[Ischemic contractures of muscle and nerve lesions (author's transl)].

Reporting 14 own cases symptomatology and treatment of the common ischemic syndromes of the extremities (Volkmann's contracture, thumb adduction contracture respectively contracture of the intrinsic hand muscles and anterior tibial syndrome) including the regularly concomitant nerve lesions are discussed. Edema and compression beyond the primary ischemia are essential factors in pathogenesis of nerve and muscle lesions. The electromyographic examination is helpful in diagnosis, prognosis and treatment of the severe sequelae of nerve and muscle. Since late diagnosis yields poor therapeutical re

Injuries among powered two-wheeler users in eight European countries: a descriptive analysis of hospital discharge data.

Powered two-wheelers (PTWs--mopeds, motorcycles, and scooters) remain the most dangerous form of travel on today's roads. This study used hospital discharge data from eight European countries to examine the frequencies and patterns of injury among PTW users (age≥14 years), the predicted incidence of the loss of functional ability, and the mechanisms of the head injuries observed (all in light of increased helmet use). Of 977,557 injured patients discharged in 2004, 12,994 were identified as having been injured in PTW collisions. Lower extremity injuries accounted for 26% (25.6-26.7, 95% C.I.)

Injury to recreational and professional cricket players: circumstances, type and potential for intervention.

This paper describes injury (circumstances and type) experienced by sub-populations at all levels of cricket and, where possible, the type of protective equipment used. The sample differs to that generally examined in the literature in that it is not restricted to evaluation of elite and professional players only. Over a 6-year period (2000-2005), 498 cases were identified. The average age of injury was 27 years and 86% of those injured were male. The population incidence rate was 2.3 per 100,000 per year, and participation incidence rate 39 per 100,000 per year. Over all age groups upper limb

Prevention and treatment of infected foot and ankle wounds sustained in the combat environment.

Combat injuries to the foot and ankle are challenging to treat due to frequent high-energy mechanisms, environmental contamination, and soft tissue and bony damage. Prevention and treatment of infections in injuries to the foot and ankle are critical to achieving the goals of tissue healing and restoration of function. The guidelines for treatment of these foot and ankle injuries are similar to those in place for civilians; however, allowances must be made for the realities of combat including an often austere environment, the need for evacuation, and limitations on resources available for tre

Medial sural artery as a salvage recipient vessel for complex post traumatic microvascular lower limb reconstruction.

BACKGROUND: Complex lower extremity trauma still poses a formidable challenge for micro vascular reconstruction. The extensive surrounding zone of trauma can make it difficult to find a suitable recipient pedicle for anastomosis. A need was felt for exploration of newer recipient vessels in lower extremity which could be harnessed for reliable and tension free micro anastomosis. The purpose of the present report is to highlight the possibility of using medial sural artery for safe micro vascular anastomosis in selected scenarios like vessel depleted extremity. METHODS: Between 2008 and 2013,

Distally based extended peroneal artery septocutaneous perforator cross-bridge flap without microvascular anastomoses for reconstruction of contralateral leg and foot soft tissue defects.

A cross-leg or cross-bridge free flap is one of the choices for the reconstruction of serious leg soft tissue defects. Here, we report on six cases of leg and foot reconstruction with a distally based extended peroneal artery septocutaneous perforator cross-bridge flap without microvascular anastomoses. The vascular pedicle includes the peroneal artery, its perforator branch, and concomitant veins. The total pedicle length ranges from 10 to 14 cm, and the size ranges from 18 x 8 to 21 x 10 cm. All flaps survived completely without complications. Compared with a cross-leg or cross-bridge free f

[Whole body spiral CT in primary diagnosis of patients with multiple trauma in emergency situations].

PURPOSE: The aim of this study was to evaluate the role of a fast whole body helical CT scanner for primary diagnosis in trauma patients. METHODS: 27 severely injured patients (9 women, 18 men; mean age 43 years) were first examined with a helical CT scanner allowing for digital radiograms up to a length of 1024 mm and continuous helical scans of up to 70 seconds (slice thickness 3 to 10 mm, pitch factor up to 2). The primary CT diagnosis was verified either by x-ray after the CT examination or during the subsequent days, by abdominal ultrasound, by additional CT scans in the following days,

Revision Total Hip Arthroplasty with a Modular Fluted Tapered Stem for a Periprosthetic Femoral Fracture.

BACKGROUND: As the number of primary total hip arthroplasty procedures performed each year continues to rise, so too do the number of complications, including periprosthetic femoral fracture1-9. Vancouver B2 and B3 periprosthetic femoral fractures are difficult to treat because they require the surgeon to simultaneously manage a femoral fracture and gain new implant fixation. Fluted tapered stems have advanced the treatment of periprosthetic femoral fractures by providing immediate axial and rotational implant fixation distal to the fracture10-18. Modular fluted tapered stems provide the added

Honey as a topical treatment for wounds.

