Impact of HIV Status on Fruit and Vegetable Consumption Among Older Adults in Tanzania: A Cross-Sectional Secondary Data Analysis.

Background/Objectives: It is well documented that people with human immunodeficiency virus (HIV) have nearly twice the risk of incident acute myocardial infarction compared to the general population. The elevated risk stems from a multi-layered interplay of factors such as persistent immune activation inherent to HIV infection and higher prevalence of traditional risk factors associated with nutritional needs. A large proportion of people living with HIV (PWH) reside in Sub-Saharan African countries such as Tanzania; however, there is a dearth of data on nutrition, particularly fruit and veget

Eating-timing patterns derived from K-means clustering and mortality risk in US adults with chronic diseases: a cohort study.

Whether joint eating-timing configurations show comparable mortality associations across chronic diseases is uncertain. We analysed 26 712 adults aged 20 years or older from the National Health and Nutrition Examination Survey (NHANES) 2005-2016 who had two reliable 24-h dietary recalls and mortality linkage. Participant-level mean first eating time and eating-window duration were standardised and classified by K-means. Survey-weighted Cox models incorporated strata and primary sampling units within six chronic disease domains; missing covariates were multiply imputed in thirty datasets and pr

Cavities and Calories: Exploring the Relationship Between Early Childhood Caries and Body Mass Index.

BACKGROUND: Early childhood dental health is a key determinant of overall health and well-being. Healthy dentition ensures proper mastication, clear speech, and acceptable appearance, all of which contribute to a child's nutrition, communication, and social development. In recent years, growing attention has been directed toward the association between dental caries and children's physical growth. Body mass index (BMI), widely recognized as an indicator of nutritional status and growth patterns, has been explored in this context. Aberrant BMI values, whether indicative of undernutrition or ove

Association of Admission Nutritional Status Evaluated by Prognostic Nutritional Index With One-Year Walking Independence After Primary Total Hip Arthroplasty for Osteonecrosis of the Femoral Head: A Retrospective Matched Cohort Study Based on 1,152 Patients.

BACKGROUND: Malnutrition is significantly associated with unfavorable outcomes, but the relevant prognostic assessment is not emphasized in patients who have osteonecrosis of the femoral head (ONFH). This study aimed to assess the association between preoperative nutritional status evaluated by the prognostic nutritional index (PNI) and walking independence at one year postoperatively in patients who have ONFH undergoing primary total hip arthroplasty (THA). METHODS: This was a retrospective analysis of prospectively collected data from patients admitted to a tertiary referral hospital who ha

Recovery Phase Nutrition and Insulin Strategies for a Collegiate Distance Runner with Type 1 Diabetes Mellitus: A Case Study.

PURPOSE: There is scant published research regarding nutrition and insulin strategies for athletic performance in collegiate distance runners with type 1 diabetes mellitus (CDRT1). Acute carbohydrate supplementation (CHOsup) and insulin reduction used to minimize hypoglycemia during exercise may result in deteriorated glycemic control post exercise in CDRT1. The present case study of a CDRT1 investigated outcomes associated with a moderate-carbohydrate (ModCHO) diet and 24 h insulin adjustment during recovery phases for improved glycemic control and reduced use of acute strategies. METHODS: D

Maternity Dietitians' Perspectives on Delivering Gestational Diabetes Care, and Postpartum Follow-Up in Ireland: A Qualitative Study.

BACKGROUND: Medical nutrition therapy is recognized as the cornerstone to optimal glycemic management of gestational diabetes (GDM) during pregnancy and registered dietitian nutritionists (RDNs) are central to delivery of this, yet data are lacking on their views of the barriers and enablers to performing this role. OBJECTIVE: This qualitative study aimed to explore the perspectives of RDNs providing medical nutrition therapy to people with GDM across the Republic of Ireland. DESIGN: Focus groups and semistructured interviews were conducted with RDNs from maternity settings whose roles inclu

Review of exclusive enteral therapy in adult Crohn's disease.

BACKGROUND: Exclusive enteral nutrition (EEN) is a potentially effective but underused therapy for Crohn's disease (CD) in adults. It is first-line induction treatment for paediatric patients but remains a second-line or third-line therapy in adults. OBJECTIVE: To analyse the evidence for EEN in adult patients with CD, and summarise this in a narrative review. METHODS: In April/May 2020 and July 2021, a literature search was performed using the Medical Subject Headings (MeSH) terms: 'Crohn's disease', 'CD', 'inflammatory bowel disease', 'IBD', 'exclusive enteral nutrition', 'enteral nutritio

Promoting mobility and healthy aging in men: a narrative review.

Maintaining mobility is an important aspect of health and well-being in older men. This literature review describes several modifiable and nonmodifiable risk factors impacting bone, muscle, and joint health. Exercise and nutritional interventions may help to prevent the progressive deterioration in bones, muscles, and joints impacting mobility in later life. Limitations in mobility are increasingly recognized as a major public health problem due to an aging population and growing number of older individuals affected by disabling comorbidities. Despite increasing numbers and debilitating conseq

Cow's Milk Consumption and Health: A Health Professional's Guide.

