Symptom-Based Dosing for Neonatal Opioid Withdrawal: The OPTimize NOW Randomized Clinical Trial.

IMPORTANCE: Infants with neonatal opioid withdrawal syndrome (NOWS) who receive pharmacologic treatment are traditionally treated with a scheduled opioid taper. An alternate approach, symptom-based dosing, may better align treatment with withdrawal severity. OBJECTIVE: To compare time from birth to medical readiness for discharge for infants with moderate to severe withdrawal treated with either a symptom-based dosing or scheduled opioid taper approach. DESIGN, SETTING, AND PARTICIPANTS: In this cluster, crossover randomized clinical trial with run-in period, 23 US hospitals cared for infant

Association of polypharmacy and potential drug-drug interactions with adverse treatment outcomes in older adults with advanced cancer.

BACKGROUND: Polypharmacy is common in older adults who are starting cancer treatment and is associated with an increased risk of potentially inappropriate medications (PIMs) and potential drug-drug interactions (PDIs). The authors evaluated the association of medication measures with adverse outcomes in older adults with advanced cancer who were receiving systemic therapy. METHODS: This secondary analysis from GAP 70+ Trial (ClinicalTrials.gov identifier NCT02054741; principal investigator, Supriya G. Mohile) enrolled patients aged 70 years and older with advanced cancer who planned to start

Thirty-day readmissions in multiple sclerosis: An age and gender-based US national retrospective analysis.

BACKGROUND: Hospital readmission rate is an important indicator of the quality of care, healthcare economic burden, and post-discharge care. Multiple sclerosis (MS) is a potentially progressive neurological disease predominantly affecting young women. The natural history of the disease involves intermittent relapses and/or accrued baseline disability overtime especially in older patients contributing to frequent hospitalizations. The readmission metrics for patients with MS have not been studied. OBJECTIVE: To estimate nationwide 30-day readmission (30-DR) rate among patients hospitalized wit

Cannabis-Related Problems, Adverse Events, Clinical Experiences and Barriers to Intervening: Perspectives of Health Care Providers in Washington State.

BACKGROUND: Patients presenting with cannabis-related adverse events (AEs) and other problems are becoming increasingly prevalent in health care settings across the U.S. and Canada, increasing the burden on providers and health systems. Understanding how healthcare providers perceive and respond to cannabis AEs can inform medical training and clinical practices, health policy, and systems-level interventions. METHODS: The study was conducted in Washington State, a U.S. state that legalized cannabis for non-medical use in 2012. Health care providers were invited to complete an anonymous online

[Design and application of a dedicated support stand for fiberoptic bronchoscopy].

The fiberoptic bronchoscope, when fully integrated with imaging systems, enables real-time observation of airway structures. It can assist in procedures such as clearing respiratory secretions, tracheal intubation assistance, and bronchoalveolar lavage. It is widely used in various settings, including intensive care unit (ICU), emergency department, anesthesiology, and critical care respiratory department. However, the fiberoptic bronchoscope currently used in clinical practice faces clinical issues such as susceptibility to accidental touch, low operational efficiency, and difficulty in adjus

Implementation of No-Test Medication Abortions in Primary Care: A Roadmap from an Adolescent and Young Adult Medicine Clinic.

STUDY OBJECTIVE: Over 2 years post-Roe, 22 states have banned or significantly limited abortion access, exacerbating existing structural barriers to abortions for adolescents and young adults (AYAs), including cost, transportation, parental notification requirements, and confidentiality concerns. We provide a roadmap for and demonstrate the feasibility of implementing no-test medication abortions (MABs) to expand abortion access in an adolescent-focused pediatric primary care setting. METHODS: An interdisciplinary team in an AYA-focused primary care clinic developed and implemented a no-test

Developing and validating an artificial intelligence-based application for predicting some pregnancy outcomes: a multi-phase study protocol.

BACKGROUND: Pregnancy complications such as preterm birth, low birth weight, gestational diabetes mellitus, preeclampsia, and intrauterine growth restriction significantly affect both maternal and neonatal health outcomes. Early identification of high-risk pregnancies is essential for timely interventions; however, traditional predictive models often lack accuracy. This study aims to develop and validate an AI-based application to improve risk assessment and clinical decision-making regarding pregnancy outcomes through a multi-phase approach. METHODS: This study comprises three phases. In Phas

The effectiveness of intratympanic injections with methylPREDnisolon versus placebo in the treatment of vertigo attacks in MENière's disease (PREDMEN trial): a study protocol for a phase-3 multicentre, double-blinded, randomised, placebo-controlled trial.

INTRODUCTION: Intratympanic corticosteroids are commonly used in the treatment of Menière's disease (MD). However, few and small randomised controlled trials (RCT) on the effectiveness of intratympanic corticosteroids have been performed. A recent Cochrane review suggested that a well-conducted placebo-controlled RCT with a large study population is required to evaluate the effectiveness of the use of intratympanic corticosteroids in MD. The following protocol describes a phase-3 multicentre, double-blinded, randomised, placebo-controlled trial to compare the effectiveness of methylprednisolon

Quality of death certification based on the documented underlying cause of death: A retrospective study.

BACKGROUND: The underlying cause of death (UCOD) documented in the death certificate is a cornerstone in the mortality data that has significant impact on national policies, health system, and socioeconomics. However, a wide range of inaccuracies have been reported worldwide and were linked to multiple factors, including sociodemographic development and lack of physician training. Hence, this study aimed to assess the quality of death certification by reviewing the reported UCOD in the death certificate and study the potential factors that might be associated with inaccuracies. METHODS: All i

Spasticity Management in Persons with Disorders of Consciousness.

INTRODUCTION: Spasticity is one of the most frequent neurological impairments affecting persons with disorders of consciousness (DoC). If left untreated, it can mask signs of consciousness by inhibiting one's ability to interact with the environment. The lack of information about spasticity specific to patients with DoC may result in insufficient or even inappropriate treatment. OBJECTIVE: To report spasticity characteristics and management in a large dedicated DoC rehabilitation program. DESIGN: Retrospective chart review. SETTING: An inpatient rehabilitation hospital. PARTICIPANTS: Patie

Isolated Aspergillus Laryngitis: Spectrum, Management, and Review of Literature.

PURPOSE: Isolated aspergillus laryngitis is rare and often diagnosed after surgical excision or biopsy for a suspected premalignant or malignant pathology. Unlike other systemic or localized aspergillosis, there are no specific guidelines for isolated laryngeal aspergillosis. Our experience of dealing with a wide variety of isolated laryngeal aspergillosis showed that this entity is very responsive to medical therapy, making extensive debridement (as often carried out in surgically accessible aspergillosis) unnecessary as it would invariably lead to long-term dysphonia. MATERIALS AND METHODS:
