Maternal Obesity and Risk of Sudden Unexpected Infant Death.

IMPORTANCE: Rates of maternal obesity are increasing in the US. Although obesity is a well-documented risk factor for numerous poor pregnancy outcomes, it is not currently a recognized risk factor for sudden unexpected infant death (SUID). OBJECTIVE: To determine whether maternal obesity is a risk factor for SUID and the proportion of SUID cases attributable to maternal obesity. DESIGN, SETTING, AND PARTICIPANTS: This was a US nationwide cohort study using Centers for Disease Control and Prevention National Center for Health Statistics linked birth-infant death records for birth cohorts in 2

Investigating the Impact of Maternal Residential Mobility on Identifying Critical Windows of Susceptibility to Ambient Air Pollution During Pregnancy.

Identifying periods of increased vulnerability to air pollution during pregnancy with respect to the development of adverse birth outcomes can improve understanding of possible mechanisms of disease development and provide guidelines for protection of the child. Exposure to air pollution during pregnancy is typically based on the mother's residence at delivery, potentially resulting in exposure misclassification and biasing the estimation of critical windows of pregnancy. In this study, we determined the impact of maternal residential mobility during pregnancy on defining weekly exposure to pa

Risk of impaired cognition after prenatal exposure to psychotropic drugs.

OBJECTIVE: Prenatal exposure to psychotropic drugs may affect the trajectories of brain development. In a register study, we investigated whether such exposure is associated with long-term impaired cognitive abilities. METHOD: Individuals born in Denmark in 1995-2008 were included. As proxies for cognitive impairment, requiring special needs education, attending special needs school, diagnoses of neurological/mental disorder, missed final examinations, and low school grade average were used. We accounted for maternal confounders. RESULTS: We identified 868 159 individuals of whom 13 983 (1.6

Weight of evidence for an association between adverse reproductive and developmental effects and exposure to disinfection by-products: a critical review.

Disinfection by-products (DBP) are a side effect of water chlorination. Some toxicological studies suggest an association between DBP exposure and adverse reproductive and developmental effects. This investigation considered all toxicological and epidemiological evidence for the various effects, outcome by outcome. The weight of evidence demonstrated that no association with DBP exposure exists for over a dozen outcomes including low and very low birth weight, preterm delivery, some specific congenital anomalies, and neonatal death. The analysis found inconsistent or very weak results for all

Everything All at Once: On Choosing an Estimand for Multi-component Environmental Exposures.

Many research questions-particularly those in environmental health-do not involve binary exposures. In environmental epidemiology, this includes multivariate exposure mixtures with nondiscrete components. Causal inference estimands and estimators to quantify the relationship between an exposure mixture and an outcome are relatively few. We propose an approach to quantify a relationship between a shift in the exposure mixture and the outcome-either in a single timepoint or longitudinal setting. The shift in the exposure mixture can be defined flexibly in terms of shifting one or more components

Effects of pet exposure in the first year of life on respiratory and allergic symptoms in 7-yr-old children. The SIDRIA-2 study.

The effects of pet exposure on the development of respiratory symptoms have recently been the matter of vivid discussion. Our objective was to determine the effects of exposure to cat or dog in the first year of life on subsequent respiratory/allergic symptoms in children in a large Italian multicentre study. As part of the SIDRIA-2 Study (Studi Italiani sui Disturbi Respiratori dell'Infanzia e l'Ambiente 2002), the parents of 20016 children (median age 7 yr) provided information on indoor exposures at different times in life and respiratory/allergic symptoms through questionnaires. Maternal serum concentrations of persistent organic pollutants and childhood acute lymphoblastic leukemia in the Finnish Maternity Cohort.

BACKGROUND/AIMS: Children are exposed to persistent organic pollutants (POPs) including polychlorinated biphenyls (PCBs) and organochlorine pesticides through in utero transfer from maternal serum. Few studies have evaluated the relationship between POPs and childhood leukemia risk. METHODS: The Finnish Maternity Cohort is a population-based pregnancy cohort with banked serum and linkages to cancer registry and other databases. We measured first-trimester levels of 25 POPs in maternal sera collected in 1986-2010 for 388 childhood acute lymphoblastic leukemia (ALL) cases (<15 years) and 388 co

Fetal exposure to bisphenols and phthalates and risk of respiratory conditions in infancy. The Generation R Next Study.

