancy. In the second case, diagnosis was established and treatment with meglumine antimoniate both undertaken shortly after delivery. The 2 infants were followed clinically and serologically for 8-9 months after birth, but no evidence of congenital VL was observed.

This study was carried out to validate the enlarged translabyrinthine approach for the surgical management of large vestibular schwannomas. A retrospective review of the charts of 53 patients with large tumours removed via the enlarged translabyrinthine approach at the Gruppo Otologico, Piacenza, Italy, during the last five years was carried out. The ability to control large tumours and the achievement of total removal with low morbidity and very few complications, demonstrate that tumour size does not influence the use of the enlarged translabyrinthine approach for managing large tumours.

In October 1990 pneumonia due to Legionella pneumophila was diagnosed in two employees working in the area of Apulia, southern Italy, where artesian wells were in construction. Although the exposure to excavation has been associated with Legionnaires' disease, in our investigation the illness occurred only in those employees who were present when the water emerged from the ground under high pressure. On the basis of this report, water appears as the most likely reservoir of the organism and the main route of infection.
