Vitamin D as a candidate host susceptibility factor in sugar-driven oral dysbiosis.

INTRODUCTION: Sugar-sweetened beverages (SSBs) represent a modifiable dietary factor that promotes oral dysbiosis by supplying fermentable sugars and generating low-pH conditions that favor acidogenic and acid-tolerant biofilms. Vitamin D plays a biologically significant role in oral host defense through regulation of the epithelial barrier, induction of antimicrobial peptides, immune modulation, inflammatory control, and mineral metabolism. However, the extent to which vitamin D modifies the SSB -oral microbiome -disease pathway in humans remains unclear. METHODS: This narrative-conceptual r

Cleft Lip/Palate Speech: Optimizing Speech Outcomes With Limited Speech Pathology Resources.

Children born with cleft palate, with or without cleft lip (CP ± L), are at high risk for speech and resonance disorders that can significantly impact communication and quality of life. Causes of cleft-related speech disorders include velopharyngeal insufficiency, malocclusion, dental anomalies, and airway obstruction. Accurate differential diagnosis is critical, as mismanagement can result in unnecessary surgery or ineffective therapy. Nonetheless, many children do not receive specialist evaluation or targeted intervention. Optimal outcomes require coordinated multidisciplinary care, with spe

Study of the effect of a photocurable bioadhesive composite hydrogel for dentin caries management.

OBJECTIVE: Conventional invasive restorations for dental caries often leads to healthy tissue loss. This study developed a photocurable composite bioadhesive hydrogel (GelMA-QAS/DMA@HAP) as a minimal invasive surface coating for the management of caries through synergistic antibacterial and remineralization pathways. METHODS: Gelatin methacryloyl (GelMA) was functionalized with a C8 alkyl quaternary ammonium salt (QAS) for antibacterial activity and methacryloyl dopamine (DMA, 5 mg/mL) for wet adhesion, followed by the incorporation of 5% (w/v) nano-hydroxyapatite (nHAP). Adhesive performance

Metabolic Profiling of Individuals with Missing Teeth and Tooth Loss.

Missing teeth have been linked to incident cardiovascular disease, diabetes, and all-cause mortality. Our previous study revealed that signs of oral infections and inflammatory conditions (i.e., periodontal disease and dental caries) are associated with disadvantageous features of circulating metabolites. This study investigates whether missing teeth and tooth loss, the end points of these diseases, are associated with similar metabolic features. The 2 Finnish population-based studies Health-2000 (n = 6,197) and FINRISK-97 (n = 6,050) were included, as was Parogene (n = 465), a cohort of patie

Clinical Effectiveness of Different Irrigation Agents in Temporomandibular Arthrocentesis: Systematic Review and Meta-Analysis.

Background: Disorders of the temporomandibular joint (TMJ) affect millions worldwide and rank among the most frequent causes of orofacial pain unrelated to dental disease. Beyond discomfort, they can restrict mandibular motion and impair chewing efficiency. Arthrocentesis has been adopted as a favored surgical approach after conservative therapy fails since joint lavage can reduce inflammation and restore mobility with minimal invasiveness. A variety of irrigants are available for this procedure, including normal saline, Ringer's lactate, and ozonated water, each offering potential therapeutic

Impact of sequential administration of detomidine, butorphanol, and midazolam on sedation, ataxia, stimulus response, and bispectral index in horses.

BACKGROUND: Standing sedation is a safe and cost-effective alternative to general anesthesia in horses, but challenges include achieving adequate drug effect to block the stimulus response without inducing ataxia or recumbency. A benefit of midazolam has been reported in equine dental procedures. Seeking synergy, a combination of lower doses of several pharmacologic agents, including midazolam, may improve the quality of sedation while minimizing adverse effects. Bispectral index (BIS) correlates with sedation scores in human ICU patients, but the correlation between sedation scores and BIS ha

The association between oral risk assessment and obstetric and newborn outcomes.