BACKGROUND: Honey is a viscous, supersaturated sugar solution derived from nectar gathered and modified by the honeybee, Apis mellifera. Honey has been used since ancient times as a remedy in wound care. Evidence from animal studies and some trials has suggested that honey may accelerate wound healing. OBJECTIVES: The objective was to determine whether honey increases the rate of healing in acute wounds (e.g. burns, lacerations) and chronic wounds (e.g. skin ulcers, infected surgical wounds). SEARCH METHODS: For this first update of the review we searched the Cochrane Wounds Group Specialise

Evaluation of Results of Primary Repair of Open Tendo-Achilles Injury in the Department of Casualty, MMCH, Bangladesh.

Achilles tendon injury has been increasing over recent years due to a variety of causes. The injury of the Achilles tendon is a disabling condition, which is difficult to treat if there is a gap or defect between the two ends of the disrupted tendon. Patients with a rupture of the Achilles tendon should be operated without delay. Most of the people in our country use flat pan in the latrine, which are at a same level of the toilet floor. Surface of the toilet pan breaks down and cause sharp cut. Some of the cases of open tendon injuries occur due to road traffic accident, cut by broken glass a

Endoscopic Transforaminal and Lateral Recess Decompression After Previous Spinal Surgery.

BACKGROUND: Leg pain and back pain after lumbar laminectomy and spinal decompression fusion surgery are common and often related to persistent lumbar foraminal or lateral recess stenosis. Although persistent symptoms often stem from incomplete decompression during the primary index surgery, recurrent symptoms may also be the result of intervertebral cage subsidence due to loss of intervertebral and neuroforaminal height. OBJECTIVE: The aim of this study was to investigate the feasibility of using the outpatient transforaminal decompression procedure as an alternative to inpatient open procedu

Adaptive prosthetics for the lower extremity.

The potential for lifestyle recovery is tremendous for most lower extremity amputees. The amazing and ever-expanding array of adaptive prosthetics can help make the devastating loss of amputation more bearable for patients, their families, and their health care team. The new amputee, in a state of shock and grief, does not know what his or her prosthetic options are. It is crucial that the surgeon is knowledgeable about what the patient can have and what the patient needs to ask for. Dana Bowman stated: Ideally, the new amputee should say to their doctor, "I'd like my leg to be lightweight, fl

Free radial forearm fasciocutaneous flap in the treatment of distal third tibial osteomyelitis.

BACKGROUND: Osteomyelitis of the lower limb, associated with soft-tissue defects, is a reconstructive challenge. Microvascular free-tissue reconstruction is an important surgical option with the superiority of free muscle and myocutaneous flaps being explored by various publications. Muscle flaps provide good quality vascularized tissue which can be contoured into defects, but their bulk can often lead to the reconstruction having a suboptimal aesthetic outcome. This result is usually most evident in the lower third of the tibia. We present our series of free radial forearm fasciocutaneous fla

The combined use of the Ilizarov method and microsurgical techniques for limb salvage.

The purpose of this article is to review clinical outcomes and propose a new classification scheme for combined use of Ilizarov Method with free tissue transfer for limb salvage. This is an Institutional Review Board-approved retrospective review of 62 patients treated with free tissue transfer and Ilizarov method over the past 15 years at a single institution. The surgical management of these patients is classified into 4 distinct approaches. The mean age was 37 years with the most common injury being Gustillo IIIB tibial fractures (61%). Eighty-seven percent of patients had failed prior fixa

Association of Persistent Interlimb Kinetic Asymmetry at Return to Sport With Increased Risk of Contralateral Anterior Cruciate Ligament Injury.

BACKGROUND: Return-to-sport (RTS) decision-making after anterior cruciate ligament (ACL) reconstruction (ACLR) is primarily based on clinical and functional criteria. Despite meeting these criteria, a substantial proportion of athletes sustain a second ACL injury, most commonly involving the contralateral knee, suggesting a persistent gap between clinical clearance and biomechanical recovery. PURPOSE: To determine whether interlimb kinetic asymmetry at the time of RTS is associated with an increased risk of subsequent contralateral ACL injury and whether kinetic asymmetry provides incremental

Accurately and effectively predict the ACL force: Utilizing biomechanical landing pattern before and after-fatigue.

BACKGROUND AND OBJECTIVE: As a fundamental exercise technique, landing can commonly be associated with anterior cruciate ligament (ACL) injury, especially during after-fatigue single-leg landing (SL). Presently, the inability to accurately detect ACL loading makes it difficult to recognize the risk degree of ACL injury, which reduces the effectiveness of injury prevention and sports monitoring. Increased risk of ACL injury during after-fatigue SL may be related to changes in ankle motion patterns. Therefore, this study aims to develop a highly accurate and easily implemented ACL force predicti