Fetal exposure to bisphenols and phthalates may lead to alterations in the development of the respiratory and immune systems in children, subsequently increasing their susceptibility to respiratory conditions in infancy. We aimed to study the associations of fetal exposure to bisphenol and phthalate metabolites with the risk of respiratory conditions in infancy. This study, involving 1295 mother-child pairs, was embedded in a population-based prospective cohort in Rotterdam, the Netherlands. We measured maternal concentrations of bisphenols and phthalate metabolites in spot urine samples colle

Sex-specific association of prenatal exposure to halogenated persistent organic pollutants (Hal-POPs) with neonatal sex hormone levels and birth size: a birth cohort study using meconium as a biomonitoring matrix.

Halogenated persistent organic pollutants (Hal-POPs) are endocrine disruptors that posing risks to fetal development via transplacental transfer. This study utilized neonatal meconium, a non-invasive matrix reflecting cumulative exposure during the second- and third-trimester, to assess prenatal levels of per- and polyfluoroalkyl substances (PFASs), polybrominated diphenyl ethers (PBDEs), organochlorine pesticides (OCPs) in 271 mother-infant pairs from Guangzhou, China. The associations between Hal-POP exposure, neonatal sex hormones, and birth size Z-scores [weight (BWZ), length (BLZ), head c

Behavioral alterations in autism model induced by valproic acid and translational analysis of circulating microRNA.

Autism spectrum disorder (ASD) is characterized by difficulties in social interaction, communication and language, and restricted repertoire of activities and interests. The etiology of ASD remains unknown and no clinical markers for diagnosis were identified. Environmental factors, including prenatal exposure to valproic acid (VPA), may contribute to increased risk of developing ASD. MicroRNA (miRNA) are small noncoding RNA that regulate gene expression and are frequently linked to biological processes affected in neurodevelopmental disorders. In this work, we analyzed the effects of resverat

Maternal periconceptional alcohol consumption and congenital limb deficiencies.

BACKGROUND: Women of childbearing age report high rates of alcohol consumption, which may result in alcohol exposure during early pregnancy. Epidemiological research on congenital limb deficiencies (LDs) and periconceptional exposure to alcohol is inconclusive. METHODS: Data from the National Birth Defects Prevention Study (NBDPS) were examined for associations between LDs and patterns of maternal periconceptional (1 month before conception through the first trimester) alcohol consumption among LD case (n = 906) and unaffected control (n = 8352) pregnancies with expected delivery dates from 1

Animal models for probing the developmental basis of disease and dysfunction paradigm.

There is a major paradigm shift taking place in science that while simple is profound. The new paradigm suggests that susceptibility to disease is set in utero or neonatally as a result of the influences of nutrition and exposures to environmental stressors/toxicants. In utero nutrition and/or in utero or neonatal exposures to environmental toxicants alter susceptibility to disease later in life as a result of their ability to affect the programming of tissue function that occurs during development. This concept, which is still a hypothesis undergoing scientific testing and scrutiny, is called

In utero exposure to diethylstilbestrol (DES) does not increase genomic instability in normal or neoplastic breast epithelium.

BACKGROUND: In 1992, the National Cancer Institute (NCI) established the Continuation of Follow-Up of DES-Exposed Cohorts to study the long-term health effects of exposure to diethylstilbestrol (DES). Genetic effects on human breast tissue have not been examined. The authors investigated whether breast tissue of women exposed in utero to DES might exhibit the genetic abnormalities that characterize other DES-associated tumors. METHODS: Subjects enrolled in the NCI Cohort were queried about breast biopsies or breast cancer diagnoses. Available tissue blocks were obtained for invasive cancers (

Evaluation of changes in methylmercury accumulation in the developing rat brain and its effects: a study with consecutive and moderate dose exposure throughout gestation and lactation periods.