BACKGROUND: Periodontal disease in pregnancy is a known risk factor for adverse obstetric and offspring outcomes, including preeclampsia and preterm birth. The American College of Obstetricians and Gynecologists (ACOG) recommends screening all pregnant patients on their first obstetric visit with a simple, self-administered, three-question oral risk assessment survey. Yet, no studies have evaluated whether a positive screen on this oral risk assessment is associated with adverse obstetric or newborn outcomes. OBJECTIVE: We sought to assess the relationship between a positive overall oral risk

AIM: Biomarker-based evaluation of endodontic outcomes offers an objective alternative to traditional clinical assessments. The human nucleotide-binding domain, leucine-rich repeat, and pyrin domain-containing-3 (NLRP3) inflammasome is emerging as a potential indicator in inflammatory dental conditions. The aim was to correlate changes in pain levels with gingival crevicular fluid (GCF) levels of the NLRP3 inflammasome in patients with symptomatic irreversible pulpitis (SIP), before and after nonsurgical endodontic treatment. MATERIALS AND METHODS: This nonrandomized matched comparative study

Association Between `Life's Essential 8' and Periodontitis: A Cross-Sectional Study From the NHANES Database.

BACKGROUND: Periodontitis is a chronic inflammatory disease, and the persistent inflammatory state it confers may lead to cardiovascular disease. Studies have shown that `Life's Essential 8 (LE8)`, an indicator of cardiovascular health, was associated with the risk of several inflammatory diseases. This study aimed to assess the association between LE8 and its subsets and the risk of periodontitis. METHODS: Data of participants who consented to periodontitis examinations were extracted from the National Health and Nutrition Examination Surveys (NHANES) database (2009-2014). LE8 contains 4 hea

Nutrition and Periodontitis: A Cross-Sectional Study from a Practice-Based Research Network.

BACKGROUND: Despite clinical interventional studies on the influence of diet on periodontal inflammatory parameters, there has been no practice-based cross-sectional study from a German population to date that has conducted both a comprehensive dental and periodontal examination and a thorough validated assessment of dietary behavior. Therefore, the aim of this pilot study was to evaluate, in a proof of concept, whether there is a correlation between the overall periodontal inflammatory surface area (PISA), periodontal clinical parameters (pocket probing depths (PPD), clinical attachment loss

Labial Gingival Recessions And The Post Treatment Proclination Of Mandibular Incisors.

BACKGROUND: Labial gingival recessions are a common periodontal condition characterized by the apical migration of the gingival margin, which can impact dental aesthetics and health. The proclination of mandibular incisors following orthodontic treatment has been implicated as a contributing factor to the development or exacerbation of such recessions. This study investigates the relationship between post-treatment incisor proclination and the occurrence of labial gingival recessions to perform clinical strategies for minimizing periodontal complications. Objectives were to evaluate the relati

A cluster randomized trial of xylitol chewing gum for prevention of preterm birth: The PPaX trial.

BACKGROUND: Maternal periodontal disease is associated with preterm and low-birthweight deliveries, but randomized trials of likely efficacious treatments (e.g., dental scaling and root planing) during pregnancy have not reduced these adverse outcomes. As an alternative, we hypothesized that periconception initiation of xylitol chewing gum would reduce the occurrence of preterm or low-birthweight deliveries among a historical high-prevalence population in Malawi. METHODS: We conducted an open-label, parallel-enrollment, matched-pair, cluster-randomized, controlled clinical trial across eight

Bacillus Calmette-Guérin vaccination as defense against SARS-CoV-2 (BADAS): a randomized controlled trial to protect healthcare workers in the USA by enhanced trained immune responses.

BACKGROUND: A large epidemic, such as that observed with SARS-CoV-2, seriously challenges available hospital capacity, and this would be augmented by infection of healthcare workers (HCW). Bacillus Calmette-Guérin (BCG) is a vaccine against tuberculosis, with protective non-specific effects against other respiratory tract infections in vitro and in vivo. Preliminary analyses suggest that regions of the world with existing BCG vaccination programs have lower incidence and mortality from COVID-19. We hypothesize that BCG vaccination can reduce SARS-CoV-2 infection and disease severity. METHODS:
