A study on nesidioblastosis in hyperinsulinemic hypoglycemia--diagnosis, treatment, and neurologic sequelae.

The medical records of six cases of nesidioblastosis were examined to determine the diagnostic approach, treatment, and neurologic sequelae. All six patients were male, and their ages at the onset of the disease ranged from one day to six months (mean 3.36 +/- 2.5 mo.). Initial clinical features were seizure, cyanosis, poor feeding, and apnea. Other subsequent symptoms were developmental delay, hyperactivity, and cold sweating. The Birth weight of the neonatal onset group was heavier than the postneonatal onset group (4.4 +/- 0.3 vs 3.26 +/- 0.04 kg). Before the diagnosis of hyperinsulinism, s

Incontinentia pigmenti in identical twins with separate skin and neurological disorders.

Incontinentia pigmenti in female identical twins is reported. The first baby showed the typical pigmentation of incontinentia pigmenti, while the second baby had hydrocephalus (colpocephaly) without pigmentation. They were identical, with a rate of 99.9% in 18 blood-type studies. Virus was not detected and cytogenetic studies proved normal. Both showed peripheral eosinophilia. The individual expressions of Incontinentia pigmenti in these identical twins were separated into cutaneous lesions and lesion of the central nervous system (intra-uterine hydrocephalus). Cutaneous lesions developed afte

[Peridural analgesia in the combined treatment of neurologic syndromes in lumbar osteochondrosis].

The authors relate the results of the treatment of 151 patients with the remarkable painful syndrome of long standing associated with lumbar osteochondrosis. In 74 patients of the main group, peridural administration of the synthetic opiates dipidolor and phentanyl provided effective analgesia in the lack of sympathetic denervation, arterial hypotension and tissue deafferentation. The selective action of the synthetic morphine-like opiates did not reduce the activity of the endogenous antinociceptive structures or destroyed the reflex mechanisms of sanogenesis. The economic effect obtained as

Studies on the pathogenesis of neurological diseases associated with Varicella-Zoster virus.

Immunocytochemical techniques and in situ hybridization with three different Varicella-Zoster Virus (VZV)-specific RNA probes have been used to study the pathogenesis of VZV-associated neurological syndromes. Varicella-Zoster Virus antigens were not detected using the avidin-biotin peroxidase technique with a polyvalent anti-VZV antibody in any of the formalin-fixed tissue sections from eight cases of VZV-associated neurological disease (encephalitis, myelitis, ganglionitis); one case was immunosuppressed although inflammatory lesions were present. Intense labelling was detected within the inf

Central post-stroke pain--neurological symptoms and pain characteristics.

The intention of the present study was to characterize patients with central post-stroke pain (CPSP) with regard to type and location of the cerebrovascular lesion (CVL), the characteristics of the pain and the neurological symptoms and signs in addition to the pain. Twenty men and 7 women with a mean age of 67 years and a mean pain duration of 44 months were examined 9-188 (mean 53) months after their stroke. The clinical symptoms and signs and the CT scans indicated that the CVL were located in the lower brain-stem in 8 patients, involved the thalamus in 9 patients and were located lateral a

Dementia of depression among patients with neurological disorders and functional depression.

Cognitive performance on the Mini-Mental State Examination (MMSE) was assessed in depressed patients (diagnosis of major depression) with cerebrovascular lesions, with Parkinson's disease, or with functional depression (no known brain lesions). Controls for patients with brain lesions or Parkinson's disease were nondepressed patients with the same conditions. Controls for functionally depressed patients were age-matched normal individuals. Depressed patients had significantly lower total MMSE scores than their nondepressed counterparts, but depression did not have an effect on cognitive perfor

Prevalence of neurological disorders in Udo, a rural community in southern Nigeria.

The residents of Udo, a rural community of Edo-speaking people (population 2,925) in Bendel State, Nigeria were screened in a door-to-door survey for neurological disorders by specially instructed primary health care workers (PHW). A standard protocol was followed by PHW in collecting census data, administering a screening questionnaire, and performing a simple neurological examination. A neurologist confirmed the presence of neurological disorders in 235 (47%) of 504 screen-positive subjects after further clinical evaluation. The point prevalence ratios, per 1,000 population, for the most com

[Biotidinase deficiency: a disease with neurologic and cutaneous expression susceptible to biotin].

The authors report 2 familial cases of biotin deficiency. The first neurological signs appeared at the age of 2 years in a boy. The diagnosis was established in his sister in the neonatal period. A review of 41 published cases summarizes the neurologic signs (seizures, ataxia, hypotonia and later, developmental delay and deafness) and the cutaneous signs (rash, alopecia). An early treatment with biotin cures or prevents the clinical signs of the disease in most cases.

Neurological manifestations of infective endocarditis. Review of clinical and therapeutic challenges.

