Implementation of computerized provider order entry in a neonatal intensive care unit: Impact on admission workflow.

OBJECTIVE: The study objective was to determine if computerized provider order entry (CPOE) systems impaired or enhanced workflow in the neonatal intensive care unit (NICU) by comparing the timing of administration of the first dose of antibiotics before and after CPOE system implementation. METHODS: We conducted a pre-post intervention comparative study of the length of time between admission and administration of initial antibiotics in neonates before and after a CPOE system was implemented. Clinical information and timing of antibiotic administration were collected on all inborn infants, w

Effect of Nasal Continuous Positive Airway Pressure on Infants With Meconium Aspiration Syndrome: A Randomized Clinical Trial.

IMPORTANCE: Nasal continuous positive airway pressure (NCPAP) as a primary respiratory therapy in meconium aspiration syndrome (MAS) has not been studied extensively. Nasal continuous positive airway pressure, when applied in newborns with MAS, may resolve atelectasis by sufficiently expanding partially obstructed small airways and stabilizing the collapsing terminal airways to enhance oxygen exchange. OBJECTIVE: To compare NCPAP vs standard care in neonates with moderate to severe respiratory failure due to MAS in reducing the need for invasive ventilation. DESIGN, SETTINGS, AND PARTICIPANT

Transfusion therapy in critically ill children.

Critically ill children in pediatric intensive care units are commonly indicated for blood transfusion due to many reasons. Children are quite different from adults during growth and development, and that should be taken into consideration. It is very difficult to establish a universal transfusion guideline for critically ill children, especially preterm neonates. Treating underlying disease and targeted replacement therapy are the most effective approaches. Red blood cells are the first choice for replacement therapy in decompensated anemic patients. The critical hemoglobin concentration may

Intravenous indomethacin for preventing mortality and morbidity in very low birth weight infants.

BACKGROUND: Patent ductus arteriosus (PDA) and intraventricular haemorrhage (IVH) are both associated with increased mortality and morbidity in preterm infants. Indomethacin has been used successfully to treat symptomatic PDA and may also prevent or limit IVH in the neonatal period. There are however potential unwanted side effects of indomethacin, in particular a potential for reduced organ perfusion that might outweigh any clinical benefits. The prophylactic use of indomethacin, where infants who may not have gone on to develop a symptomatic PDA or IVH would be exposed to indomethacin, warra

Compliance with antibiotic therapy guidelines in french paediatric intensive care units: a multicentre observational study.

BACKGROUND: Bacterial infections (BIs) are widespread in ICUs. The aims of this study were to assess compliance with antibiotic recommendations and factors associated with non-compliance. METHODS: We conducted an observational study in eight French Paediatric and Neonatal ICUs with an antimicrobial stewardship programme (ASP) organised once a week for the most part. All children receiving antibiotics for a suspected or proven BI were evaluated. Newborns < 72 h old, neonates < 37 weeks, age ≥ 18 years and children under surgical antimicrobial prophylaxis were excluded. RESULTS: 139 suspected

A pulmonary score for assessing the severity of neonatal chronic lung disease.

BACKGROUND: Limited data are available to describe the spectrum of severity of neonatal chronic lung disease. In the multicenter Supplemental Therapeutic Oxygen for Prethreshold Retinopathy of Prematurity trial, all infants had some degree of pulmonary dysfunction, because eligibility required a median oxygen saturation of < or =94% with room air. Infants randomized to the supplemental oxygen group (oxygen saturation target of 96-99%) had more pulmonary morbidity than did those in the conventional group (oxygen saturation target of 88-94%). This prompted the retrospective development of a pulm

Persistent pulmonary hypertension of the newborn: experience in a single institution.

Persistent pulmonary hypertension of the newborn (PPHN) remains one of the most challenging situations in the neonatal intensive care unit, and it is associated with high mortality and morbidity. The optimal treatment for PPHN is controversial. We report our 9-year experience in the management of PPHN through a retrospective review of 29 neonates with persistent pulmonary hypertension. The diagnosis of PPHN is made by echocardiography and/or preductal and postductal oxygen tension difference. The treatment modalities include supportive medical care, vasodilator therapy, mechanical ventilation

Streptococcus group B isolates in a regional hospital area.