Methylmercury (MeHg) can be transferred to the fetus through the placenta and to newborn offspring through breast milk. The higher mercury (Hg) accumulation and susceptibility to toxicity in the fetus than in the mother during the gestation period is well known. However, the contribution of MeHg exposure through breast milk to the brain Hg concentration in offspring is not clear. The purposes of this study were to evaluate the changes in Hg concentration in the brain of offspring and its effects on the developing rat brain, based on consecutive and moderate doses of MeHg throughout gestation a

Effects of cocaine administration during early organogenesis on prenatal development and postnatal growth in mice.

Cocaine use has been associated with adverse developmental effects in humans. However, clinical reports both confirm and deny an association between cocaine use and malformations. Similarly, differences in species and strain, as well as route and timing of cocaine administration, have added to the difficulties in determining the teratogenicity of cocaine in animal models. This study was undertaken to compare the effects of dose, route, and timing of cocaine administration in ICR mice during early organogenesis. A single intraperitoneal (ip) administration of cocaine ( > or = 60 mg/kg) on Day 9

Re-analysis of a Genome-Wide Gene-By-Environment Interaction Study of Case Parent Trios, Adjusted for Population Stratification.

We investigate the impact of confounding on the results of a genome-wide association analysis by Beaty et al., which identified multiple single nucleotide polymorphisms that appeared to modify the effect of maternal smoking, alcohol consumption, or multivitamin supplementation on risk of cleft palate. The study sample of case-parent trios was primarily of European and East Asian ancestry, and the distribution of all three exposures differed by ancestral group. Such differences raise the possibility that confounders, rather than the exposures, are the risk modifiers and hence that the inference

Herbal agents and over-the-counter medications in pregnancy.

Exposures to over-the-counter and herbal products are frequent in pregnant women. Limited information exists on the effects of many of these agents during pregnancy; it is not safe to assume that because these products are available without a prescription that they are without danger to the pregnant woman and her fetus. The basic principles utilized in deciding whether to employ prescription medications such as dose, embryological timing and potential toxic fetal effects also apply to herbal medications and over-the-counter agents. Conventional reasoning indicates that maternal exposure to poo…

Impact of prenatal alcohol exposure on the newborn and the child.

Maternal use of alcohol during gestation is associated with FAS and less severe problems in exposed children, such as FAE and ARBD. This article reviewed the literature concerning the status of affected and exposed neonates and the available information on the developmental course of infants and children. Physical and behavioral characteristics of gestational alcohol exposure were described, and outcomes of retrospective and prospective research were reviewed. Currently, it is evident that heavy use and bingeing are associated with an increased risk to the offspring across a range of outcomes.

Impact of Deoxynivalenol, Cytokines, and Particulate Matter on Human Trophoblast Cells.

Placental inflammation and dysfunction play critical roles in adverse pregnancy outcomes, including preeclampsia and intrauterine growth restriction. Exposure to environmental factors, such as dietary toxins, infectious agents, and pollution, is thought to increase the risk of these outcomes. Despite the widespread use of HTR-8/SVneo and BeWo cells as trophoblast models, their comparative sensitivity to maternal exposome-related stimuli has not been systematically evaluated within the same experimental framework. we examined the responses of HTR-8/SVneo and BeWo trophoblast cell lines to mycot

Beyond cigarettes: waterpipe tobacco use as a key driver of paediatric asthma in Jordan.

BACKGROUND: Asthma is a prevalent chronic disease among children worldwide. Exposure to parental environmental tobacco smoke is a known risk factor for paediatric asthma, but the specific impact of different tobacco products, such as waterpipe, is not well established. In Jordan, where waterpipe smoking is culturally accepted and widespread, understanding its association with childhood asthma is critical. This study aimed to assess the association between parental tobacco use-particularly waterpipe smoking-and the presence of physician-diagnosed paediatric asthma in Jordan. METHODS: A hospita