The nervous system is frequently involved in patients with infective endocarditis. When a careful review of presenting complaints is undertaken, neurological symptoms have been found in as high as 29% of patients. Because these manifestations may be so protean in nature, for example, stroke or transient ischaemic attack (the most common), toxic encephalopathy, meningitis, brain abscess, visual loss, seizures, headache, backache, or acute mononeuropathy, the neurologist needs to consider infective endocarditis as a possible diagnosis in many patients. During the past two decades, infective endo

[Syncopal consciousness disorders and drop attacks from the neurologic viewpoint].

The most important neurological disorders leading to syncope and/or drop attack are presented. With respect to epilepsy it is important to consider generalized absence seizures (petit mal), generalized tonic-clonic seizures (grand mal) and some types of complex partial seizures. Additionally, some sleep and arousal disorders must be mentioned, such as narcolepsy, disorders of excessive somnolence associated with sleep-induced respiratory impairment, as well as the Kleine-Levin-Critchley syndrome. Vagotonic, asympathicotonic, sympathicotonic and central autonomic disorders are comprised in the

Ultrasonographic features of carotid plaque and the risk of subsequent neurologic deficits.

In a prospective study, 214 consecutive patients considered not to be candidates for surgical intervention were evaluated by means of duplex scanning. Of the patients, 135 had no symptoms and 79 had a history of previous neurologic symptoms. In 139 sides duplex scanning demonstrated nonhemodynamic stenosis (lumen diameter reduction, less than 50%) and in 99 sides, hemodynamic stenosis (lumen diameter reduction, 50% or greater). Of the 238 carotid artery plaques examined, 167 were homogenous and 71 were heterogenous. During a mean follow-up of 34 months, 27 new focal neurologic deficits occurre

Neurological surgery during the Great War: the influence of Colonel Cushing.

Despite von Bergmann's work in the Franco-Prussian War and Makins' experiences in the Boer conflict, military surgeons in World War I were unprepared for the nature and extent of intracranial injuries. Poor triage, disorganized transportation, incomplete surgery, and sepsis resulted in a mortality of over 50%. In 1915, as a volunteer to the Ambulance Américaine near Paris, Harvey Cushing spent 5 weeks observing the Allied medical system. He quickly recognized the technical importance of early, definitive intracranial surgery and the logistical requirements for a unified triage system located a

Neurologic disease in biopsy-proven giant cell (temporal) arteritis.

Neurologic findings were studied in 166 consecutive patients with biopsy-proven giant cell (temporal) arteritis. Neurologic problems occurred in 51 patients (31%): neuropathies (23), TIA/strokes (12), neuro-otologic syndromes (11), tremor (6), neuropsychiatric syndromes (5), tongue numbness (3), and myelopathy (1). Neuro-ophthalmologic problems occurred in 35 patients (21%): amaurosis fugax (AF) (17), permanent vision loss (PVL) (14), scintillating scotoma (8), and diplopia (3). Abnormalities in large arteries in 52 patients (31%) included bruits and diminished pulses. The carotid artery was i

Correlation between history and urodynamics in neurologically normal incontinent women.

We compared the history of 100 consecutive women complaining of incontinence with their urodynamic findings. The real urodynamic situation proved unpredictable on the basis of the medical history. A good correlation between history and urodynamics was found in 80% of the women complaining of stress incontinence. The importance of a uroflowmetry to screen voiding difficulties in all patients is demonstrated. To get to a proper diagnosis in most women complaining of incontinence, technical urodynamic investigation seems mandatory.

Effect of sulfamethoxazole and trimethoprim on neurologic dysfunction in a patient with Joseph's disease.

Joseph's disease, an autosomal dominant, degenerative neurologic disease found in the Portuguese, is clinically manifested by extrapyramidal, pyramidal, and cerebellar dysfunction. A patient with the type II form of the disease reported improvement in gait and lessened spasticity while taking sulfamethoxazole and trimethoprim (Bactrim) for a noninfectious dysuria syndrome. A double-blind, placebo-controlled trial of Bactrim utilizing a subjective performance scale, physical examination, and six timed tests was performed in this patient. Results revealed lessened spasticity, improvement in walk

A neurologist looks at neurological disease temporally related to DTP immunization.

Pertussis vaccine was originally accused of provoking a short latency explosive encephalopathy with serious mental and physical consequences. Reports of recurrence of encephalopathy, worse after each dose, strengthened the notion of causality. Anecdotal associations can be no more than hypothesis-generating. With no distinctive clinical or pathological neurology, a major epidemiological study was necessary to answer the question "Does whooping cough vaccine cause brain damage in children"? The British national Childhood Encephalopathy Study (NCES) seemed to indicate that very rarely the answer…

[Bloch-Sulzberger incontinentia pigmenti with associated neurologic and ophthalmologic complications].

A female infant showing linear skin lesions arranged in an irregular way with bullae, vesicles, and erythema, which had predominantly affected her extremities since her birth, was examined by us at the age of one week. Histologic and immunofluorescence findings confirmed the preliminary diagnosis of incontinentia pigmenti. Electron microscopic investigations were additionally made. In the course of the disease, we observed the characteristic development of a warty and a pigmented stage. The disease was complicated by epileptic seizures, motor disturbances, and ocular defects.