A survey of the frequency of group B streptococcal infections in the Sharon area (Israel) was done in this laboratory. In the female genital tract streptococcus group B was found in 11.8%. This microorganism was recovered in lower frequencies (1.6%-7.4%) in other infection sites (CSF, wounds, throat, blood, and urine). The streptococci were identified as belonging to group B by biochemical properties such as resistance to bacitracin and capability to hydrolyze hippurate. Later the isolates were classified serologically. Serotypes Ib, Ic, and II were predominant in the vaginal smears (25%-28% e

Surgery for congenital heart disease in low-birth weight neonates: a comprehensive statewide Connecticut program to improve outcomes.

Low-birth weight (LBW) remains a significant risk factor in surgery for congenital heart disease (CHD). The timing of surgery and the choice of complete repair vs palliative measures are controversial issues. Delay of surgery to achieve weight gain may result in poorer outcomes. The results of a statewide, cardiac surgery protocol in LBW infants using specific selection criteria for repair vs palliation or delay over a 42-month period are reviewed. From September 1999 through February 2003, 32 low-birth weight infants (1,320 to 2,500 grams) underwent surgery for congenital heart disease in Con

Pharmacological interventions for prevention and treatment of upper gastrointestinal bleeding in newborn infants.

BACKGROUND: Upper gastrointestinal bleeding is typically a mild, self-limiting condition that can affect both preterm and term neonates, although it can be severe particularly when associated with co-morbidities. Pharmacological interventions with a proton pump inhibitor (PPI), H2 receptor antagonist (H2RA), antacid, bismuth and sucralfate may have effects on both the prevention and treatment of upper gastrointestinal bleeding in infants. OBJECTIVES: To assess how different pharmacological interventions (PPIs, H2RAs, antacids, sucralfate or bismuth salts) administered to preterm and term neon

The fatal triad in pcr-negative pediatric autopsies: A focus on aspiration, occult sepsis, and underlying immune dysfunction.

BACKGROUND: To identify plausible pathophysiological mechanisms underlying death in pediatric autopsy cases with negative culture and PCR results in which the cause of death remained undetermined despite comprehensive evaluation. METHODS: Autopsy reports, histopathology, microbiology, perimortem procalcitonin (PCT), and anthropometric data were retrospectively reviewed (Jan 1, 2020-Dec 31, 2024). Three composite indices captured patterns consistent with aspiration-related respiratory failure, culture-negative systemic inflammation/multiorgan dysfunction, and immune or immunometabolic insuffic

Melatonin and circadian regulation of inflammatory-oxidative pathways in pediatric neurological disorders.

Circadian rhythms are intrinsic 24-hour cycles that regulate nearly all aspects of human physiology and play essential roles in brain development, immune regulation, and redox balance. Adult studies have explored the links between circadian rhythms and neurological or metabolic diseases, but research in pediatric populations remains limited. In infants and children, circadian systems undergo rapid maturation, and this developmental window is particularly vulnerable to disruption from maternal, environmental, or endogenous factors. The aim of this review is to examine how circadian mechanisms i

Sample entropy correlates with intraventricular hemorrhage and mortality in premature infants early in life.

BACKGROUND: Mortality and intraventricular hemorrhage (IVH) are common adverse outcomes in preterm infants and are challenging to predict clinically. Sample entropy (SE), a measure of heart rate variability (HRV), has shown predictive power for sepsis and other morbidities in neonates. We evaluated associations between SE and mortality and IVH in the first week of life. METHODS: Participants were 389 infants born before 32 weeks of gestation for whom bedside monitor data were available. A total of 29 infants had IVH grade 3 or 4 and 31 infants died within 2 weeks of life. SE was calculated wi

Investigating ticagrelor in a preclinical pipeline as a novel therapeutic to prevent preterm birth.

IN BRIEF: Preterm birth is the leading cause of perinatal morbidity and mortality, and new therapies that delay preterm birth and improve neonatal outcomes are urgently needed. This study investigates whether ticagrelor inhibits uterine contractility and inflammation in preclinical in vitro, ex vivo (human) and in vivo (mouse) studies, to explore the potential of repurposing ticagrelor for the prevention of preterm birth. ABSTRACT: Preterm birth remains a significant global health challenge, affecting approximately 10% of pregnancies and resulting in one million deaths globally every year. To
