Catheter ablation for symptomatic atrial fibrillation (PVI-SHAM-AF): a randomised, double-blind, sham-controlled, multicentre trial.

BACKGROUND: Guidelines recommend catheter ablation for symptom relief in patients with atrial fibrillation. The aim of this trial was to ascertain whether catheter ablation improves atrial fibrillation-related quality of life more than a sham procedure. METHODS: PVI-SHAM-AF was a double-blind, multicentre, randomised trial conducted at nine study sites in Germany and Poland. Patients aged 18 years or older with symptomatic paroxysmal or persistent atrial fibrillation were randomly assigned in a 2:1 ratio to catheter ablation or a sham procedure using an automated online randomisation system w

Tubing the trabeculectomy: a novel filtration technique.

PURPOSE: To evaluate the safety and efficacy of trabeculectomy, modified with a subscleral polytetrafluoroethylene (Teflon) tube for posterior aqueous diversion, in eyes with refractory glaucoma. METHODS: In this prospective pilot study, 43 eyes with refractory glaucoma underwent fornix-based trabeculectomy with placement of a 26 G IV cannula (tube), within a subscleral tunnel designed to create a posterior outflow pathway. Success was defined as an IOP of ≥6 mmHg and ≤18 mmHg, with at least 30% reduction from preoperative treated values and was deemed absolute success if no anti glaucoma med

Diabetic macular atrophy, a pilot exploration of its characteristics, progression, and visual implications.

OBJECTIVES: To characterise diabetic macular atrophy (DMA) and identify risk factors for progression to retinal pigment epithelium (RPE) and outer retinal atrophy. METHODS: In this retrospective longitudinal cohort study, we analysed 103 eyes from 70 patients with diabetic retinopathy (DR) and DMA, identified between 2015 and 2024. DMA was defined by clear-bordered hypopigmented RPE lesions on fundus photography with corresponding choroidal hypertransmission on optical coherence tomography (OCT). Lesions were classified as focal or diffuse and staged using modified criteria from the Classific

Cerebellar Tracts Microstructural Characteristics and Severity of Language Impairment in Post-stroke Patients with Aphasia: A DTI Study.

Aphasia is a language impairment resulting from stroke. While numerous studies have explored the association between supratentorial white matter damage and post-stroke aphasia using diffusion tensor imaging (DTI), the role of infratentorial structures, particularly the cerebellum, remains underinvestigated. Evidence suggests that the cerebellum may play a supportive role in language processing, but its contribution across different aphasia types is not well understood. This study aimed to explore cerebellar white matter involvement in individuals with Broca's, conduction, and anomic aphasia us

Chronic toxicity of pandemic-driven chloroquine diphosphate use in aquatic ecosystems: zebrafish (Danio rerio) as a model species.

Chloroquine diphosphate (CQD) is used for treating systemic and endemic diseases, with increased consumption during the COVID-19 pandemic. Its widespread use has led to increased detection in aquatic environments, where previous studies have reported oxidative stress and physiological disturbances in aquatic organisms. This study evaluated the chronic effects of CQD on zebrafish (Danio rerio) exposed for 21 days to environmentally relevant concentrations (0.6, 1.8, 5.4, 16.2, and 48.6 µg/L). Biomarkers related to metabolism (proteins, glucose, triglycerides), mucosal immunity (immunoglobulins)

An Investigation of Heterogeneity in Cerebellar Cortical Pathology Across Essential Tremor Cases: A Cluster Analysis.

Recent years have witnessed an evolution of our understanding of the underlying neuropathological features of essential tremor (ET). Studies have revealed a cascade of neurodegenerative changes in the cerebellar cortex, centered on the Purkinje cell (PC) population and its surrounding neuronal populations and connections. A fundamental question is whether there is heterogeneity across ET cases with respect to the expression of this cerebellar pathology. We examined postmortem tissue from 155 ET brains using a set of 15 quantitative metrics of cerebellar cortical pathology determined by histolo

Investigating Cerebello-Cortical Networks With EEG: Advances and Future Challenges.

The cerebellum is widely recognized for its contributions to motor, cognitive, and affective processes through dynamic cerebello-cortical networks. Recent studies using cerebellar-cortical electroencephalography (EEG), a technique that enables noninvasive, millisecond-resolution recordings of cerebellar and cortical activity, have revealed disease-specific spectral and network alterations in patients with movement and neurodegenerative disorders, including ataxia, essential tremor (ET), Parkinson's disease (PD), dystonia, as well as in healthy individuals. Synchronous cerebellar-cortical EEG r

Spaceflight-associated neuro-ocular syndrome (SANS): expert consensus on diagnosis and management.

Spaceflight-associated neuro-ocular syndrome (SANS) is a disorder primarily characterised by changes in the eye and optic nerve, but changes in brain features are also noted after spaceflight. SANS is caused by exposure to prolonged microgravity and at least one feature of SANS has been reported in 81% of National Aeronautics and Space Administration (NASA) astronauts. Early clinical features of SANS are observed in almost all individuals undergoing spaceflight, with the condition exacerbated by the duration of spaceflight. Knowledge of the features of SANS and the aetiology has rapidly grown

Clinical features and temporal trends in a large Japanese LHON cohort, 1995-2024: a retrospective study.

BACKGROUND/OBJECTIVES: Leber hereditary optic neuropathy (LHON) exhibits population-specific characteristics in Japan, but long-term temporal changes remain unclear. SUBJECTS/METHODS: We retrospectively analysed 179 patients with genetically confirmed LHON at Inoue Eye Hospital (1995-2024). Age at onset was evaluated using median (τ = 0.5) quantile regression, with temporal effects expressed per decade. Female proportion, m.11778 G > A frequency, and-among adults ( ≥ 20 years)-smoking and alcohol use were assessed using logistic regression. Age-band distributions (10-year intervals) were comp

Proton beam therapy for oropharyngeal cancer (TORPEdO): a phase 3, randomised controlled trial.

BACKGROUND: The clinical benefits of intensity-modulated proton therapy (IMPT) compared with intensity-modulated radiation therapy (IMRT) for patients with oropharyngeal squamous cell carcinoma remain uncertain with respect to treatment-related effects on physical function and quality of life. We aimed to compare late functional, patient-reported, disease control, and survival outcomes between IMPT and IMRT. METHODS: We did a phase 3 trial (TORPEdO) in 20 UK National Health Service hospitals. We randomly assigned (2:1) patients with locally advanced oropharyngeal squamous cell carcinoma to IM

Blood Glucose Homeostasis is Preserved in Patients with Mild-to-moderate Spinocerebellar Ataxia Type 2.

Spinocerebellar ataxia type 2 (SCA2) is a neurodegenerative disorder that shows cerebellar glucose hypometabolism, systemic hypermetabolism and weight loss. This study aimed to explore novel molecular mechanisms of disease for SCA2, based on the assessment of blood glucose homeostasis. A case-control and correlational study was conducted in 79 normoglycemic Cuban patients with SCA2 and 83 sex- and age-matched control subjects during a fasting state. A subset of 20 patients with SCA2 and 19 control individuals underwent an oral glucose tolerance test (OGTT). Several indices for assessing blood