The most recent scientific evidence supports the consumption of cow's milk and dairy products as part of a balanced diet. However, these days, the public and practicing physicans are exposed to a stream of inconsistent (and often misleading) information regarding the relationship between cow's milk intake and health in the lay press and in the media. The purpose of this article, in this context, is to facilitate doctor-patient communication on this topic, providing physicians with a series of structured answers to frequently asked patient questions. The answers range from milk and milk-derived

Education and micronutrient deficiencies: an ecological study exploring interactions between women's schooling and children's micronutrient status.

BACKGROUND: Formal education can be a nutrition-sensitive intervention that supports the scale-up and impact of nutrition-specific actions. Maternal education has long been linked to child survival, growth, and development while adult earnings and nutrition are tied to years in school as a child. However, less is known about the relationship between maternal education and the micronutrient status of children, women and the general population. METHODS: Using country-level data and an ecological study design, we explored the global associations between women's educational attainment and: a) ane

Negotiating the complexities of exocrine and endocrine dysfunction in chronic pancreatitis.

Chronic pancreatitis is a chronic inflammatory disease of the pancreas characterised by irreversible morphological change and typically causing pain and/or permanent loss of function. This progressive, irreversible disease results in destruction of healthy pancreatic tissue and the development of fibrous scar tissue. Gradual loss of exocrine and endocrine function follows, along with clinical manifestations such as steatorrhoea, abdominal pain and diabetes. Nutrition in chronic pancreatitis has been described as a problem area and, until recently, there was little research on the topic. It is

Correlates of isoniazid preventive therapy failure in child household contacts with infectious tuberculosis in high burden settings in Nairobi, Kenya - a cohort study.

BACKGROUND: Sub-Saharan Africa continues to document high pediatric tuberculosis (TB) burden, especially among the urban poor. One recommended preventive strategy involves tracking and isoniazid preventive therapy (IPT) for children under 5 years in close contact with infectious TB. However, sub-optimal effectiveness has been documented in diverse settings. We conducted a study to elucidate correlates to IPT strategy failure in children below 5 years in high burden settings. METHODS: A prospective longitudinal cohort study was done in informal settlings in Nairobi, where children under 5 year

Comparison of Nutrition Profile and Diet Record Between Veteran and Nonveteran End-Stage Renal Disease Patients Receiving Hemodialysis in Veterans Affairs and Community Clinics in Metropolitan South-Central Texas.

BACKGROUND: U.S. military veterans have high rates of chronic disease and social disadvantage, which are risk factors for protein-energy wasting (PEW). It is not known whether this translates into high prevalence of PEW in veterans with end-stage renal disease. METHODS: We compared the clinical, socioeconomic, and nutrition status and the diet of 33 veteran and 38 nonveteran clinically stable patients receiving maintenance hemodialysis (MHD) in south-central Texas. RESULTS: The whole cohort included 82% Mexican Americans (MAs), 72% type 2 diabetics, and 73% males. The body mass index was 28.

Hay Fever and asthma as markers of atopic immune response and risk of colorectal cancer in three large cohort studies.

BACKGROUND: In a previous analysis of 12 cancers in the Cancer Prevention Study II (CPS-II) cohort including follow-up from 1982-2000, having both hay fever and asthma was associated with lower colorectal cancer mortality. The combination of these allergic conditions may be a marker for allergy-related immune responses that could inhibit colorectal carcinogenesis. METHODS: We examined the association of having both hay fever and asthma with colorectal cancer mortality among 1,023,191 participants in CPS-I, followed from 1959-1972, and 1,102,092 participants in CPS-II, now followed from 1982-2

Enteral feeding algorithm for infants with hypoplastic left heart syndrome poststage I palliation.

BACKGROUND: Infants with hypoplastic left heart syndrome (HLHS) experience a high incidence of growth failure in the postoperative period following stage I palliation. Because of an increased risk of necrotizing enterocolitis in this population, clinicians may be reluctant to initiate early enteral feedings. Published guidelines for initiating and advancing enteral feedings in this population are limited. OBJECTIVE: To test the safety and efficacy of an enteral feeding algorithm in infants with HLHS following stage I palliation. DESIGN: Single-center, prospective case series with historical

The vitamin rheostat: A unified model of epigenetic feedback in vitamin homeostasis.

Recent evidence implicates epigenetic mechanisms in vitamin metabolism, yet most studies remain focused on individual vitamins. In this mini-review, we extend beyond single-vitamin paradigms to propose the Vitamin Rheostat: a unifying model in which vitamins influence regulatory signaling networks that shape their own uptake, utilization, and degradation. By integrating fat-soluble vitamins (nuclear receptor-mediated signaling) and water-soluble vitamins (methyl-donor and enzymatic cofactor systems), this model provides a predictive and experimentally testable framework that may help explain i
